Acute Dizziness, Vertigo, and Unsteadiness.

Vestibular symptoms, including dizziness, vertigo, and unsteadiness, are common presentations in the emergency department. Most cases have benign causes, such as vestibular apparatus dysfunction or orthostatic hypotension. However, dizziness can signal a more sinister condition, such as an acute cerebrovascular event or high-risk cardiac arrhythmia. A contemporary approach to clinical evaluation that emphasizes symptom duration and triggers along with a focused oculomotor and neurologic examination can differentiate peripheral causes from more serious central causes of vertigo. Patients with h

Neurologic Conditions: Dizziness and Vertigo.

Dizziness is seen frequently in patients in the family medicine and emergency department settings. The differential diagnosis of dizziness can be expansive but with a targeted history and physical examination, a correct diagnosis often can be established and appropriate treatment offered. Common etiologies of dizziness include hypotension, benign paroxysmal positional vertigo (BPPV), and Meniere disease. Strokes and malignancies also can cause this symptom. Imaging is indicated if intracranial pathology is suspected. BPPV typically is self-limited but maneuvers such as the Epley maneuver can b

[Vertigo and dizziness. Ophthalmological addendum].

The ophthalmologist's main task in the interdisciplinary approach to vertigo is the examination of ocular motility to rule out central ocular motility disorders. Further neuro-ophthalmological examinations (e.g. pupil reactions and optic disc) may provide unspecific findings that can be helpful. The ophthalmologist may be able to deliver puzzle-solving key findings in some multidisciplinary diseases (e.g. Cogan I, Vogt-Koyanagi-Harada disease and Behcet's disease). Ocular alterations do not cause vertigo but a variety of more diffuse complaints that some patients refer to as vertigo.

Mastication-induced vertigo and nystagmus.

Even though trigeminovestibular connections are well established in animals, mastication-induced dizziness has been described only as a vascular steal phenomenon in humans. We determined induction or modulation of nystagmus in two index patients with mastication-induced vertigo, 12 normal controls, and 52 additional patients with peripheral (n = 38, 26 with vestibular neuritis/labyrinthitis and 12 with Meniere's disease) or central (n = 14, 11 with Wallenberg syndrome, two with cerebellar infarction, and one with pontine infarction) vestibulopathy during their acute or compensated phase. Vertigo: clinical assessment and diagnosis.

Dizziness is a common presenting complaint to emergency departments and medical admission units; the key is to differentiate between central and peripheral causes of vertigo. This review focuses on bedside assessment of vertigo and, by looking at patterns of symptoms and signs, provides guidance as to when to suspect a central cause.

Alternobaric vertigo--really a hazard?

OBJECTIVE: To determine the prevalence of alternobaric vertigo (AV) in sport divers and to find out whether AV led to dangerous situations underwater. Furthermore, to examine whether objective neurootologic tests are associated with the manifestation of AV. DESIGN: Retrospective cohort study. PARTICIPANTS: Sixty-three sport divers with an average diving experience of 10 years and 650 dives were questioned regarding their medical and diving history and the manifestation of vertigo during diving. METHODS: Microscopic otoscopy, tympanometry, stapedius reflexes, hearing threshold for air and bo

[Treatment costs of otogenic vertigo].

PURPOSE: In this decision-tree analysis, the costs of otogenic vertigo treatment were investigated from the third-party payer's perspective. Either the combination preparation, with cinnarizine 20 mg and dimenhydrinate 40 mg as active substances, or betahistine (12 mg betahistinedimesilate) was administered. METHODS: A core model, based on clinical studies, was developed and a cost-effectiveness analysis was conducted. Both differences in effectiveness of the alternative treatments and adverse reactions and side effects were included. The number of cases, in which no more symptoms of dizzines

Vertigo and medico-legal problems.

Forensic medicine deals with the interactions of medical science with the law. All branches of medicine can play a role in dealing with medico-legal problems. Clinical and forensic evaluators need to be able to recognize variations of symptoms or syndromes that "don't read the textbooks". Atypical presentations of dizziness or vertigo have the potential to cause much confusion in diagnosis, treatment planning, and legal adjudication, if not correctly recognized. This article describes the forensic medicine in the legal system and forensic aspects of other medical specialities, gives an overvie

Benign positional vertigo: prognostic factors.

OBJECTIVE/HYPOTHESIS: We sought to assess prognostic factors associated with initial successful treatment and recurrence of benign paroxysmal positional vertigo (BPPV) with the canalith repositioning (CRP) maneuver with mastoid vibration. STUDY DESIGN AND SETTING: We conducted a retrospective chart review of 104 consecutive BPPV patients at a tertiary care referral center during 1999. METHODS: The main outcome measures were resolution of BPPV and absence of nystagmus on Hallpike maneuver. RESULTS: Ninety-three patients (89.4%) experienced initial symptom resolution, and 21 of the initially

[Diagnostic evaluation of "acute" vertigo].

Numerous studies of outpatients have been effected with the object of evaluating the role of the different causes of vertigo, and these have clearly evidenced the importance of the individual disciplinary sectors and, consequently, of the clinicoinstrumental exams necessary in order to reach the correct etiopathogenetic interpretation of the dizzy syndrome. We report the experience of a retrospective study carried out in 310 patients suffering from dizziness who were sent to us from the Emergency Room with a generic diagnosis of "acute vertigo". A detailed medical history was taken in every ca

[Benign paroxysmal vertigo in childhood].

Benign paroxysmal vertigo of childhood is a clinical disorder that usually begins at age 4. The child's otoneurological examination is normal in the intercrisis and the brief episodes of true vertigo are typical. In this paper we describe six new cases that had a follow-up of at least one year after diagnosis. Only a few of the patients had demonstrated vasomotor migraine, but all the children had a family history of migraine, which supports the hypothesis that this disorder is a migraine equivalent. The pathogenesis may be related with a transitory vascular disturbance that produces ischemia

Otoneurologic evaluation of child vertigo.

In this study 282 children with vertigo are subdivided (according to previous experiences) into three large groups: (1) vertigo and cochlear diseases; (2) vertigo as an isolated symptom; and (3) vertigo and C.S.N. diseases. Due to the difficult etiopathogenetic investigation of the patients from the second group, the authors focused on that group as they are less studied, are without associated symptoms (deafness--first group; CNS diseases--second group) and where vertigo appears as an idiopathic and an isolated symptom. A careful anamnestic, clinical and instrumental analysis leads to the fol

Cervical vertigo--reality or fiction?

Neck afferents not only assist the coordination of eye, head, and body, but they also affect spatial orientation and control of posture. This implies that stimulation of, or lesions in, these structures can produce cervical vertigo. In fact, unilateral local anesthesia of the upper dorsal cervical roots induces ataxia and nystagmus in animals, and ataxia without nystagmus in humans. If cervical vertigo exists outside these experimental conditions, it is obviously characterized by ataxia and unsteadiness of gait, and not by a clear rotational or linear vertigo. Neurological, vestibular, and psy

[Vertigo secondary to periaqueductal syndrome].

We present a clinical case of extrinsic periaqueductal syndrome caused by an epidermoid cyst in the pineal gland. This is a unusual disorder and those localized in the epiphysis are extremely rare. A bibliographic review of the incidence, clinical aspects, diagnosis and therapy is carried out in this article.

Repetitive paroxysmal nystagmus and vertigo.

A 55-year-old woman had paroxysms of vertigo and visual blurring associated with complex combined torsional, horizontal, and vertical nystagmus. These episodes occurred regularly at 2-minute intervals, each attack lasting for 15 seconds. Between attacks, there was a much finer asymptomatic nystagmus whose components were in the opposite direction to those associated with the paroxysmal attacks. A brain MRI revealed an arteriovenous malformation in close proximity to the left vestibular nucleus, with evidence of previous bleeding. Caloric testing demonstrated a left-sided vestibular paresis. [Cerebral SPET in vertigo syndrome].

Diagnosis of vertigo in the absence of permanent tissue damage is still difficult. Since a functional deficit should be associated with a reduction in the blood perfusion, authors performed a cerebral SPET in 23 patients with central vertigo, 10 with peripheral vertigo and 17 normal controls, and compared the results with those obtained by CT, NMR and doppler investigation. Cerebral SPET showed higher specificity and sensitivity than other techniques employed in the investigation of peripheral vertigo. These results demonstrated the usefulness of cerebral SPET in the diagnosis of vertigo, part

[Vertigo and middle ear pathology].

The association of vertigo and diseases of the middle ear is not uncommon. It occurs especially during the course of chronic or acute otitis but can also occur as a sequela. Vestibular involvement is due to several pathophysiological mechanisms that influence the choice of treatment. The same is true for expansive processes of the tympanic cavity. When associated to otospongiosis, vertigo occurring during Ménière's disease can transiently contraindicate an operation on the tapes. On the contrary, vertigo resulting from traumatic lesion of the middle ear can suggest surgical intervention. Surge
