Implementation of a Transitional Care Model for Stroke: Perspectives From Frontline Clinicians, Administrators, and COMPASS-TC Implementation Staff.

BACKGROUND AND OBJECTIVES: Stroke is a chronic, complex condition that disproportionally affects older adults. Health systems are evaluating innovative transitional care (TC) models to improve outcomes in these patients. The Comprehensive Post-Acute Stroke Services (COMPASS) Study, a large cluster-randomized pragmatic trial, tested a TC model for patients with stroke or transient ischemic attack discharged home from the hospital. The implementation of COMPASS-TC in complex real-world settings was evaluated to identify successes and challenges with integration into the clinical workflow. RESEA

Association between transitional care factors and hospital readmission after transcatheter aortic valve replacement: a retrospective observational cohort study.

BACKGROUND: Studies have shown that patients who undergo trans-catheter aortic valve replacement (TAVR) have high rates of hospital readmission. Our objectives were to identify the causes of readmission after TAVR, determine whether transitional care factors were associated with a reduction in readmission and to identify other predictors that could be used to target quality improvement efforts. METHODS: We conducted a chart abstraction study that included all patients who underwent TAVR in Ontario, Canada between 2007 and 2013 and survived to hospital discharge. These data were linked to prov

Effects of a transitional palliative care model on patients with end-stage heart failure: a randomised controlled trial.

OBJECTIVE: To examine the effects of home-based transitional palliative care for patients with end-stage heart failure (ESHF) after hospital discharge. METHODS: This was a randomised controlled trial conducted in three hospitals in Hong Kong. The recruited subjects were patients with ESHF who had been discharged home from hospitals and referred for palliative service, and who met the specified inclusion criteria. The interventions consisted of weekly home visits/telephone calls in the first 4 weeks then monthly follow-up, provided by a nurse case manager supported by a multidisciplinary team.

Retention in Rheumatology Care Across the Juvenile Idiopathic Arthritis Care Pathway: from Diagnosis Through Transition.

OBJECTIVE: Juvenile idiopathic arthritis (JIA) is a heterogeneous group of childhood-onset arthritides, many of which require sustained pediatric rheumatology care and transition to adult rheumatology care. We examined longitudinal retention in rheumatology care from diagnosis through adulthood. METHODS: We conducted a population-based longitudinal study of an inception cohort of children/adolescents diagnosed with JIA between 2010 and 2018 and followed until 2024. For each follow-up year, we determined whether patients were engaged in care (at least one pediatric rheumatologist visit), lost

Association between palliative care consultation and care transition outcomes among hospitalized advanced cancer patients.

BACKGROUND: The period after hospital discharge is a high-risk phase for patients with advanced cancer, often involving acute-care use that reflects transitional care quality. The impact of inpatient palliative care (PC) consultation on short-term post-discharge outcomes, however, remains uncertain. We assessed whether inpatient PC consultation was associated with differences in 30-day post-discharge outcomes. METHODS: Using electronic medical records from a tertiary hospital linked with national claims data, we identified patients with lung, stomach, colorectal, liver, or pancreatobiliary ca

Newly Escalated Psychotropic Medications in the Trauma Intensive Care Unit: Prescribing Patterns During Care Transitions.

BACKGROUND: Previous retrospective studies involving mixed intensive care unit (ICU) populations found antipsychotic continuation rates ranging from 21% to 61% at hospital discharge. Surgical ICU admission was cited as a risk factor for antipsychotic continuation, although data in the trauma population are limited. OBJECTIVE: The purpose of this study was to evaluate the number of critically ill trauma patients with escalated psychotropic medications in the surgical ICU and the rate of continuation at ICU transfer and hospital discharge. METHODS: This single-center, retrospective, Institutio

Emergency department care transition barriers: A qualitative study of care partners of older adults with cognitive impairment.

INTRODUCTION: After emergency department (ED) discharge, persons living with cognitive impairment (PLWCI) and their care partners are particularly at risk for adverse outcomes. We sought to identify the barriers experienced by care partners of PLWCI during ED discharge care transitions. METHODS: We conducted a qualitative study of 25 care partners of PLWCI discharged from four EDs. We used the validated 4AT and care partner-completed AD8 screening tools, respectively, to exclude care partners of older adults with concern for delirium and include care partners of older adults with cognitive im

Association of Primary Care Continuity With Outcomes Following Transition to Adult Care for Adolescents With Severe Mental Illness.

IMPORTANCE: Adolescents with severe mental illness often age out of pediatric care without a clear transfer of care to adult services. The extent to which primary care provides stability during this vulnerable transition period is not known. OBJECTIVE: To analyze the association between primary care continuity during the transition from pediatric to adult care and need for acute mental health services in young adulthood. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study used linked health and demographic administrative data for all adolescents aged 12 to 16 years with sev

Medication Safety During Transitions of Care From Paramedicine to Emergency Nursing Care: A Scoping Review Protocol.

INTRODUCTION: Transitions of care in the emergency department are critical points for patient safety, particularly during handovers from emergency medical services (EMS) personnel to emergency department clinicians. This rapid and high-pressure environment often prioritizes urgent clinical care over structured communication, increasing the risk of information loss and medication errors, which can lead to serious patient harm, prolonged hospital stays, preventable complications, and death. OBJECTIVE: To systematically map academic and gray literature examining medication-related safety risks d

Exploring Patient and Caregiver Work and Continuity of Care During Hospital-to-Home Transitions of Care in the Canadian Context.

BACKGROUND: Patients and caregivers are expected to engage in work that involves both physical and cognitive tasks to manage their care and well-being during hospital-to-home transitions. Patient and caregiver work is defined as the health-related tasks they must manage, as well as the physical and cognitive effort required to complete those tasks. When continuity of care is well-supported, work for patients and caregivers may be reduced or better managed. However, there is a need to better understand how continuity of care affects patients' and caregivers' work to optimise their outcomes. OB

Pharmacist's Support in the Transition From Hospital to Home Care in Patients With Multiple Medications: Avoidance of Fragmentation of Care.

Transitions from hospital to home care are high-risk periods, often leading to medication errors and care fragmentation. This case report highlights the essential role of pharmacists in ensuring safe medication transitions and continuity of care, particularly through the use of the simple suspension method (SSM), a technique that simplifies the administration of medication for patients with dysphagia. The case involves a woman in her 80s with multiple chronic conditions and swallowing difficulties, hospitalized for pneumonia. During her hospital stay, a pharmacist introduced and monitored SSM,

Nurses' experiences in transitioning to work in the long-term care and home and community care sectors: A phenomenological exploration.

BACKGROUND: Nurses in long-term care (LTC) and home and community care (HCC) are well acquainted with the high workloads and manifold job stressors that pervade these sectors, making it increasingly difficult to employ and retain a strong workforce. In response, nursing bridging education programs have been proposed to enhance recruitment and retention. While the efficacy of such programs is supported, the transition to practice experience of new nurse graduates is often associated with feelings of under-preparedness. Understanding the lived experience of nurses transitioning into new roles in

Health Care Transition From Pediatric- to Adult-Focused Care in X-linked Hypophosphatemia: Expert Consensus.

CONTEXT: X-linked hypophosphatemia (XLH) is an inherited skeletal disorder that can lead to lifelong deleterious musculoskeletal and functional consequences. Although often perceived as a childhood condition, children and adults both experience the negative effects of XLH. Adolescents and young adults (AYAs) benefit from effective health care transition (HCT) preparation to support the transfer from pediatric- to adult-focused care. Whereas transition timelines, milestones, and educational tools exist for some chronic conditions, they do not meet the unique needs of patients with XLH. EVIDENC
