Genetic and Epidemiological Similarities, and Differences Between Postoperative Intraperitoneal Adhesion Development and Other Benign Fibro-proliferative Disorders.

Intraperitoneal adhesions complicate over half of abdominal-pelvic surgeries with immediate, short, and long-term sequelae of major healthcare concern. The pathogenesis of adhesion development is similar to the pathogenesis of wound healing in all tissues, which if unchecked result in production of fibrotic conditions. Given the similarities, we explore the published literature to highlight the similarities in the pathogenesis of intra-abdominal adhesion development (IPAD) and other fibrotic diseases such as keloids, endometriosis, uterine fibroids, bronchopulmonary dysplasia, and pulmonary, i

[Diagnosis and treatment of radiation intestinal injury: current status and perspectives].

Radiation intestinal injury (RII) refers to the intestinal complication resulting from radiation therapy of pelvic, abdominal or retroperitoneal tumor, which involves the small intestine, colon and rectum. Although the advances in radiotherapy technology have decreased the injury of adjacent tissues, 90% of the patients receiving radiotherapy have acute symptoms, the quality of life is affected due to gastrointestinal symptoms in 50% of patients, and 20%-40% of patients have moderate to severe symptoms. Based on the pathological stage, characteristics and clinical manifestations, RII can be di

Diagnosis of Liver Involvement in Primary Sjögren Syndrome.

Liver involvement was one of the first extraglandular manifestations to be reported in patients with primary Sjögren syndrome (SS). In the 1990s, a study of liver involvement in patients with primary SS integrated the evaluation of clinical signs of liver disease, liver function and a complete panel of autoantibodies. Recent developments in the field of hepatic and viral diseases have significantly changed the diagnostic approach to liver involvement in SS. The most recent studies have shown that, after eliminating hepatotoxic drugs and fatty liver disease, the two main causes of liver disease

Roles of IL-17, Th1, and Tc1 cells in patients with IgG4-related sclerosing sialadenitis.

OBJECTIVES/HYPOTHESIS: Immunoglobulin G4 (IgG4)-related sclerosing sialadenitis is a recently recognized disease entity characterized by high serum IgG4 concentration and IgG4-producing plasma cell expansion in affected organs, which show fibrotic or sclerotic changes. However, little is known about the roles of CD4+ and CD8+ T cells or interleukin (IL)-17 in this disease. The purpose of this study was to evaluate the characteristics of CD4+ and CD8+ T cells and IL-17 in patients with IgG4-related sclerosing sialadenitis. STUDY DESIGN: A retrospective clinical study at the Yamagata University

IgG4-related sclerosing cholangitis mimicking cholangiocarcinoma.

IgG4-related disease has gained increased attention worldwide. While the initial focus was on autoimmune pancreatitis which was first described in Asian populations and turned out to be of relevance in Western populations too, the scope has recently broadened towards a notion of a multi-systemic disease with very diverse manifestations such as autoimmune pancreatitis, IgG4-related sclerosing cholangitis (IgG4-SC), retroperitoneal fibrosis and tubulointerstitial nephritis. IgG4-SC (also known as IgG4-associated cholangitis, IAC) represents a rare but clinically challenging differential diagnosi

Acute reversible kidney injury secondary to bilateral ureteric obstruction.

BACKGROUND: Acute reversible kidney injury (ARKI) secondary to bilateral ureteric obstruction (BUO) is a common urological problem. Our goals were to describe the etiology, management and outcomes of such patients identified between 2006 and 2009 and to compare them with a similar historical study published in 1982. METHODS: Chart review was performed on 49 patients with AKRI secondary to BUO. ARKI was defined as ≥33% decrease in serum creatinine after intervention. Those with malignant and benign causes of obstruction were identified and management and outcome data were collected. RESULTS:

Clinical clues to suspicion of IgG4-associated sclerosing cholangitis disguised as primary sclerosing cholangitis or hilar cholangiocarcinoma.

BACKGROUND AND AIM: This study aimed to determine the clinical characteristics of immunoglobulin G4 (IgG4)-associated sclerosing cholangitis (ISC) and provide clinical clues differentiating ISC from primary sclerosing cholangitis (PSC) or hilar cholangiocarcinoma (CCC). METHODS: Sixteen patients with ISC manifesting as hilar/intrahepatic strictures were analyzed for clinical characteristics and compared with patients with PSC and hilar CCC as disease controls for histology and serum IgG4 levels. RESULTS: Distinguished biliary imaging findings of ISC included multifocal biliary tree involveme

Prognosis of autoimmune pancreatitis.

Autoimmune pancreatitis (AIP) is responsive to steroid therapy, but some AIP patients improve spontaneously, or only improve after biliary drainage. Pancreatic enlargement and irregular narrowing of the main pancreatic duct usually improves in almost all patients, but marked atrophy of the pancreas develops in some patients. Biliary stenosis improves to various degrees, and a biliary drainage tube can be withdrawn. Other extrapancreatic lesions, including swelling of the salivary or lacrimal glands, lymphadenopathy, and retroperitoneal fibrosis also improve with steroid therapy. Pancreatic end

Systemic extrapancreatic lesions associated with autoimmune pancreatitis.

Autoimmune pancreatitis (AIP) is frequently associated with sclerosing cholangitis (SC). SC with AIP has a cholangiographic appearance that is often confused with primary sclerosing cholangitis (PSC) but only the former responds well to corticosteroid therapy. Detailed study of cholangiographic findings allows discrimination of SC with AIP from PSC. Band-like strictures, a beaded or pruned-tree appearance, and diverticulum-like outpouching were significantly more frequently observed in cases of PSC. In contrast, segmental strictures, dilation after confluent stricture, and strictures of the lo

Long-term prognosis of autoimmune pancreatitis with and without corticosteroid treatment.

OBJECTIVES: Autoimmune pancreatitis (AIP) is a unique form of chronic pancreatitis, and has a favourable response to corticosteroid treatment (CST). Little is known, however, about the long-term outcome of AIP. This study aimed to document the prognosis without and with CST, and to examine the indication for CST. PATIENTS AND METHODS: The prognosis and clinical features of 23 patients without CST and 19 patients treated with CST from onset were investigated. In addition, factors concerning the late occurrence of unfavourable events related to AIP were examined. RESULTS: The patients without

[Urogenital lesions diagnosed incidentally during evaluation for blunt renal injuries].

OBJECTIVES: Pre-existing pathology of the urogenital system may interfere with the pathophysiology of trauma, alter the radiographic imaging and influence the way of therapeutic approach. The aim of our study was to record the pathological lesions of the urogenital tract found incidentally during evaluation for blunt renal trauma and subsequently comment on their importance. MATERIAL AND METHODS: The files of 675 patients referred to our department from 1982 to 1998 for further evaluation of possible blunt renal injury were reviewed. Our standard procedure is to perform an ultrasonogram and i

Wire-reinforced ureteral stents to rescue from nephrostomy tube in extrinsic ureteral obstruction.

INTRODUCTION: Ureteral obstruction due to extrinsic compression is associated with significant morbidity and mortality. Management options for this condition include renal drainage with percutaneous nephrostomy (PCN) or internal ureteral stent placement. A significant portion of patients will have disease progression leading to internal stent obstruction which is almost uniformly managed with PCN. We evaluated a novel, wire-reinforced internal ureteral stent as an alternative to PCN in those patients who fail initial internal ureteral stent placement. MATERIALS AND METHODS: A retrospective ch

Chronic periaortitis with thoracic aorta and epiaortic artery involvement: a systemic large vessel vasculitis?

OBJECTIVE: Chronic periaortitis (CP) is a rare disease characterized by fibro-inflammatory tissue surrounding the abdominal aorta and the iliac arteries. Anecdotal reports have shown that CP may also involve other vascular districts, particularly the thoracic aorta. The aim of this study was to investigate the thoracic aorta and epiaortic artery involvement in CP. METHODS: Patients were eligible if they had undergone imaging studies assessing inflammatory involvement of the thoracic aorta and its major branches (e.g. contrast CT, MRI or PET-CT). We explored the patterns of thoracic vessel inv

Genetic variability of the envelope gene of Type D simian retrovirus-2 (SRV-2) subtypes associated with SAIDS-related retroperitoneal fibromatosis in different macaque species.

BACKGROUND: D-type simian retrovirus-2 (SRV-2) causes an AIDS-like immune deficiency syndrome (SAIDS) in various macaque species. SAIDS is often accompanied by retroperitoneal fibromatosis (RF), an aggressive fibroproliferative disorder reminiscent of Kaposi's sarcoma in patients with HIV-induced AIDS. In order to determine the association of SRV-2 subtypes with SAIDS-RF, and study the evolution and transmission of SRV-2 in captive macaque populations, we have molecularly characterized the env gene of a number of SRV-2 isolates from different macaque species with and without RF. RESULTS: We s

Radiographic and clinical findings in unusual abdominal aortic aneurysms.

Atherosclerotic abdominal aortic aneurysms typically present either asymptomatically or with abdominal or back pain. Inflammatory, mycotic, and traumatic aortic aneurysms and aneurysms complicated by venous fistula are less common. The different clinical presentations of these latter aneurysms should raise a suspicion to their diagnosis and appropriate preoperative evaluation. The radiographic and clinical features of inflammatory, mycotic, traumatic, and complicated atherosclerotic aneurysms are presented and discussed.

IgG4-related disease: findings from a retrospective cohort in Colombia.

OBJECTIVE: IgG4-related disease (IgG4-RD) is a recently described fibroinflammatory disorder. The most common presentations include salivary and lacrimal gland hypertrophy, orbitary disease, autoimmune pancreatitis, retroperitoneal fibrosis, and tubulointerstitial nephritis. Lymphoplasmacytic infiltration, fibrosis, IgG4+ cell hyperplasia, and elevated IgG4 serum levels are the primary pathophysiological findings related to the disease. We describe for the first time the epidemiology and clinical manifestations in our country. METHODS: A descriptive study of patients from a retrospective coho
