Electrophysiological and inotropic characterization of a novel class III antiarrhythmic agent, GLG-V-13, in the mammalian heart.

GLG-V-13, a novel 3,7-diheterabicyclo(3.3.1)nonane, was examined both in vivo and in vitro to characterize its electrophysiological, hemodynamic, and inotropic properties. In anesthetized guinea pigs, GLG-V-13 [0.5-500 micrograms/kg intravenously (i.v.), n = 6] lengthened the epicardial monophasic action potential (MAP) duration, the atrioventricular (AV) conduction time and the RR interval in a dose-dependent manner. At the highest dose, these variables were increased by 30, 13, and 23%, respectively. No significant effects were noted on QRS duration or blood pressure (BP). In rabbit atrial a

Impact of Physiologic Pacing Versus Right Ventricular Pacing Among Patients With Left Ventricular Ejection Fraction Greater Than 35%: A Systematic Review for the 2018 ACC/AHA/HRS Guideline on the Evaluation and Management of Patients With Bradycardia and Cardiac Conduction Delay: A Report of the American College of Cardiology/American Heart Association Task Force on Clinical Practice Guidelines and the Heart Rhythm Society.

BACKGROUND: It is unclear whether physiologic pacing by either cardiac biventricular pacing (BiVP) or His bundle pacing (HisBP) may prevent adverse structural and functional consequences known to occur among some patients who receive right ventricular pacing (RVP). AIM: Our analysis sought to review existing literature to determine if BiVP and/or HisBP might prevent adverse remodeling and be associated with structural, functional, and clinical advantages compared with RVP among patients without severe left ventricular dysfunction (>35%) who required permanent pacing because of heart block. [Feasibility of combination mapping utilizing a duo-decapolar electrode catheter and the CARTO system for incisional reentrant atrial tachycardia with cycle length alternation: a case report].

A 43-year-old woman had undergone patch closure operation for atrial septal defect 27 years ago. She was referred to our hospital for evaluation of frequent palpitations since 1 year ago. Electrophysiological study was performed with recording of the coronary sinus, His bundle, and low lateral right free wall electrography utilizing a steerable duo-decapolar electrode catheter(Livewire, Daig). Supraventricular tachycardia with cycle length alternation of 300 and 320 msec similar to atrial flutter was reproducibly provoked by burst pacing from the coronary sinus. During the supraventricular tac

A rare mental foramen variation and the role of cone-beam computed tomography in preventing nerve injury: A case report.

BACKGROUND: The interforaminal space is frequently used for dental implant placement due to its strategic location between the bilateral mental foramina. However, neurosensory disturbances following surgical intervention near the mental foramen (MF) can occur in up to 43% of cases. Understanding anatomical variations of the MF is crucial to preventing surgical complications. Traditional two-dimensional radiographs have limitations in accurately visualizing anatomical structures, whereas cone-beam computed tomography (CBCT) provides high-resolution three-dimensional imaging that enhances diagno

Electrophysiology of the right anterior approach to the atrioventricular node: studies in vivo and in the isolated perfused dog heart.

INTRODUCTION: Previous reports have described electrophysiologic properties and rate-dependent responses in the transitional cell zone of the posterior AV nodal input (slow pathway). The purpose of this study was to investigate the electrophysiology of the anterior transitional cell zone (fast pathway) in vivo and in a Langendorff preparation perfused with a nonblood solution containing butanedionemonoxime to inhibit contraction. METHODS AND RESULTS: In five anesthetized dogs, the His-bundle electrogram recorded from the aortic root included atrial activity in close proximity to the anterior

The rise, fall, and resurrection of the ventromedial hypothalamus in the regulation of feeding behavior and body weight.

Early researchers found that lesions of the ventromedial hypothalamus (VMH) resulted in hyperphagia and obesity in a variety of species including humans, which led them to designate the VMH as the brain's "satiety center." Many researchers later dismissed a role for the VMH in feeding behavior when Gold claimed that lesions restricted to the VMH did not result in overeating and that obesity was observed only with lesions or knife cuts that extended beyond the borders of the VMH and damaged or severed the ventral noradrenergic bundle (VNAB) or paraventricular nucleus (PVN). However, anatomical

Optimizing Perioperative Care for Ventricular Tachycardia Ablation in High-Risk Patients Supported by Impella 5.5.

Impella 5.5 (Abiomed Inc., Danvers, MA, USA) is a surgically implanted mechanical circulatory support device that helps support hemodynamically compromised patients. The device's risks and benefits must be entirely known, especially in the electrophysiology lab. Due to unexpected hemodynamic changes during pace mapping and ablation, such as ventricular tachycardia (VT) and asystole, it is sometimes necessary to implement chemical support with inotropic agents such as epinephrine or mechanical support with devices such as an Impella. We present the case of a 72-year-old male with a biventricula

Topography of NPY-, somatostatin-, and VIP-immunoreactive, neuronal subpopulations in the guinea pig celiac-superior mesenteric ganglion and their projection to the pylorus.

The topography of the peptidergic neuronal subpopulations in the guinea pig celiac-superior mesenteric ganglion was studied analyzing the distribution of immunoreactivity to neuropeptide Y (NPY), somatostatin (SOM), and vasoactive intestinal polypeptide (VIP)/polypeptide HI (PHI). For comparison, the ganglion was also studied using antisera against the 2 catecholamine-synthesizing enzymes tyrosine hydroxylase (TH) and dopamine beta-hydroxylase (DBH). Approximately 65% of the neuronal cell bodies contained NPY-like immunoreactivity (NPY-LI), whereas 25% of the principal ganglion cells contained

The Clinical and Angiographic Profile and Outcomes of Patients With Left Bundle Branch Block (LBBB): An Observational Study From a Tertiary Care Center in North India.

Background and objective Left bundle branch block (LBBB) is a common electrocardiographic abnormality resulting from impaired conduction in both the His-Purkinje system's anterior and posterior left fascicles. LBBB prevalence varies with age, gender, race, and underlying cardiovascular conditions. It affects 0.06-0.1% of the general population, rising to 6-7% in those over 80, and is often detected incidentally in older males. Although once considered a ST-segment-elevation myocardial infarction (STEMI) equivalent in chest pain, recent data indicate a low risk of myocardial infarction. Diagnos

Capitate Osteochondral Graft for Reconstruction of Unstable Proximal Interphalangeal Joint Injury.

BACKGROUND: Hemi-hamate osteochondral grafting is a surgical technique that is utilized to reconstruct the proximal interphalangeal (PIP) joint in cases of unstable dorsal fracture-dislocation with >50% articular surface involvement. However, hemi-hamate osteochondral grafting can be technically challenging, has been reported to have various technical modifications, and can lead to complications such as overstuffing of the joint. This surgical technique article describes successful PIP joint reconstruction with use of a hemi-capitate osteochondral graft, which may offer a viable alternative to

Ajmaline Challenge To Unmask Infrahisian Disease In Patients With Recurrent And Unexplained Syncope, Preserved Ejection Fraction, With Or Without Conduction Abnormalities On Surface ECG.

Background: Pharmacological challenge with class I antiarrhythmic drug is a recommended diagnostic test in patients with unexplained syncope only in the presence of bundle branch block, when non-invasive tests have failed to make the diagnosis. Its role in patients with minor or no conduction disturbances on 12-leads ECG has not been evaluated yet. It is also not clear which are the values of His-Ventricular interval to be considered diagnostic. We sought to evaluate the role of ajmaline challenge in unmasking the presence of an infrahisian disease in patients with recurrent and unexplained sy

Myocardial inflammatory infiltrate in human chronic chagasic cardiomyopathy: Immunohistochemical findings.

Chagas' disease is the most common form of chronic myocarditis in the world. It is characterized by a progressive chronic myocarditis that leads to cardiomegaly, arrhythmias, cardiac failure, and thromboembolic phenomena. This communication reports studies on the immunohistochemistry of chronic infiltrates in 30 endomyocardial biopsies and in contracting and specialized myocardium of autopsies of four patients suffering from Chagas' cardiomyopathy. Expression of the following antigens was studied: common leucocyte antigen (CLA-CD45R), L-26(CD20), CD68, kappa and lambda light chains and T-UCLH-

Ventricular pacing burden in patients with left bundle branch block after transcatheter aortic valve replacement therapy.

INTRODUCTION: Electrophysiological testing has been proposed in the latest European Society of Cardiology (ESC) guidelines for cardiac pacing to identify left bundle branch block (LBBB) patients with infrahisian conduction delay (IHCD) after transcatheter aortic valve replacement (TAVR). While in general IHCD is defined by a His-ventricular (HV) interval of >55 ms, a cut-off of ≥70 ms to trigger pacemaker (PM) implantation has been proposed in the latest ESC guidelines. The ventricular pacing (VP) burden during follow-up in such patients is largely unknown. As such, we aimed to assess the VP b

Radiofrequency catheter ablation of atrial arrhythmias. Results and mechanisms.

BACKGROUND: Radio frequency catheter ablation is accepted therapy for patients with paroxysmal supraventricular tachycardia and has a low rate of complications. For patients with atrial arrhythmias, catheter ablation of the His bundle has been an option when drugs fail or produce untoward side effects. Although preventing rapid ventricular response, this procedure requires a permanent pacemaker and does not restore the atrium to normal rhythm. Therefore, we evaluated the safety and efficacy of radiofrequency ablation directed at the atrial substrate. METHODS AND RESULTS: Thirty-seven patients

Radiofrequency catheter modification of the sinus node for "inappropriate" sinus tachycardia.

BACKGROUND: Radiofrequency catheter ablation is the treatment of choice for patients with paroxysmal supraventricular tachycardias refractory to medical therapy. However, in symptomatic patients with inappropriate sinus tachycardia resistant to drug therapy, catheter ablation of the His' bundle with permanent pacemaker insertion is currently applied. We evaluated the safety and efficacy of radiofrequency modification of the sinus node as alternative therapy for patients with inappropriate sinus tachycardia. METHODS AND RESULTS: Sixteen patients with disabling episodes of inappropriate sinus t

[Antiarrhythmic therapy in patients with heart failure].

In patients with severe chronic heart failure, many deaths are sudden due to life-threatening ventricular arrhythmias. Supraventricular arrhythmias such as paroxysmal or chronic atrial fibrillation may also cause serious complications in those patients due to acute loss of atrial contraction, pump failure during rapid ventricular response and embolic events. Two therapeutic strategies are currently available for therapy and prevention of malignant ventricular arrhythmias and subsequent sudden arrhythmic death: antiarrhythmic drug therapy and implantable defibrillators. However, selection of th
