Interventional radiology in the management of complications after pancreatic surgery: a single-center experience.

PURPOSE: To assess the impact of interventional radiology (IR) as first-line rescue therapy for major complications after pancreaticoduodenectomy and to quantify its effect on surgical re-exploration and 90-day mortality. MATERIALS AND METHODS: In this retrospective single-center cohort, 200 consecutive patients undergoing pancreaticoduodenectomy (2014-2025) were reviewed. Complications were recorded using ISGPS/ISGLS definitions; postoperative pancreatic fistula (POPF) included biochemical leak. Patients were grouped by initial management strategy (IR-first vs. primary surgery); crossover to

Conversion to open surgery during robotic pancreatoduodenectomy in surgical high-risk groups: patterns, risk factors, and outcomes in elderly and/or obese patients.

BACKGROUND: Conversion to laparotomy during robotic pancreatoduodenectomy (RPD) carries important clinical implications, yet its incidence, predictors, and consequences in elderly and/or obese patients remain poorly characterized. This study aimed to define the conversion rate, identify preoperative predictors, and assess the impact of conversion on postoperative outcomes in elderly and/or obese surgically high-risk patients undergoing attempted RPD. METHODS: A retrospective analysis was performed using a multi-institutional database. Patients were included if they underwent attempted RPD and

Pancreaticoduodenectomy for Ampullary Carcinoma after Clinical Resolution of Necrotizing Pancreatitis with Giant Walled-Off Necrosis: A Case Report.

INTRODUCTION: Ampullary carcinoma is typically treated with pancreaticoduodenectomy (PD). Nevertheless, performing curative resection remains remarkably challenging when the clinical course is complicated by necrotizing pancreatitis and extensive walled-off necrosis (WON). Although resection is occasionally performed after mild pancreatitis, few reports describe curative surgery following severe necrosis requiring necrosectomy. We report a rare case managed with necrosectomy for extensive WON, followed by PD after regression of the necrotic area. CASE PRESENTATION: A 77-year-old man presented

Awake pancreaticoduodenectomy without intubation: exploring short-term clinicopathological outcomes of epidural vs. general anesthesia.

BACKGROUND: There is some evidence to suggest that general anesthesia may influence oncological outcomes, such as survival and disease-free recurrence, in addition to surgical outcomes. This study compares the clinico-oncological outcomes of pancreatic cancer patients who had a pyloric-preserving pancreaticoduodenectomy (PPPD) under epidural anesthesia without endotracheal intubation (EA) and those who received general anesthesia (GA). METHODS: A retrospective cohort investigation comparing pancreatic cancer patients with PPPD under GA and EA. The procedure's feasibility and 30-day clinico-pa

Gerbode defect resulting from Group B Streptococcus infective endocarditis: a case report.

BACKGROUND: Gerbode defect is an unusual abnormal communication between the left ventricle and the right atrium and is a serious complication of aortic infective endocarditis. Group B Streptococcus is an uncommon cause of infective endocarditis and has a markedly destructive effect on valvular tissue. Acute fistulation between the left ventricle and the right atrium associated with this form of infective endocarditis is a life-threatening, aggressive complication that often requires urgent surgical intervention. However, the identification of actual communication is often extremely difficult.

The long-term outcomes of laparoscopic versus open pancreatoduodenectomy for ampullary carcinoma showed similar survival: a case-matched comparative study.

BACKGROUND: Few studies have compared the oncological benefit of laparoscopic (LPD) and open pancreatoduodenectomy (OPD) for ampullary carcinoma. The aim of this study was to compare the oncological results of these two approaches. METHODS: Between 2011 and 2020, 103 patients who underwent PD for ampullary carcinoma, including 31 LPD and 72 OPD, were retrospectively analyzed. Patients were matched on a 1:2 basis for age, sex, body mass index, American Society of Anaesthesiologists score, and preoperative biliary drainage. Short- and long-term outcomes of LPD and OPD were compared. RESULTS: T

The Application of Selective Hepatic Inflow Vascular Occlusion with Anterior Approach in Liver Resection: Effectiveness in Managing Major Complications and Long-Term Survival.

BACKGROUND: Hepatectomy is always a challenge to surgeons and requires an appropriate approach for specific tumors to achieve effective complication management. Selective hepatic pedicle clamping is more considerable strategy when comparing with total hepatic pedicle clamping in the balance between reducing blood loss and transfusion with causing the hepatic parenchyma damages (two main complications affecting liver resection result). OBJECTIVES: In this study, we aim to describe the application of selective hepatic inflow vascular occlusion (SHIVO) and anatomical anterior approach in liver r

Advances in neonatal surgery.

Neonatal surgery has reached a high degree of sophistication. We are now entering a new era of widespread screening of the unborn by means of ultrasound, with planned intrauterine, intrapartum, and immediate postpartum interventions. Many pediatric surgical centers are now focusing their investigative efforts on elucidating the cellular, molecular, and biochemical response to disease and therapeutic agents. The author presents the topic of neonatal surgery to some of the newer applications, techniques, and approaches.

Association Between Combined Preoperative Alkaline Phosphatase-Lactate Dehydrogenase Status and Postoperative Outcomes After Pancreaticoduodenectomy for Periampullary Carcinoma.

BACKGROUND: Periampullary carcinoma is a heterogeneous group of tumours arising within 2 cm of the papilla of Vater, including pancreatic, ampullary and biliary cancers, and is most commonly treated with pancreaticoduodenectomy (PD), a complex procedure associated with substantial postoperative morbidity. The combination of alkaline phosphatase (ALP) and lactate dehydrogenase (LDH) provides more information than either marker individually regarding hepatobiliary tumour burden and tumour metabolic activity, respectively. This study aimed to examine the association between combined ALP-LDH statu

Mirizzi's syndrome: identification and management strategy.

Fourteen cases of Mirizzi's syndrome are presented here. Clinical presentation was pain (14), jaundice (14), fever (10) and peritonitis (1). A clinical diagnosis of choledocholithiasis was considered in all the patients. Pre-operative diagnosis of Mirizzi's syndrome was made in five patients on the basis of cholangiogram and the remaining cases were diagnosed at surgery. The stage (type) of Mirizzi's syndrome was based on the extent of erosion of the common bile duct. Four patients had type I, seven type II and three type III lesions. Associated choledocholithiasis was present in five and acut
