The Recombinant Bacille Calmette-Guérin Vaccine VPM1002: Ready for Clinical Efficacy Testing.

The only licensed vaccine against tuberculosis (TB), bacille Calmette-Guérin (BCG), protects against severe extrapulmonary forms of TB but is virtually ineffective against the most prevalent form of the disease, pulmonary TB. BCG was genetically modified at the Max Planck Institute for Infection Biology to improve its immunogenicity by replacing the urease C encoding gene with the listeriolysin encoding gene from Listeria monocytogenes. Listeriolysin perturbates the phagosomal membrane at acidic pH. Urease C is involved in neutralization of the phagosome harboring BCG. Its depletion allows for

AIDS defining diseases in the UK: the impact of PCP prophylaxis and twelve years of change.

We examined all reports of adult AIDS cases made to the 2 national surveillance centres in the UK for changes in AIDS defining conditions between January 1982 and September 1994. Differences and changes among persons diagnosed since January 1988 who had and had not been aware of their HIV infection prior to their AIDS diagnosis were of particular interest. Pneumocystis carinii pneumonia (PCP) is the AIDS defining disease most often reported at the initial AIDS diagnosis. Its proportion of all AIDS cases has increased significantly between January 1982 and December 1987 and decreased markedly t

Incidence and risk factors of adverse drug reactions in multidrug-resistant/rifampicin-resistant tuberculosis (MDR/RR-TB) regimens containing new drugs: a retrospective study of two national multicenter cohorts.

BACKGROUND: Treatment of multidrug-resistant/rifampicin-resistant tuberculosis (MDR/RR-TB) faces severe challenges, including prolonged courses, marked drug toxicity, and poor patient compliance. While regimens containing new drugs (bedaquiline, delamanid) have substantially improved MDR/RR-TB outcomes, systematic research on the epidemiological characteristics and risk control of adverse drug reactions (ADRs) remains insufficient. This study, based on a national multi-center clinical cohort, retrospectively analyzed data from 2,151 patients, aiming to explore the characteristics and risk fact

Trends and determinants of unfavourable outcomes in paediatric tuberculosis: insights from a 20-year cohort in Cameroon.

BACKGROUND: Tuberculosis (TB) remains a leading cause of childhood morbidity and mortality in resource-limited settings. Despite progress in TB care, unfavourable treatment outcome persists, highlighting the need to identify determinants and address gaps in paediatric TB management. This study aimed to assess treatment outcomes and identify determinants of unfavourable outcomes in children treated for TB at a referral centre in Cameroon. METHODS: This retrospective cohort study included children aged <15 years diagnosed with TB and followed at the Jamot Hospital of Yaoundé from 2001 to 2020.

The safety of pyrazinamide in pediatric drug-sensitive tuberculosis treatment: A real-world study.

BACKGROUND: Pyrazinamide (PZA) is a cornerstone of first-line treatment for tuberculosis (TB), yet its safety profile in children is not well-established. This evidence gap creates a significant discrepancy between international guidelines and real-world clinical practice. We aimed to analyze the usage patterns and safety of PZA in a large cohort of pediatric TB patients. METHODS: We conducted a multicenter retrospective study at 11 referral hospitals from 2017 to 2022. From an initial cohort of all hospitalized children with TB, we identified 552 patients with drug-sensitive TB who constitut

Mortality among persons with tuberculosis in Zambian hospitals: A retrospective cohort study.

Tuberculosis (TB) mortality in Zambia remains high at 86 per 100,000 populations, translating to approximately 15,000 TB-related deaths annually. We conducted a nationwide retrospective cohort study to understand predictors, time to death, and probable causes of mortality among persons on TB treatment in Zambia. We reviewed medical records for persons with TB registered in 54 purposively selected hospitals in Zambia between January and December 2019. We fitted a Cox proportional hazards model to identify predictors of mortality. Of the 13,220 records abstracted, 10,987 were analyzed after excl

Genome sequences of BCG Pasteur ATCC 35734 and its derivative, the vaccine candidate BCGΔBCG1419c.

BACKGROUND: Bacillus Calmette-Guérin (BCG) remains the only vaccine to prevent tuberculosis (TB) during childhood, with relatively low to no efficacy against pulmonary TB in adolescents and adults. BCG consists of close to 15 different substrains, where genetic variations among them might contribute to the variable protective efficacy afforded against pulmonary TB. We have shown that the vaccine candidate, BCGΔBCG1419c, which is based on BCG Pasteur, improved protection against chronic TB in murine models, as well as against pulmonary and extrapulmonary TB in guinea pigs. Here, to confirm dele

FujiLAM for the diagnosis of childhood tuberculosis: a systematic review.

BACKGROUND: Childhood tuberculosis (TB) remains underdiagnosed. The novel lateral flow FujiLAM assay detects lipoarabinomannan (LAM) in urine, but data on performance in children remain limited. METHODS: We conducted a systematic review assessing the diagnostic performance of FujiLAM for diagnosing paediatric TB. The last search was conducted in November 2021. RESULTS: We included three studies with data from 698 children for FujiLAM. For FujiLAM, sensitivity using a microbiological reference standard were 60% (95% CI 15 to 95), 42% (95% CI 31 to 53) and 63% (95% CI 50 to 75), respectively.

T Helper Cell Subsets in the Pleural Fluid of Tuberculous Patients Differentiate Patients With Non-Tuberculous Pleural Effusions.

BACKGROUND: Tuberculous pleural effusion (TPE) is one of the most common forms of extrapulmonary tuberculosis (Tb). Patients with TPE or malignant pleural effusions (MPE) frequently have a similar lymphocytic pleural fluid profile. Since the etiology of PE in various diseases is different, identifying the cellular components may provide diagnostic clues for understanding the pathogenesis. OBJECTIVE: We determined the frequency of T helper (Th) subtypes in the PEs for differentiation of Tb and non-Tb patients. METHODS: Thirty patients with TPE, 30 patients with MPE, 14 patients with empyema (

Neurotuberculosis: an update.

Tuberculous (TB) meningitis (TBM), accounting for 70-80% of cases of neurotuberculosis, is one of the most severe forms of extrapulmonary tuberculosis. Two-thirds of new TB cases come from eight countries. Polymorphisms in toll-interleukin-1 receptor domain and in leukotriene A4 hydrolase (LTA4H) gene, affect the risk of inflammation in TBM. The common site of tuberculoma in children is cerebellum, and they may rarely develop tuberculous encephalopathy which has a high mortality. Young females with a high cerebrospinal fluid (CSF) protein have an increased predisposition to develop optochiasma

Risk of tuberculosis among Alabama children and adolescents treated with tumor necrosis factor inhibitors: a retrospective study.

BACKGROUND: Tumor Necrosis Factor inhibitors (TNFi) have dramatically improved the outlook for patients with inflammatory arthritides and bowel disease (IBD), but are associated with increased infection risks, including tuberculosis (TB). Pediatric inflammatory diseases are uncommon, and the risk of TB in children taking TNFi remains unclear. The objective of this study was to report the incidence of TB disease among TNFi recipients at a single pediatric medical center serving most of Alabama compared to that of the general population of Alabama children. METHODS: Instances of TNFi usage amon

Outcomes of comprehensive care for children empirically treated for multidrug-resistant tuberculosis in a setting of high HIV prevalence.

BACKGROUND: Few studies have examined outcomes for children treated for multidrug-resistant tuberculosis (MDR-TB), including those receiving concomitant treatment for MDR-TB and HIV co-infection. In Lesotho, where the adult HIV seroprevalence is estimated to be 24%, we sought to measure outcomes and adverse events in a cohort of children treated for MDR-TB using a community-based treatment delivery model. METHODS: We reviewed retrospectively the clinical charts of children ≤15 years of age treated for culture-confirmed or suspected MDR-TB between July 2007 and January 2011. RESULTS: Nineteen
