Alcoholism as an addiction: the disease concept reconsidered.

The concept of alcoholism as a disease is compared to the concept of addiction in regard to individual intentionality, the resultant social and legal accountability and especially the total phenomenology of the addictive process. It is held that alcoholism is essentially an addiction like any other that if active, can culminate in various diseases and in death but that its most significant and characteristic volitional disorder: starting to drink when sober, is not explained by the disease concept. The concept of alcoholism as a disease is basically due to a reversal of the perception of cause

Respiratory apraxia in amyotrophic lateral sclerosis.

Respiratory dysfunction is a critical problem in amyotrophic lateral sclerosis (ALS). We report a patient with ALS who had respiratory apraxia. A 74-year-old female presented with progressive dysarthria and dysphagia. Clinical signs and evidence of widespread denervation on electromyography (EMG) confirmed the diagnosis of ALS. She had no signs of dementia. Irregular volitional inspiratory movements on verbal command were noticed, in contrast with rhythmic automatic inspiration - respiratory apraxia. Limb and buco-facial movements showed no signs of apraxia. EMG of respiratory muscles was norm

Age and sex influence the activation-dependent stiffness of the pectoralis major.

The pectoralis major fiber regions contribute uniquely to the mobility and the stability of the shoulder complex. It is unknown how age and sex influence the stiffness of these regions during volitional contractions, but this knowledge is critical to inform clinical interventions targeting the pectoralis major. The aim of the present study was to determine if the activation-dependent stiffness of the pectoralis major fiber regions differs between the sexes and if it is altered with age. Ultrasound shear wave elastography was used to acquire shear wave velocity from the clavicular and the stern

Long-term wheel running compromises diaphragm function but improves cardiac and plantarflexor function in the mdx mouse.

Dystrophin-deficient muscles suffer from free radical injury, mitochondrial dysfunction, apoptosis, and inflammation, among other pathologies that contribute to muscle fiber injury and loss, leading to wheelchair confinement and death in the patient. For some time, it has been appreciated that endurance training has the potential to counter many of these contributing factors. Correspondingly, numerous investigations have shown improvements in limb muscle function following endurance training in mdx mice. However, the effect of long-term volitional wheel running on diaphragm and cardiac functio

Vocal cord function and bispectral index in pediatric bronchoscopy patients emerging from propofol anesthesia.

OBJECTIVE: In children undergoing bronchoscopy for evaluation of stridor or respiratory symptoms, movement of the vocal cords is routinely assessed at the conclusion of flexible bronchoscopy with children still anesthetized. The effect of anesthesia on vocal cord function is not well described. This study aimed to characterize the relationship between depth of propofol anesthesia, as measured by Bispectral Index (BIS), and vocal cord movement in pediatric patients. METHODS: Children between 6 months and 12 years of age presenting for diagnostic flexible bronchoscopy were enrolled in this pros

Palatal and pharyngeal reflexes in health and in motor neuron disease.

Palatal and pharyngeal sensation and motor responses, and volitional palatal movement, were tested in 171 healthy adults and 43 patients with motor neuron disease. In healthy adults palatal and pharyngeal sensation and volitional palatal elevation were present in all; the palatal and pharyngeal motor responses could not be elicited on first testing in two (1.1%) and seven (4.1%) subjects respectively. Pharyngeal motor responses were more easily elicited in older subjects and palatal responses were more easily elicited in women. Eye watering and retching were the most common accompanying featur

High ventilation breathwork practices: An overview of their effects, mechanisms, and considerations for clinical applications.

High Ventilation Breathwork (HVB) refers to practices employing specific volitional manipulation of breathing, with a long history of use to relieve various forms of psychological distress. This paper seeks to offer a consolidative insight into potential clinical application of HVB as a treatment of psychiatric disorders. We thus review the characteristic phenomenological and neurophysiological effects of these practices to inform their mechanism of therapeutic action, safety profiles and future clinical applications. Clinical observations and data from neurophysiological studies indicate that

Monosynaptic Projections to Excitatory and Inhibitory preBötzinger Complex Neurons.

The key driver of breathing rhythm is the preBötzinger Complex (preBötC) whose activity is modulated by various functional inputs, e.g., volitional, physiological, and emotional. While the preBötC is highly interconnected with other regions of the breathing central pattern generator (bCPG) in the brainstem, there is no data about the direct projections to either excitatory and inhibitory preBötC subpopulations from other elements of the bCPG or from suprapontine regions. Using modified rabies tracing, we identified neurons throughout the brain that send monosynaptic projections to identified e

Closed Loop Microfabricated Facial Reanimation Device Coupling EMG-Driven Facial Nerve Stimulation with a Chronically Implanted Multichannel Cuff Electrode.

Permanent facial paralysis and paresis (FP) results from damage to the facial nerve (FN), and is a debilitating condition with substantial functional and psychological consequences for the patient. Unfortunately, surgeons have few tools with which they can satisfactorily reanimate the face. Current strategies employ static (e.g., implantation of nonmuscular material in the face to aid in function/cosmesis) and dynamic options (e.g., gracilis myoneurovascular free tissue transfer) to partially restore volitional facial function and cosmesis. Here, we propose a novel neuroprosthetic approach for

The value of respiratory muscle testing in a child with congenital muscular dystrophy.

Respiratory muscle testing is often limited to noninvasive volitional tests such as vital capacity and maximal static pressures. We report the case of a 12-year-old boy with congenital muscular dystrophy (CMD) in whom invasive and non-volitional respiratory muscle tests showed an elective diaphragmatic dysfunction with the preservation of expiratory muscle strength. This finding, coupled with a clinical phenotype associating diffuse muscle atrophy with finger hyperlaxity and proximal contractures, strengthened the suspicion of Ullrich CMD. Skin-cultured fibroblasts showed intracellular retenti

Behavioral and neurological foundations for the moral and legal implications of intoxication, addictive behaviors and disinhibition.

Disinhibition and addictive behaviors are related and carry moral implications. Both typically involve diminished consideration of negative consequences, which may result in harm to oneself or others. Disinhibition may occur on state and trait levels, and addictive substances may elicit disinhibitory states, particularly when intoxication is reached. Data suggest that trait disinhibition and addictions may be conceptualized as involving misdirected motivation with underlying biological bases including genetic factors, alterations in neurotransmitter systems and differences in regional brain fu

Catheterization Method and Functional Recovery of Neurogenic Bladder in Spinal Cord Injury.

IMPORTANCE: Neurogenic bladder dysfunction affects up to 80% of individuals with spinal cord injury (SCI), greatly impacting quality of life and health outcomes. Although current guidelines recommend intermittent catheterization primarily for infection prevention, its potential influence on recovery of volitional bladder control remains poorly understood. OBJECTIVE: To determine whether intermittent catheterization, compared with indwelling catheterization, is associated with a higher likelihood of regaining volitional bladder control within 1 year after discharge in individuals with SCI, and

Long-term urological and gynecological outcomes following complete primary repair in females with bladder exstrophy.

INTRODUCTION: Long-term continence outcomes in patients undergoing complete primary repair of exstrophy (CPRE) have shown that a subset of patients do not achieve continence until after puberty. We aim to update the continence outcomes as well as describe gynecological outcomes for females having undergone CPRE. MATERIALS AND METHODS: This was a retrospective review between 1989 and 2019 at a single institution. Inclusion criteria were females with classic bladder exstrophy who underwent CPRE. Exclusion criteria were variant diagnoses and age <4 years. Continence was defined as dry for ≥3 h (

[Clinical phenomenology and pathophysiology of daytime and nighttime imperative movements].

Imperative movements have an intermediate position between voluntary and involuntary movements and are also referred to as semi-voluntary, or induced. Their common characteristic is the urge, forcing the patient to perform an action that can have a different duration and degree of complexity - from a short twitch (with tics) to prolonged episodes of general motor restlessness (for example, akathisia or stereotypes). The ability to slow down this movement for a short or longer period of time by volitional effort is associated with the urge to make a movement, which predetermines the patient's p

Neurons as will and representation.

Memory recollections and voluntary actions are often perceived as spontaneously generated irrespective of external stimuli. Although products of our neurons, they are only rarely accessible in humans at the neuronal level. Here I review insights gleaned from unique neurosurgical opportunities to record and stimulate single-neuron activity in people who can declare their thoughts, memories and wishes. I discuss evidence that the subjective experience of human recollection and that of voluntary action arise from the activity of two internal neuronal generators, the former from medial temporal lo

Cognitive models of apraxia and motor control: linking theories and methods across two complementary research domains.

Apraxia is a complex movement disorder that frequently occurs following left hemisphere stroke. Studies on patients with apraxia constitute an especially interesting body of literature for motor control researchers who seek to understand the cognitive mechanisms involved in the voluntary control of movement. Reciprocally, among apraxia researchers, great interest exists concerning the ways in which methods and theory from the field of motor control can be brought to bear in the clinical and empirical evaluation of this disorder. Here we will review representative evidence on the etiology, freq

[Antidepressants].

Tricyclic antidepressants possess anticholinergic, alpha 1 anti-adrenergic, and H1 antihistaminic properties, and an overdose affects patients' quality of life, which has led to the development of novel antidepressant drugs. Selective serotonin reuptake inhibitors (SSRIs) are non-sedating drugs that selectively reuptake serotonin and are effective against anxiety. Adverse effects of SSRIs include gastrointestinal disturbances, sexual dysfunction, and bleeding tendency. Serotonin noradrenaline reuptake inhibitors (SNRIs) are non-sedating agents that are expected to improve volition. SNRIs are e

Adult consequences of repeated nicotine and Δ 9 -tetrahydrocannabinol (THC) vapor inhalation in adolescent rats.

The use of Electronic Drug Delivery Systems (EDDS, "e-cigarettes") to ingest nicotine and Δ 9 -tetrahydrocannabinol (THC) has surged in adolescent populations in the United States, as five times as many high-school seniors vape nicotine daily as use tobacco. At the same time 19.5% of seniors use cannabis at least monthly, with 12% using EDDS to deliver it. This study was conducted to examine the impact of repeated adolescent vapor inhalation of nicotine and THC in rats. Female Sprague-Dawley rats were exposed to 30-minute sessions of vapor inhalation, twice daily, from Post-Natal Day (PND) 31
