Trauma activation patients: evidence for routine alcohol and illicit drug screening.

BACKGROUND: Statistics from the National Trauma Data Bank imply that discretionary blood alcohol and urine drug testing is common. However, there is little evidence to determine which patients are appropriate for routine testing, based on information available at trauma center arrival. In 2002, Langdorf reported alcohol and illicit drug rates in Trauma Activation Patients. METHODOLOGY/PRINCIPAL FINDINGS: This is a retrospective investigation of alcohol and illicit drug rates in consecutive St. Elizabeth Health Center (SEHC) trauma patients. SEHC Trauma Activation Patients are compared with th

Risk factors for mortality in head-injured patients with probability of survival greater than 0.5.

BACKGROUND: Traumatic brain injury is one of the major problems and leading cause of death worldwide. The present study was aimed to identify factors responsible for mortality by comparing survivors and nonsurvivors in patients that had a low probability of mortality. MATERIAL AND METHOD: A nested case-control study was conducted at Sawanpracharak Hospital from January 1, 2007 to December 31, 2007. All head injury deaths that had TRISS-PS greater than 0.5 were enrolled as the "case" patients. Head injured patients with TRISS-PS greater than 0.5 and that survived were chosen as the "control" p

Predicting risk-adjusted mortality for trauma patients: logistic versus multilevel logistic models.

BACKGROUND: Theoretical advantages of random-intercept multilevel (ML) logistic regression (LR) modeling over standard LR include separating variability due to patient-level and hospital-level predictors, shrinkage of estimates for lower-volume hospitals toward the overall mean, and fewer hospitals falsely identified as outliers. STUDY DESIGN: We used Nationwide Inpatient Sample data from 2002 to 2004 to construct LR models of hospital mortality after admission with a principal ICD-9 Clinical Modification injury diagnosis (ICD-9 Clinical Modification codes 800 to 904, 910 to 929, 940 to 957,

[Efficacy of spiral computed tomography in diagnosis of thorax injuries due to combined trauma].

Spiral computed tomography of the thorax was used in 112 patients with mechanical trauma. High efficacy of this method in diagnosis of fracture of posterior and lateral costal parts (69.6%), pneumo- and hydrothorax (100%), traumatic infiltration and gas-filled cysts of the lung (100%), pulmonary hematoma (90%) was demonstrated. Localization of bone fragments in pleural cavity (100%) had special significance. It is demonstrated that the method has low efficacy in the diagnosis of injuries of anterior costal part and sternum (49.3 and 20%). Indications for this method were formulated. The result

Multiple laparotomies are a predictor of fascial dehiscence in the setting of severe trauma.

The purpose of this study was to determine the incidence of wound dehiscence after repeat trauma laparotomy. We performed a retrospective analysis of adult trauma patients who underwent laparotomy at an urban level 1 trauma center during the past 5 years. Patients were divided into single (SL) and multiple laparotomy (ML) groups. Demographic, clinical, and outcome data were collected. Data were analyzed using chi2, t testing, and ANOVA. Overall dehiscence rate was 0.7 per cent. Multiple laparotomy patients had damage control, staged management of their injuries, or abdominal compartment syndro

Pericardial tamponade type injury: a 17-year study in an urban trauma center in Japan.

We defined injuries to the heart and the pericardium together with hemopericardium as pericardial tamponade type injury regardless of symptoms or signs due to pericardial tamponade. The aim of the study is to examine the important factors related to the diagnosis and treatment of this type of injury. A retrospective chart review was conducted of traumatized patients admitted with hemopericardium to our institution between 1978 and 1995. Ten out of the 19 consecutive patients with pericardial tamponade type injury demonstrated shock and showed a higher Injury Severity Score and mortality (7/11)

The role of the Revised Trauma Score in penetrating trauma.

INTRODUCTION: The Revised Trauma Score (RTS) is one of the primary tools used to predict mortality in trauma patients. RTS is calculated at admission based on respiratory rate, systolic blood pressure, and Glasgow Coma Scale score. Its accuracy has been evaluated in several studies; however, most have predominantly included victims of blunt trauma, making its applicability to penetrating injuries a matter of ongoing debate. OBJECTIVE: To compare the prognostic accuracy of the RTS for mortality prediction in patients with blunt trauma versus penetrating trauma. METHODS: This retrospective coh

Trauma or growth after a natural disaster? The mediating role of rumination processes.

The aim of this study was to test a cognitive model of posttraumatic symptoms (PTS) and posttraumatic growth (PTG) after exposure to a natural disaster. It was hypothesized that although subjective severity of trauma would be related to the severity of PTS, this relation would be mediated by brooding and cognitive strategies related to the presence of repetitive negative content in thoughts. Furthermore, the relation between severity and PTG would be fully mediated by deliberate rumination (DR), cognitive strategies related to conscious efforts focused on handling the event. To evaluate the co

An exploration of e-scooter injuries and severity: Impact of restriction policies in Helsinki, Finland.

INTRODUCTION: The emergence of shared electric scooter (e-scooter) services has introduced a new mobility option in numerous urban areas worldwide. Safety concerns surrounding e-scooter riding have prompted some cities to impose bans or restrictions on shared e-scooters. This study aims to assess the impact of e-scooter restriction policies, on the spatiotemporal distribution of e-scooter injuries and factors influencing injury severity in Helsinki, Finland, in 2021 and 2022. These restrictions include banning shared e-scooter use from midnight to 5 a.m. on weekends and reducing speeds during

What happened matters: Trauma type and cumulative trauma exposure in refugee youth psychopathology.

OBJECTIVE: Trauma exposure-a contributor to psychological risk for refugee youth-is typically assessed using cumulative indices; however, recent findings indicate that trauma type may better predict psychological outcomes. This study investigated the utility of two methods of classifying trauma exposure-cumulative trauma and exposure to specific types of trauma (i.e., trauma subtypes)-in predicting the severity of symptoms related to posttraumatic stress disorder (PTSD) and anxiety for refugee youth. METHOD: 96 Syrian and Iraqi youth resettled as refugees in the United States self-reported tr

Tracheal intubation in traumatic brain injury: a multicentre prospective observational study.

BACKGROUND: We aimed to study the associations between pre- and in-hospital tracheal intubation and outcomes in traumatic brain injury (TBI), and whether the association varied according to injury severity. METHODS: Data from the international prospective pan-European cohort study, Collaborative European NeuroTrauma Effectiveness Research for TBI (CENTER-TBI), were used (n=4509). For prehospital intubation, we excluded self-presenters. For in-hospital intubation, patients whose tracheas were intubated on-scene were excluded. The association between intubation and outcome was analysed with ord

Injury from falls in infants under one year.

AIM: Falls in infants are a common cause of injury. Compared to older children, infants under age one are likely to have distinctive causation and injury patterns, as they are pre-mobile or have limited independent mobility and falls are more directly the responsibility of the care giver. There is little known about the mechanistic factors, predictors of injury and injury patterns in this age group. METHODS: We conducted a retrospective review of infants under age one who presented after a fall to a paediatric trauma centre in Sydney, Australia. Circumstances and mechanisms of the fall, injur
