Acute negative-pressure hydrocephalus: Management algorithm and value of early endoscopic ventriculostomy.

INTRODUCTION: Acute negative-pressure hydrocephalus is an uncommon, underrecognized patology with a high morbidity and mortality. We propose an algorithm to facilitate the management of these patients, promoting the early diagnosis and the use of endoscopic third ventriculostomy as initial therapeutic option. METHODS: We performed an observational retrospective study in which patients diagnosed with acute negative-pressure hydrocephalus were included. Patient age and symptoms, primary etiology of hydrocephalus, previous shunt, infection and surgical procedures, time from clinical deterioratio

Usefulness of postoperative ventriculography and intracranial pressure monitoring following endoscopic third ventriculostomy.

PURPOSE: The authors sought to determine whether the insertion of an external ventricular drain (EVD) at the time of surgery to monitor intracranial pressure (ICP) and ventriculography done within the first day following an endoscopic third ventriculostomy (ETV) is of benefit in postoperative patient management. METHODS: Following IRB approval, ETV procedures done by the senior author between January 1, 2007, and December 31, 2016, were reviewed. Included in a consecutive fashion were all patients who underwent an ETV with placement of an external ventricular drain (EVD) that was preceded pre

Endoscopic third ventriculostomy with choroid plexus cauterization outcome: distinguishing success from failure.

OBJECTIVE Endoscopic third ventriculostomy with choroid plexus cauterization (ETV/CPC) offers an alternative to shunt treatment for infantile hydrocephalus. Diagnosing treatment failure is dependent on infantile hydrocephalus metrics, including head circumference, fontanel quality, and ventricle size. However, it is not clear to what degree these metrics should be expected to change after ETV/CPC. Using these clinical metrics, the authors present and analyze the decision making in cases of ETV/CPC failure. METHODS Infantile hydrocephalus metrics, including bulging fontanel, head circumference

Endoscopic third ventriculostomy: preoperative considerations and intraoperative strategy based on 300 procedures.

PURPOSE: Although successful endoscopic third ventriculostomy (ETV) has been reported for many indications, peculiarities of the surgical technique in each separate indication require particular respect. METHODS: A detailed account of the authors' surgical technique, their presurgical considerations, and their intraoperative strategies to perform ETV is presented. Surgery of representative obstructive hydrocephalus cases in posterior fossa lesions (cerebellar infarction, posterior fossa tumor), in distortion of the ventricular system (intracranial hemorrhage, basilar artery aneurysm) and in m

Severe parkinsonism following endoscopic third ventriculostomy for non-communicating hydrocephalus--case report.

A 23-year-old male with non-communicating hydrocephalus caused by intraventricular hemorrhage from an arteriovenous malformation was treated by endoscopic third ventriculostomy (ETV) after repeated ventricular cerebrospinal fluid drainage and removal of ventriculoperitoneal shunt due to infection. Postoperatively, the patient developed robust rigidity and akinesia with mutism, suggesting secondary parkinsonism. These symptoms showed marked improvement after administration of levodopa, and were not aggravated after discontinuation. The pathophysiology of this case might involve dysfunction of t

Endoscopic third ventriculostomy in idiopathic normal pressure hydrocephalus: an Italian multicenter study.

OBJECTIVES: The aim of the report is to define the indications and results of endoscopic third ventriculostomy (ETV) in idiopathic normal pressure hydrocephalus and to discuss the physiopathological mechanism of this procedure. METHODS: The cases of 110 patients with idiopathic normal pressure hydrocephalus who underwent ETV in four Italian neurosurgical centers were retrospectively reviewed. The postoperative outcome was correlated with patient age, length of clinical history, preoperative clinical score, symptoms of clinical onset, type of hydrocephalus, and intraoperative findings. Selective endovascular treatment of a traumatic basilar aneurysm after endoscopic third ventriculostomy.

A 4-year-old girl suffered intraventricular and subarachnoid hemorrhage during endoscopic third ventriculostomy. Cerebral angiography revealed a traumatic basilar aneurysm secondary to basilar artery injury. The aneurysm was treated with selective endovascular embolization using Guglielmi detachable coils. We review some therapeutic features of traumatic basilar aneurysms after endoscopic third ventriculostomy and describe the feasibility of endovascular selective therapy to manage these lesions successfully.

Endoscopic ventriculostomy versus shunt operation in normal pressure hydrocephalus: diagnostics and indication.

In contrast to the shunt operation the indication for an endoscopic ventriculostomy in patients diagnosed for normal pressure hydrocephalus is not scientifically established. Between September 1997 and December 1999 we operated on 48 patients diagnosed for normal pressure hydrocephalus. The diagnosis was established by means of the intrathecal lumbar or ventricular infusion test, the cerebrospinal fluid tap test and MRI-CSF flow studies pre- and postoperatively. In 37 patients (77%) we have implanted a ventriculo-peritoneal shunt, and in 11 patients (23%) we performed the endoscopic assisted t

Functional analysis of third ventriculostomy patency with phase-contrast MRI velocity measurements.

Our purpose was to explore the utility of cine phase-contrast MRI velocity measurements in determining the functional status of third ventriculostomies, and to correlate the quantitative velocity data with clinical follow-up. We examined six patients with third ventriculostomies and 12 normal subjects by phase-contrast MRI. The maximum craniocaudal to maximum caudocranial velocity range was measured at regions of interest near the third ventricular floor, and in cerebrospinal fluid anterior to the upper pons and spinal cord on midline sagittal images. Ratios of the velocities of both the third

A systematic review of reviews on ventriculostomy related infection definitions: A fundamental problem.

Despite being a common, well-recognized and important complication to External Ventricular Drainage (EVD), a consensus definition for Ventriculostomy Related Infections (VRI) has not yet been established. We conducted a review to qualitatively assess definition heterogeneity and objectivity among Randomized Controlled Trials (RCTs); and investigated systematic reviews, meta-analyses, and reviews of the literature for definition citation accuracy and common methodological approaches and points of discussion related to VRI definitions. RCTs were grouped into arbitrarily chosen infection rate bra

Surgical Anatomy of the Third Ventricle Floor: Implications for Endoscopic Third Ventriculostomy.

Neuroendoscopy has made a breakthrough in the field of neurosurgery. Since its first application at the start of the 20th century, it has evolved enormously and become more specialized. Endoscopic Third Ventriculostomy (ETV) is a type of ventriculoscopies where the floor of the third ventricle is opened to divert the CSF from the third ventricle to the subarachnoid space. Nowadays, ETV is the gold-standard treatment for selected cases of hydrocephalus. Its success depends on, among others, the anatomy of the floor of third ventricle. From anterior to posterior, structures related to the floor

Combined Predictive Model for Endoscopic Third Ventriculostomy Success in Adults and Children.

BACKGROUND: The selection of patients in whom endoscopic third ventriculostomy (ETV) can be effective remains poorly defined. The ETV success score (ETVSS) and the presence of bowing of the third ventricle have been identified as independent factors for predicting success, each with limitations. The objective of this study is to elaborate a combined predictive model to predict ETV success in a mixed cohort of patients. METHODS: Demographic, intraoperative, postoperative, and radiologic variables were analyzed in all ventriculostomies performed consecutively at a single institution from Decemb

Spontaneous Third Ventriculostomy in Cases of Aqueductal Stenosis: A Retrospective Case Series.

BACKGROUND: Spontaneous third ventriculostomy (STV) is a rare occurrence in cases of obstructive hydrocephalus where the walls of the third ventricle rupture, communicating the ventricular system, and the subarachnoid space leading to arrest of active hydrocephalus. We aim to review our series of STVs while reviewing previous reports. METHODS: A retrospective review of cases undergoing cine phase-contrast magnetic resonance imaging (PC-MRI) from 2015 to 2022 of any age with imaging evidence of arrested obstructive hydrocephalus was performed. Patients in which aqueductal stenosis was radiolog

Feasibility of awake endoscopic third ventriculostomy in selected patients of obstructive hydrocephalus.

BACKGROUND: Endoscopic third ventriculostomy (ETV) is usually performed under general anesthesia (GA) with proper head immobilization. However a few patients with hydrocephalus (HCP) may not be suitable for GA. Once the surgeon is familiar with endoscopic ventricular anatomy and gains adequate surgical experience with the procedure, ETV can be attempted under local anesthesia (LA) in selected patients. Here we discuss our experience of treating 32 patients of HCP with ETV under LA. METHODS: 32 symptomatic HCP patients with in the age range of 13 and 65 years, conscious, alert, cooperative and

Endoscopic third ventriculostomy for noncommunicating hydrocephalus by vertebrobasilar dolichoectasia: A case report.

BACKGROUND: Vertebrobasilar dolichoectasia (VBD) is a vasculopathy characterized by the elongation, widening, and tortuosity of a cerebral artery. Rarely, hydrocephalus results when the extended basilar artery impairs communication of the cerebral ventricle and cerebrospinal fluid dynamics. We experienced such a case when a patient underwent endoscopic third ventriculostomy (ETV) for noncommunicating hydrocephalus with VBD. CASE DESCRIPTION: A 54-year-old man presented with cognitive dysfunction and was diagnosed with VBD by magnetic resonance imaging (MRI). Seven years later, he exhibited su

Transependymal Edema as a Predictor of Endoscopic Third Ventriculostomy Success in Pediatric Hydrocephalus.

BACKGROUND: The Endoscopic Third Ventriculostomy Success Score (ETVSS) is based on the clinical features of hydrocephalus except for radiological findings. A previous study suggested that transependymal edema (TEE) as a radiological finding may be a reliable predictor of endoscopic third ventriculostomy (ETV) success in patients of all ages. We aimed to investigate whether TEE on preoperative magnetic resonance imaging can predict ETV success in pediatric patients. METHODS: Medical and radiological records of all pediatric patients with an initial ETV in our hospital between 2013 and 2019 wer

Ventriculostomy and endoscopic biopsy of pineal mass with hydrocephalus and unexpected definitive diagnosis.

A 72-year-old female with no relevant medical history consulted in the emergency room for a two-week history of headache, vomiting and gait disturbance. Head CT scan revealed obstructive hydrocephalus secondary to a pineal mass with compression of the third ventricle. Magnetic Resonance showed another mass located in the right Lushcka foramen. Endoscopic third-ventriculostomy and biopsy of pineal mass were performed. Pathological analysis was consistent with metastasis of carcinoma. Full-body CT scan showed a lung mass related to primary carcinoma. The patient received systemic treatment for m
