Primary tuberculosis of the chest wall is a rare disease and very difficult to diagnose without clinical suspicion. Here, we present an unusual case of necrotizing fasciitis due to an aggressive form of chest wall tuberculosis. A 22-year-old male presented in emergency with acute-onset swelling and redness over the right side of the neck and chest wall. He had no history of any drug reaction, trauma, and unknown bite. The patient underwent aggressive debridement followed by split-thickness graft under intensive care monitoring. Radiological imaging and Ziehl-Neelsen (ZN) staining of pleural fl

Tuberculosis remains a public health issue worldwide. Identifying its risk factors, such as vitamin D deficiency, is mandatory so as to target the preventive strategies. We aimed to study the association between vitamin D deficiency and extrapulmonary tuberculosis. We conducted a case-control study including all cases of extrapulmonary tuberculosis hospitalized in the infectious diseases department over a two-year period from April 2017 until April 2019. We included 45 cases of extrapulmonary tuberculosis and 45 controls matched by gender and age. Vitamin D deficiency was significantly more fr

A 33-year-old gravid female from Pakistan presented to the Emergency Department with persistent, intractable low back pain and neuropathic left L5 leg pain, associated with a left foot drop. There was a notable history of weight loss for 1 year. Investigations revealed a large collection in the right posterior paraspinal muscles tracking from a large bony defect in the right half of her sacrum extending into the pelvis. The collection was suggestive of an abscess and underwent US-guided aspiration. Culture, PCR examination, and bone biopsy were culture-negative for tuberculosis (TB). Samples t

Hemophagocytic lymphohistiocytosis (HLH) is a severe hyperinflammatory syndrome caused by unregulated immune activation, leading to a "cytokine storm" and multisystem failure. In people living with HIV (PLWH), it is frequently triggered by opportunistic infections like Mycobacterium tuberculosis. This report describes a 28-year-old transgender woman with HIV (on ART, undetectable viral load) admitted with a 20-day history of high fever, diarrhea, and hepatosplenomegaly. Admission labs showed severe pancytopenia (Hb 6.2 g/dL, WBC 1,590/µL, platelets 102,000/µL) and a CD4 count of 164 cells/mm³.

We report a case of disseminated tuberculosis (TB) manifesting as a soft tissue mass of the back associated with pain and difficulty walking. Its clinical presentation poses diagnostic challenges and can sometimes mimic neoplasms. The first imaging study with computed tomography was suggestive of metastatic bone disease or locally advanced malignancy. Advanced imaging studies, such as magnetic resonance imaging (MRI) or computed tomography (CT), are required to evaluate the extent of organ involvement. A definitive diagnosis necessitates histopathological analysis and microbiological confirmat

BACKGROUND: Bone marrow tuberculosis (TB) is a rare but severe form of extrapulmonary TB, often presenting with nonspecific symptoms such as fatigue, weakness, and cytopenia, making diagnosis challenging.
CASE PRESENTATION: We report the case of a 71-year-old male with a history of pituitary surgery and chronic corticosteroid use, who presented with pancytopenia, hyponatremia, and a chronic productive cough. Initial investigations, including chest CT, abdominal and pelvic ultrasound, liver elastography, dynamic liver MRI, blood cultures (from peripheral blood), and sputum cultures (from expec

We report the case of a 41-year-old woman who initially presented with headache, right hemifacial hypoesthesia, and a right abducens palsy. Initial external investigations failed to identify an etiology, and cerebral MRI was considered normal, although later review revealed subtle meningeal thickening. Approximately two months later, she developed a right pupil-involving oculomotor nerve palsy, followed by progressive visual loss in the right eye, a right inferior quadrantanopia, and a left abducens palsy. Repeat MRI demonstrated a new contrast-enhancing lesion in the right cavernous sinus, va

Tuberculosis (TB) is a leading cause of mortality attributed to an infectious agent. TB primarily targets the lungs, but in about 16% cases can affect other organs as well, giving rise to extrapulmonary TB (EPTB). However, an optimal regimen for EPTB treatment is not defined. Although the recommended treatment for most forms of EPTB is the same as pulmonary TB, the pharmacokinetics of EPTB therapy are not as well studied. To address this gap, we formulate a whole-body physiologically-based pharmacokinetic (PBPK) model for EPTB that for the first time includes the ability to simulate drug conce

BACKGROUND: Extrapulmonary Tuberculosis (EPTB) accounts for 15%-53% of all TB cases. In recent years, cartridge-based nucleic acid amplification test (CBNAAT) has emerged as an important diagnostic tool since the diagnostic yield is higher. We conducted this study to evaluate the diagnostic yield of CBNAAT in EPTB.
METHODS: One hundred and four patients with EPTB were included in the study. Samples were subjected to CBNAAT, AFB smear, culture for Mycobacterium tuberculosis and histopathology examination (HPE). Yield of each was estimated as compared to a composite reference standard (CRS).

BACKGROUND: Improved understanding of the epidemiology and mortality risk factors of extrapulmonary tuberculosis (EPTB) may facilitate successful diagnosis and management.
METHODS: We analyzed national surveillance data from Ukraine to characterize EPTB subtypes (ie, localized in different anatomic sites). We calculated annual reported incidence, stratified by age, sex, and human immunodeficiency virus (HIV) status. Using Cox regression, we estimated mortality risk factors.
RESULTS: Between January 2015 and November 2018, 14 062 adults/adolescents (≥15 years) and 417 children (<15 years) had

We aim to highlight the ignorance and incidences of deliberate hiding of medical facts in society. The cause of it can be, the associated taboo with these diseases. The fear of social outcasts is the major barrier preventing diseased from accepting the diagnosis as well as treatment. Though the medical facility has improved significantly and has come up with a complete cure for these diseases, still these facilities are not able to trickle down to the lower socioeconomic group. The reason for facilities not reaching the lower socioeconomic strata is the self-made shield created by these people

Extrapulmonary tuberculosis (TB) is a well-recognized cause of pyrexia of unknown origin. However, clinical presentation of TB in children with isolated hematological abnormalities is extremely rare. Anemia, usually normocytic, normochromic, leukopenia, leukocytosis, thrombocytopenia, thrombocytosis, and monocytosis are more common complications of TB rather than pancytopenia. Only anecdotal case reports and small case series are available in this regard. We are reporting an 18-year-old boy who presented with on and off low-grade fever for 3 months and anorexia and progressive pallor for 1 mon

INTRODUCTION: this work aims to bring out the epidemiological characteristics of extrapulmonary tuberculosis (EPTB) in the province of Larache (Morocco) and to investigate the effect of gender and age on its localization and treatment outcome.
METHODS: it consists in a retrospective study based on 2962 cases of EPTB, reported during the period 2000 to 2012.
RESULTS: the mean age was 31.74 ± 18.83 years, with a median age of 26. Males are more affected by this form of tuberculosis, with a male to female sex-ratio of 1,15. The EPTB affects particularly the young population whose age is between

Strongyloides stercoralis infection is a common and neglected public health problem in many areas of the world. Here we report the case of a 21-year-old woman who emigrated from Cambodia to Japan and presented with swelling of the right supraclavicular region of 2 months' duration. She had no other symptoms. Tuberculous lymphadenitis was diagnosed based on a fine-needle aspiration biopsy of the right supraclavicular lymph node. The laboratory examination revealed mild eosinophilia (eosinophils 1348/µL).S. stercoralis and Hymenolepis nana were detected serologically and in faeces examination. T

BACKGROUND: Fine needle aspiration cytology (FNAC) is a precise, cost-effective, and minimal invasive technique for diagnosis of extrapulmonary tuberculosis (EPTB). This study was aimed to analyze clinical and cytological characteristics of EPTB with particular emphasis on pathogenesis.
METHODS: This was a retrospective study analyzing 237 cases of EPTB diagnosed on FNAC. Depending on the presence of necrosis and granulomas, cases were divided into three classes. Association of cytomorphology with positivity for acid-fast bacilli was analyzed by Chi-square test to demonstrate any specific pat

Limited data are available on which factors are associated with strong immunologic responses to T-SPOT.TB. We investigated the factors associated with strong positive responses in patients with extrapulmonary tuberculosis (E-TB). Of 173 patients with E-TB who gave positive results on T-SPOT.TB, 26 (15%) with a strong positive response (defined as ≥1,000 spot-forming units (SFU)/2.5×10(5) PBMC to ESAT-6 or CFP-10) and 71 (41%) with a low positive response (≤ 99 SFU (6-99 SFU)/2.5×10(5) PBMC) were further analyzed. Miliary TB was independently associated with a strong positive response to T-SPOT

Expansion of cerebral tuberculomas or their new appearance as a manifestation of paradoxical reaction in patients under antituberculous chemotherapy is well documented. Distinguishing paradoxical reaction from disease progression or treatment failure is an important issue in tuberculosis management. Five cases of cerebral tuberculomas are reported here as manifestations of paradoxical reaction in patients with pulmonary and extrapulmonary tuberculosis on antituberculous treatment. Case 1 and 2 had tuberculous meningitis, Case 3 had miliary tuberculosis, Case 4 had miliary tuberculosis and dest
