Comparing two types of loading during inspiratory muscle training in patients with weaning difficulties: An exploratory study.

BACKGROUND: Inspiratory muscle training improves respiratory muscle function and may improve weaning outcomes in patients with weaning difficulties. Compared to the commonly used pressure threshold loading, tapered flow resistive loading better accommodates pressure-volume relationships of the respiratory muscles, which might help to facilitate application of external loads and optimise training responses. OBJECTIVE: The objective of this study was to compare acute breathing pattern responses and perceived symptoms during an inspiratory muscle training session performed against identical exte

Variable pressure support ventilation vs. biphasic positive airway pressure/airway pressure release ventilation in experimental acute respiratory failure.

BACKGROUND: Spontaneous breathing during mechanical ventilation (MV) can improve cardiorespiratory function and lung protection. Biphasic Positive Airway Pressure (BIPAP) and Airway Pressure Release Ventilation (APRV) are common MV modes that combine spontaneous breathing with controlled ventilation. Variable pressure support ventilation (PSV), which varies breath-by-breath pressure support randomly, has been shown to improve gas exchange and reduce lung injury compared to conventional MV modes. This study aimed to compare the short-term effects of variable PSV with BIPAP in a model of acute r

Effect of respiratory rate and size of cannula on pressure and dead-space clearance during nasal high flow in patients with COPD and acute respiratory failure.

Nasal high flow (NHF) is an efficient oxygenation tool for the treatment of respiratory failure. The study investigated the effect of breathing pattern on positive airway pressure and dead-space clearance by NHF. The breathing cycle during NHF was characterized in 26 patients with acute respiratory failure (ARF) and stable COPD and after mechanical ventilation (post-MV) via tracheostomy where also pressure was measured in the trachea. Dead-space clearance was measured in airway models during different breathing patterns. NHF reduced the respiratory rate (RR) and TI/TE through prolonging the TE

[Comprehensive assessment of the respiratory function of nasal packs with airway tubes].

UNLABELLED: Intranasal packs with breathing tubes are one of the possible methods of postoperative management of patients after endonasal surgery. The role of these devices is to provide the possibility of nasal breathing of patients to facilitate the course of the postoperative period. However, the possibility of providing proper nasal breathing with these devices has not been studied. The aim of the work is to study the calculated and clinical possibilities of nasal breathing when using intranasal packs with breathing tubes after endonasal surgical interventions. MATERIAL AND METHODS: We st

P0.1 is a useful parameter in setting the level of pressure support ventilation.

OBJECTIVE: The purpose of this study was to investigate whether changes in breathing pattern, neuromuscular drive (P0.1), and the work involved in breathing might help to set the individual appropriate level of pressure support ventilation (PSV) in patients with acute respiratory failure (ARF) requiring ventilatory assistance. DESIGN: A prospective, interventional study. SETTING: An 8-bed multidisciplinary intensive care unit (ICU). PATIENTS: Ten patients with ARF due to adult respiratory distress syndrome (ARDS), sepsis or airway infection were included in the study. Chronic obstructive pu

Hyperventilation with He-O2 breathing is not decreased by superimposed external resistance.

The purpose of this study was to determine the effect of imposed external resistance on the ventilatory response to He-O(2) breathing during peak exercise. To accomplish this purpose, separate inspiratory and expiratory external resistances were applied to offset for the decrease in intrapulmonary airway resistance with He-O(2) breathing. Seven men and three women (69+/-3 years, mean+/-S.D.) with normal pulmonary function performed graded cycle ergometry to exhaustion breathing room air, He-O(2) (79% He, 21% O(2)), He-O(2) with imposed expiratory resistance, and He-O(2) with imposed inspirator

Predominantly unilateral laryngomalacia in infants with unilateral vocal fold paralysis.

BACKGROUND: Neonatal unilateral vocal fold paralysis may arise iatrogenically, idiopathically, or in the context of an underlying neurologic disorder. Management is often supportive, focusing on diet modification to allow for safe oral feeding. We describe the clinical course of six infants with unilateral vocal fold paralysis who developed predominantly unilateral laryngomalacia ipsilateral to the affected vocal fold with associated severe respiratory symptoms and feeding difficulty. METHODS: Retrospective review of six infants with unilateral vocal fold paralysis and predominantly unilatera

Effects of an asymmetrical high flow nasal cannula interface in hypoxemic patients.

BACKGROUND: Optimal noninvasive respiratory support for patients with hypoxemic respiratory failure should minimize work of breathing without increasing the transpulmonary pressure. Recently, an asymmetrical high flow nasal cannula (HFNC) interface (Duet, Fisher & Paykel Healthcare Ltd), in which the caliber of each nasal prong is different, was approved for clinical use. This system might reduce work of breathing by lowering minute ventilation and improving respiratory mechanics. METHODS: We enrolled 10 patients ≥ 18 years of age who were admitted to the Ospedale Maggiore Policlinico ICU in

Slow breathing impacts inter-organ dynamics modulating brain function and risk behavior.

Successful decision-making requires that external information be interpreted in the context of the body's state. Within the framework of body-brain interaction, deliberately modifying one's autonomic state can shape how we evaluate the world, ultimately influencing choices. Yet, it remains unclear whether and how intentional autonomic regulation affects human decision-making. In this study, we tested instructed use of prolonged exhalation, a slow-breathing technique designed to boost parasympathetic activity during risky decision-making. Participants followed distinct breathing protocols while

Measurement of air trapping, intrinsic positive end-expiratory pressure, and dynamic hyperinflation in mechanically ventilated patients.

Severe airflow obstruction is a common cause of acute respiratory failure. Dynamic hyperinflation affects tidal ventilation, increases airways resistance, and causes intrinsic positive end-expiratory pressure (auto-PEEP). Most patients with asthma and chronic obstructive pulmonary disease have dynamic hyperinflation and auto-PEEP during mechanical ventilation, which can cause hemodynamic compromise and barotrauma. Auto-PEEP can be identified in passively breathing patients by observation of real-time ventilator flow and pressure graphics. In spontaneously breathing patients, auto-PEEP is measu

Patient-Ventilator Interaction During Sleep.

Sleep in the ICU is subjectively and objectively poor. Patients in the ICU have short sleep duration and highly fragmented sleep. Poor sleep in the ICU is associated with worse outcomes, particularly the development of ICU delirium. Patients undergoing noninvasive or invasive mechanical ventilation struggle with sleep. For those patients on noninvasive ventilation, settings may need to be adjusted compared to wake to account for the physiological changes that occur with the sleep state. Specifically, upper airway collapsibility (ie, obstructive sleep apnea) must be considered as do changes in

Comparison of Diaphragmatic Stretch Technique and Manual Diaphragm Release Technique on Diaphragmatic Excursion in Chronic Obstructive Pulmonary Disease: A Randomized Crossover Trial.

BACKGROUND: Chronic Obstructive Pulmonary Disease (COPD) impairs the function of the diaphragm by placing it at a mechanical disadvantage, shortening its operating length and changing the mechanical linkage between its various parts. This makes the diaphragm's contraction less effective in raising and expanding the lower rib cage, thereby increasing the work of breathing and reducing the functional capacity. AIM OF THE STUDY: To compare the effects of diaphragmatic stretch and manual diaphragm release technique on diaphragmatic excursion in patients with COPD. MATERIALS AND METHODS: This ran

Remifentanil improves breathing pattern and reduces inspiratory workload in tachypneic patients.

BACKGROUND: Properly titrated opiates decrease respiratory rate but do not affect tidal volume or induce respiratory acidosis. OBJECTIVE: To determine whether remifentanil improves breathing pattern or reduces inspiratory effort in patients with acute respiratory failure and tachypnea or rapid shallow breathing. METHODS: We studied 14 patients who developed tachypnea and/or rapid shallow breathing if the pressure support level was reduced. During pressure support ventilation, each patient received 30-min infusions, separated by 30 min, of remifentanil and placebo. Measurements were obtained

[Use of helium and oxygen mixtures in status asthmaticus].

Four patients in status asthmaticus and with alveolar hypoventilation were given a helium-oxygen (He-O2) mixture to breathe during 4 hours. Clinical improvement and return to normal of arterial blood gas levels were rapid and dramatic in all patients. The He-O2 mixture, which is a low density gas, causes a reduction in pulmonary resistance and an improvement in overall alveolar ventilation.

Spontaneous Massive Pneumomediastinum in a Previously Well Infant With COVID-19.

A 3-month-old boy presented with a 3-hour history of a neck lump and difficulty breathing after 5 days of fever and reduced feeding. Pneumomediastinum with subcutaneous emphysema were identified, and the child was intubated because of severe work of breathing, requiring significant levels of oxygen and ventilatory pressure. Computed tomography chest scan revealed massive pneumomediastinum and significant bilateral parenchymal disease. The child deteriorated cardiovascularly, so the mediastinum was dissected by cardiothoracic surgeons and 2 drains were placed. The patient clinically improved wi

Determinants of weaning success in patients with prolonged mechanical ventilation.

INTRODUCTION: Physiological determinants of weaning success and failure are usually studied in ventilator-supported patients, comparing those who failed a trial of spontaneous breathing with those who tolerated such a trial and were successfully extubated. A major limitation of these studies was that the two groups may be not comparable concerning the severity of the underlying disease and the presence of comorbidities. In this physiological study, we assessed the determinants of weaning success in patients acting as their own control, once they are eventually liberated from the ventilator. M
