Pediatric trigger thumb: clinical management update.

PURPOSE OF REVIEW: Pediatric trigger thumb is a common problem in children accounting for 1 in 2000 visits to the pediatric hand clinic. Misdiagnosis as a fracture or dislocation is common, so proper identification is key. The ideal treatment strategy remains a subject of debate, and there are studies documenting successful outcomes with both nonoperative and surgical treatment. RECENT FINDINGS: Recent literature highlights that observation is reasonable, as spontaneous resolution can occur in 30-50% of cases over a several-year period, with those having an interphalangeal angle of less than

Thumb basal joint arthritis in 2023.

Thumb carpometacarpal or basal joint arthritis is the second most common location for osteoarthritis in the hand. It mainly affects women over 50years of age. Basal joint arthritis causes pain, loss of strength during pinch grips, and eventually stiffness and progressive deformity of the thumb column. Conservative treatment must be implemented first. It aims to spare the joint by using standardized methods. It must be initiated as soon as pain starts, not once the deformity has settled in. There is broad agreement that surgery is indicated when pain relief is not achieved after at least 6month

Trigger Thumb in Twins: Case Report.

Although trigger finger is common, pediatric trigger thumb is uncommon. In trigger thumb the finger is held in flexed position. The etiology of trigger finger is unknown and can occur isolated without any relation to other syndromes, however there are some evidences that suggest genetic etiology. We reported 2.5 year old twins both having bilateral trigger thumb. Grandfather of the twins had the disease. Although trigger thumb and finger have the same presentation, they can involve different anatomical structures. Bent or straightening of thumb or finger would produce painful popping and click

MR imaging findings of trigger thumb.

Trigger finger (or trigger thumb), also known as sclerosing tenosynovitis, is a common clinical diagnosis that rarely presents for imaging. Because of this selection bias, many radiologists may not be familiar with the process. Furthermore, patients who do present for imaging frequently have misleading examination indications. To our knowledge, magnetic resonance (MR) imaging findings of trigger thumb have not been previously reported in the literature. In this article, we review the entity of trigger thumb, the anatomy involved, and associated imaging findings, which include flexor pollicis l

Congenital thumb deformities and associated syndromes.

Upper limb defects occur in approximately 3.4 per 10,000 live births. Major thumb defects represent 16% of these upper limb defects (Tay SC, Moran SL, Shin AY, et al. The hypoplastic thumb. J Am Acad Orthop Surg 2006;14:354-366). Embryologically, hand development begins by the fifth week. This occurs simultaneously with the growth and development of the cardiovascular, neurologic, and hematopoietic systems. Therefore, congenital anomalies seen in the hands of infants may indicate significant anomalies in these other systems, requiring a comprehensive physical evaluation. Although the cause of

Thumb reconstruction by micro-vascular techniques.

Sixty-three patients with thumb reconstruction by total or partial toe transfer have been reviewed. Mean age was 25 years. Males (84%) and manual workers (76%) dominated the series. The rate of failure was 3%. Second toe transfer gave a functionally acceptable thumb with 10 mm two point discrimination, 59% of strength in pinching (compared to normal side), 30 degrees of range of flexion but with a flessum deformity (average 27 degrees) and a poor cosmesis score (1.5 on a 5 point scale). Partial toe transfers were useful in amputations at metacarpophalangeal (MP) level and distal to this area.

Cleft hand, syndactyly and hypoplastic thumb.

The clinical features of 58 patients with typical cleft hand were examined and compared with 86 patients with syndactyly between the long and ring fingers, 27 patients with syndactyly between the ring, little and other fingers, 53 patients with hypoplastic thumb and 100 patients with symbrachydactyly. The clinical findings of the typical cleft hand resembled those of syndactyly. There were two unusual cases of typical cleft hand associated with hypoplastic thumb, congenital heart anomalies and absence of the axial triradius. One of these also had cleft lip and palate. The critical embryonic pe

Thumb reconstruction using second-toe transfer.

Although toe transfer is a relatively new operation, its early results have been highly satisfactory. This article presents the author's experience from 1977 to 1983 with a total of 45 thumb reconstructions using the second toe. All but three of these patients have resumed employment.

Thumb Metacarpophalangeal Hyperextension in Patients Undergoing Basilar Thumb Joint Suspensionplasty: Are Preoperative Passive Measurements Predictive of Failure?

PURPOSE: To evaluate whether a 30° passive metacarpophalangeal (MCP) joint hyperextension threshold was associated with postoperative mechanical and patient-reported outcomes when leaving MCP joint hyperextension unaddressed after performing trapeziectomy with suspensionplasty. METHODS: This prospective observational study evaluated adults with symptomatic thumb carpometacarpal osteoarthritis and passive MCP joint hyperextension ≥10° who underwent trapezial resection and suspensionplasty without concurrent MCP joint stabilization. Patients were stratified by baseline passive MCP joint hyperex

Pediatric trigger thumb treated with A1 pulley release with or without postoperative thumb immobilization: a retrospective study.

BACKGROUND: This study aimed to compare clinical outcomes in patients with trigger thumb who underwent A1 pulley release, with or without postoperative thumb immobilization. METHODS: We retrospectively analyzed 207 patients with trigger thumb, comprising 97 in the splinted group and 110 in the non-splinted group, who underwent surgical treatment at our center. Recurrence rates, differences in extension angles between affected and unaffected interphalangeal (IP) joints, and postoperative complications were compared between the two groups. RESULTS: According to Baek's criteria, all patients ex

The added value of measuring thumb and finger strength when comparing strength measurements in hypoplastic thumb patients.

BACKGROUND: When interventions to the hand are aimed at improving function of specific fingers or the thumb, the RIHM (Rotterdam Intrinsic Hand Myometer) is a validated tool and offers more detailed information to assess strength of the involved joints besides grip and pinch measurements. METHODS: In this study, strength was measured in 65 thumbs in 40 patients diagnosed with thumb hypoplasia. These 65 thumbs were classified according to Blauth. Longitudinal radial deficiencies were also classified. The strength measurements comprised of grip, tip, tripod and key pinch. Furthermore palmar abd

Treatment of a type 3B hypoplastic thumb using extra phalanges from the contralateral duplicated thumb: case report.

A boy was born with a right duplicated thumb (Wassel type 6) and a left radial club hand (type 3) associated with a hypoplastic thumb (type 3B). He underwent surgical centralization of the left wrist when he was 13 months old. At age 38 months, he underwent reconstruction of the carpometacarpal joint of the hypoplastic left thumb. This procedure involved transplantation of the radial ray of the right duplicated thumb to the base of the left thumb. When he was 6 years old, the patient underwent an abductor digiti minimi muscle transfer to create opposition for the left thumb and deepening of th

Surgery for thumb (trapeziometacarpal joint) osteoarthritis.

RATIONALE: Surgery is used to treat persistent pain and dysfunction due to osteoarthritis at the base of the thumb, when conservative management is no longer effective. This is an update of a Cochrane review first published in 2005. OBJECTIVES: To evaluate the benefits and harms of different surgeries for people with thumb (trapeziometacarpal) osteoarthritis. SEARCH METHODS: We searched CENTRAL, MEDLINE and Embase up to 19 May 2025. ELIGIBILITY CRITERIA: We included randomised controlled trials (RCTs) or quasi-RCTs that compared at least two surgical techniques for trapeziometacarpal osteoa

Open Release of Pediatric Trigger Thumb.

UNLABELLED: Open release of a trigger thumb has been shown to be the most reliable option to restore full interphalangeal (IP) joint extension and thus normal thumb-joint motion in children1. The aim of this procedure is to restore free gliding of the flexor pollicis longus (FPL) tendon in its canal in children with fixed IP joint flexion contractures or those in whom nonoperative treatment modalities have failed. DESCRIPTION: The surgical procedure is easy to perform and straightforward; however, attention must be given to several details in order to avoid surgical failure and complications.

Triphalangeal thumb: clinical features and treatment.

Triphalangeal thumb is a rare congenital anomaly in which the thumb has three phalanges. Clinical presentation of triphalangeal thumb can vary considerably and can be present in both hands or unilateral. The thumb can be long with a finger-like appearance. The presence of clinodactyly depends on the shape of the extra phalanx varying from wedge-shaped to rectangular. Various joints, ligaments, muscles, and tendons of the first ray can be hypoplastic or absent, with varying degrees of stiffness or instability. The aim of surgical treatment is to reconstruct or correct the anatomic anomalies to ob
