IntroductionFreshwater sources (eg, lakes, ponds, rivers, or streams) can be contaminated by pathogens. Ingesting water from these sources can cause illness if the water is insufficiently treated.MethodThe Waterborne Disease and Outbreak Surveillance System (WBDOSS) collects data on waterborne disease outbreaks associated with drinking water, recreational water, other nonrecreational water, and undetermined exposures to water. We analyzed outbreaks reported to WBDOSS that involved ingesting water from freshwater sources in outdoor settings.ResultFrom 2000 through 2022, public health officials

In 1951, Riley's classic 3-compartment model of gas exchange estimated pulmonary shunt, alveolar dead space, and an effective compartment representing the functioning lung. But trial-and-error steps and conversion charts made its application impractical. We implemented estimates of alveolar and effective PCO2 to simplify computations, making it useful when more advanced technologies are unavailable. Using stepwise computations, we studied 10 healthy individuals and 43 outpatients with mild to severe chronic obstructive pulmonary disease and, in another study, 32 healthy subjects during 12 h of

BackgroundThe Texas coral snake, Micrurus tener, is one of three elapid species native to the continental United States. The purpose of the study was to describe the clinical features of their envenomation.MethodsWe reviewed all human coral snake bites reported to the Texas Poison Center Network between January 2000 and December 2023. We excluded informational calls and cases in which the snake responsible for the bite was not confirmed as M. tener. Finally, we excluded bite victims who did not seek medical attention and those for whom no clinical information was available. The data collected

IntroductionHeadache is a hallmark symptom of chronic mountain sickness (CMS), a high altitude disease thought to be induced by excessive erythrocytosis (EE) in highlanders. Nevertheless, headache characteristics related to permanent high altitude residence remain understudied, as does the association between headache occurrence and hemoglobin concentration ([Hb]) in highlanders.MethodsA pilot cross-sectional study was conducted in La Rinconada, Peru (altitude 5100 m), over a 2-y period. During all comers' medical consultations, highlanders with no known history of chronic medical conditions (

IntroductionAccidental hypothermia increases mortality in patients with traumatic injury, making hypothermia management essential in prehospital trauma care. Chemical heat blankets are commonly used for this purpose. These blankets require time to get warm, with agitation often used to accelerate the exothermic reaction. Many search and rescue teams use chemical heat blankets, but optimal activation of the blankets remains uncertain. This study investigated how varying oxygen exposure durations affect the thermal performance of these blankets to guide their optimal use in search and rescue sce

Ice caving accidents are rare, but when they occur, they represent unique logistical and medical challenges for both explorers and rescuers. Factors such as the dynamic nature of the environment, exposure to cold temperatures, and complex extrication logistics should be taken into account. A hybrid approach between mountain and cave rescue techniques may be necessary. In this article we describe in detail a case of a rescue operation following a 15-m fall into a moulin on a glacier in Switzerland. We discuss the combined use of mountain and cave rescue techniques, adapting to the unique struct

Hammerhead worms are becoming increasingly common in the southeastern United States as the invasive species continues to spread. Limited data exist on this species acting as a pseudoparasite, with only 3 documented cases found on literature review of Bipalidae species living inside human hosts. We present one such case from Shreveport, Louisiana, of a patient who arrived at the emergency department after a hammerhead worm was expelled from his sinus passage. A 13-y-old male with a history of autism and developmental delay had been complaining of headache for 2 to 3 d. He had a spontaneous nose

Snakebites are a critical health issue in remote regions of the Amazon basin. I report a case of Bothrops bilineatus envenomation in a 29-y-old biologist conducting fieldwork in Parque Nacional del Manu, Peru. Following significant logistic delays-including an initial suboptimal antivenom dose and limited access to additional vials-the patient developed progressive edema, necrosis, coagulopathy, and signs of compartment syndrome. Management involved 2 partial doses of antivenom (1 at 1600 and the other at 2100), a subsequent transfer to a tertiary care center, antibiotics, analgesia, and event

The Veratrum genus of plants contains multiple toxic alkaloid compounds, and ingestion of Veratrum species may result in clinically significant human illness. Although many cases of Veratrum ingestion have been reported in the Eastern United States and Europe, no case report of human toxicity from the most common California species, Veratrum californicum, exists in the literature. We report on the clinical course and treatment of a patient with a known Veratrum californicum ingestion. We discuss the identification of the Veratrum species and compare our case to other cases of Veratrum ingestio

This case study describes adjunctive hyperbaric oxygen treatment (HBOT) of grade 3-4 frostbite, along with vasodilators and rheological agents. The patient in this study presented with established frostbite 3 days after an 11-day walk in cold, snowy, wet conditions in Tasmania, Australia. Hyperbaric oxygen treatment in this case seemed to reduce the penumbra of ischemia in both feet and thus possibly reduced the level of joint amputation in the toes. If available, HBOT is an adjuvant option for the treatment of frostbite, along with the more established treatments. More studies, however, are r

IntroductionPreventing illness and maintaining hygiene in the genital and anal regions are concerns when hikers spend extended time in the backcountry. This cross-sectional survey study assessed prevention and hygiene strategies used by female long-distance backpackers and whether these approaches are associated with an increased incidence of symptoms of diarrhea, urinary tract infection, vaginitis, and dermatitis.MethodsA total of 868 long-distance backpackers who hiked in 2021 completed a survey about the prevention and hygiene strategies they used, and results were divided by anatomic sex.

Mosquito-borne infections are a common cause of illness among returning travelers. Frontline healthcare providers should maintain a high degree of suspicion for these infections when evaluating ill returning travelers. This narrative review provides an overview of the most common mosquito-borne infections in travelers. We conducted a search of the medical literature for updates in the past 5 y. We reviewed resources of the US Centers for Disease Control (Yellow Book, Advisory Committee on Immunization Practices), and US Food and Drug Administration. Arboviral infections and malaria are the mos

A 46-y-old male was swimming in the Connecticut River near Hatfield, Massachusetts, when he suffered an unprovoked attack from a North American beaver (Castor canadensis). The beaver attacked the man 3 separate times, inflicting multiple bite wounds to his arms and torso, ultimately attaching itself to his arm without releasing. The patient was brought to our emergency department, where the wounds were treated, and rabies postexposure prophylaxis was begun. The beaver tested positive for rabies shortly thereafter. Here we review the case of an unprovoked attack by a beaver and clinical risk as

Snakebites in unusual anatomical locations may lead to life-threatening consequences. Merrem's hump-nosed viper (Hypnale hypnale) is a medically important snake in Sri Lanka and India that causes many bites and envenomings. Their bites occur almost exclusively on upper and lower limbs and commonly result in local effects, with some patients developing systemic envenoming. No antivenom is available for treating envenoming by H. hypnale. We report an unusual case of H. hypnale bite on the tongue of a 10-month-old infant resulting in rapid local swelling of the tongue and floor of the mouth, requ

We present an unusual case of concomitant exercise-associated hyponatremic encephalopathy (EAHE), exertional rhabdomyolysis (ER), and acute kidney injury (AKI) in a Grand Canyon hiker. Our case patient, an adult 41-year-old male, consumed an excessive amount of water during his descent into the Canyon during hot weather. The next day, he was unable to hike out due to severe leg pain and disorientation, and ultimately evacuated by helicopter, having a grand mal seizure in flight. Despite having no serum sodium level, medics administered an intravenous (IV) bolus of 3% hypertonic saline (HTS) be

Snakebite is a significant public health issue in which venom-induced consumption coagulopathy is a common and serious complication that results from the activation of the coagulation pathway by snake toxins. We report a male patient, 56 y old, who was thought to have been bitten by a snake on his left foot. He was transported to a nearby hospital where he received analgesics and 3 snake polyvalent antivenom vials, and then he was transported to our hospital after 12 h. He presented with 2 small puncture wounds, pain, blistering, and edema of the left foot. On the 2nd day, the patient develope
