The memory plate for four-corner fusion of scaphoid non-union advanced collapse.

The aim of this study was to evaluate the clinical outcomes of the memory plates for four-corner fusion (FCF) of scaphoid non-union advanced collapse (SNAC). From 2005-2010, 11 cases of SNAC received FCF with the memory plate designed by the authors. At an average follow-up of 21 months, there were no non-unions and postoperative infections. One patient complained of dorsal impingement. The pain relief rate was 100% and the average grip strength increased from 11.3% preoperatively to 56.3% postoperatively of the contralateral side. The primary results of FCF with memory plates are promising. B

Minimal invasive management of scaphoid fractures: from fresh to nonunion.

The peculiar shape of scaphoid hinders a precise evaluation of its fracture configuration, displacement and accuracy of screw placement. Its tenuous vascular supply risks the complications of delayed union, nonunion and avascular necrosis. Scaphoid is the focus of ligamentous attachment governing carpal kinematics. Preservation of its anatomy and vascularity is critical for normal wrist function. A new fracture classification clearly denoting every fracture type and guiding the management is introduced. The minimal invasive management of different scaphoid fracture conditions, including acute

Pediatric Scaphoid Nonunions: Does Insurance Status Play a Role?

OBJECTIVE: There are no studies that evaluate the effects that insurance or socioeconomic status may have on treatment and outcomes of scaphoid nonunions in children. We evaluated whether there is an association between insurance status and (1) time from injury to hand surgeon evaluation, (2) time from specialist evaluation to surgery, and (3) postoperative outcomes of scaphoid fracture nonunions in children. METHODS: A chart review was performed for patients who underwent surgical treatment of scaphoid fracture nonunion from January 2015 and April 2021 at a large tertiary care pediatric hosp

[Necrosis of the carpal scaphoid after chemotherapy. Case report].

Preiser's disease--first described by Preiser in 1910--presents the rare avascular necrosis of the carpal scaphoid. There is no consensus with regard to cause and therapeutic procedure to this day. The initially postulated idiopathic origin of the disease is controversial and rare. It is mainly caused by trauma or repetitive microtrauma, but can also occur as a complication of drug therapy. The rare occurrence of necrosis of the carpal scaphoid following chemotherapy is presented. The treatment was resection of the scaphoid and mediocarpal arthrodesis. Clinical appearance, diagnostic work-up,

Trapeziectomy and Ligament Reconstruction Using a Reinforced Half-Slip of the Extensor Carpi Radialis Longus Tendon for Thumb Carpometacarpal Osteoarthritis.

BACKGROUND: Carpometacarpal (CM) joint osteoarthritis of the thumb is characterized by pain and impaired thumb function in terms of pinch and range of motion. Here, we newly adopted a trapeziectomy with a graft-augmented ligament reconstruction procedure, in which the reconstructed ligament is reinforced using a half-slip extensor carpi radialis longus (ECRL) tendon. METHODS: From 2015 to 2022, 101 hands of 95 patients with CM joint osteoarthritis of the thumb underwent trapeziectomy with graft-augmented ligament reconstruction using the half-slip ECRL tendon to reconstruct the ligament. Treatment of wrist and hand fractures with natural magnets: preliminary report.

The Authors, after having defined the phenomenon and the biological characteristics of natural magnets, evaluate their ability in accelerating the formation of bone callus in hand and wrist fractures compared to treatment with immobilization in a plaster cast. Forty patients (4 females and 37 males) between 20 and 86 years of age were treated. A small natural magnet was inserted in each of the plaster casts (diameter: 2cm, height: 0.5cm) made of 4 blocks in Neodymium-Iron-Boron, capable of generating 4 magnetic poles (2 positive and 2 negative) of diagonal alternate polarity that produced a sy

[Results after minimally invasive therapy of acute scapholunate dissociation].

In 27 cases with acute scapholunate ligament injury, minimal invasive treatment was performed. After closed reduction scapholunate transfixation was achieved by Kirschner wires. Postoperatively, a short-arm cast was applied for six weeks. All patients have been reexamined on average 26 months after operation. Follow-up results have been good concerning clinical examination and patients' satisfaction. Radiological examination has shown recurrence of scapholunate instability in 15 cases. In fact, 11% of the patients had dynamic and 44% static instability at follow-up. Recurrence has been most fr

Patient expectations and outcomes of successful scaphoid surgery: a prospective cohort analysis using the trauma expectation and trauma outcome measure (TEFTOM).

BACKGROUND: Scaphoid fractures are common carpal injuries that are commonly treated operatively. Increasingly, patient-reported outcome measures (PROMs) highlight the influence of expectations on perceived success. The Trauma Expectation and Trauma Outcome Measure (TEFTOM) is a validated instrument assessing expectations (TEF) and outcomes (TOM) across domains of pain, function, disability, injury satisfaction, and overall satisfaction. While applied in other orthopaedic trauma conditions, TEFTOM has not been previously studied in scaphoid surgery. METHODS: This prospective cohort study enrol

Assessment of scaphoid mobility in patients with suspected scapholunate instability: a radiographic and four-dimensional computed tomography study.

INTRODUCTION: This study was performed to evaluate the validity and reliability of the anteroposterior intrascaphoid angle (APISA) on radiographs, and the radioscaphoid angle (RSA) on dynamic four-dimensional computed tomography (4DCT) as indicators of the extent of scaphoid flexion-extension. We also evaluated the contribution of these two measurements relative to the stage of scapholunate instability for identifying reduced scaphoid mobility. METHODS: One-hundred and fifty patients referred with suspected chronic scapholunate dissociation were investigated with anteroposterior radiograph of

The Prevalence of Scapholunate Signal Abnormalities on Magnetic Resonance Imaging.

BACKGROUND: Although trauma is a major cause of symptomatic scapholunate interosseous ligament (SLIL) pathology, many patients do not recall a specific injury or repetitive trauma. We report on: (1) the prevalence of SLIL signal changes in patients who underwent wrist magnetic resonance imaging (MRI) for various indications; and (2) the prevalence of SLIL signal changes on MRI in patients without prior wrist trauma. METHODS: This is a retrospective study evaluating 1021 patients who underwent wrist MRI or magnetic resonance arthrogram. We collected data on SLIL signal changes on MRI. Patients

Weight-Bearing Computed Tomography Analysis of Scapholunate Ligament Injury.

PURPOSE: Weight-bearing computed tomography allows for assessment of carpal alignment and stability under physiologic load conditions, which may allow for detection of subtle dynamic instabilities that may not be apparent in nonweight-bearing images. The purpose of this study was to evaluate carpal position under physiologic load in healthy and scapholunate interosseous ligament (SLIL)-injured wrists using weight-bearing computed tomography. METHODS: Ten healthy controls and 10 participants with arthroscopy (Geisler III or IV) or magnetic resonance imaging-proven SLIL injury underwent compute

Reconstruction of an Early-Stage Scapholunate Advanced Collapse Wrist with the 3-Ligament Tenodesis Procedure: A Controversial Reappraisal.

BACKGROUND: The authors hypothesized that the 3-ligament tenodesis (3-LT) procedure is still sufficient-even in scapholunate advanced collapse (SLAC) cases-to reduce pain and improve wrist function. The authors compared patient-reported outcomes of scapholunate interosseus ligament (SLIL) injury patients with SLAC, to SLIL injury patients treated with 3-LT, and then to patients who underwent proximal row carpectomy (PRC), as a control group. METHODS: The authors included all patients with a traumatic SLIL injury and associated SLAC components treated with 3-LT and completed patient-rated wris

Scapholunate Ligament Reconstruction With Internal Brace Augmentation Techniques for Chronic Scapholunate Dissociation: A Clinical Follow-up Study.

INTRODUCTION: The scapholunate ligament is the most important stabilizer of the scapholunate articulation. The management of chronic irreversible injuries of this ligament in the absence of preexisting arthritis of the wrist joint remains controversial. Recently, surgeons introduced a novel surgical technique using an internal brace (IB). Several biomechanical studies on this technique have been conducted using cadavers; however, very few studies have discussed the results in detail in actual clinical practice. Therefore, herein, we investigated the radiological and functional results of patie
