OS040. Effect of folic acid supplementation in pregnancy on preeclampsia- Folic acid clinical trial (FACT).

INTRODUCTION: Observational studies suggest that folic acid supplementation during pregnancy can reduce the risk of preeclampsia (PE). No randomized controlled trial has been conducted to demonstrate the effect of folic acid supplementation on PE. OBJECTIVES: FACT aims to determine efficacy on a new PE prevention strategy of high dose folic acid supplementation from early pregnancy (8(0/7) to 16(6/7)weeks of gestation) until delivery in women with high risk of developing PE. DESIGN: FACT is an international, multi-centre, double-blind, placebo controlled clinical trial of 3656 women. Eligibl

Exploring clinical conundrums in obstetric anesthesia through interactive polls and panel discussion: insights from Canadian obstetric anesthesiology experts.

PURPOSE: This Special Article aims to synthesize the results of a live audience poll and practice tips from Canadian obstetric anesthesiology experts during a panel session at the Canadian Anesthesiologists' Society 2024 Annual Meeting (Victoria, BC, Canada). We explored six hypothetical case scenarios, each representing a clinically plausible situation that lack a definitive management approach. These scenarios highlight areas where no consensus exists and no single "correct" solution has been established. SOURCE: We gathered live poll data about six case scenarios from participants who atte

General Anesthesia for Cesarean Delivery for Thrombocytopenia in Hypertensive Disorders of Pregnancy: Findings From the Obstetric Airway Management Registry.

BACKGROUND: In resource-limited environments, spinal anesthesia (SA) is preferred for cesarean delivery. In women at risk of spinal epidural hematoma, particularly those with hypertensive disorders of pregnancy, thrombocytopenia should be excluded before neuraxial blockade. In the context of emergency surgery for fetal distress, this investigation may be hampered by laboratory services being unavailable or off-site. METHODS: The Obstetric Airway Management Registry (ObAMR) is currently active across all anesthesia training institutions affiliated with the University of Cape Town. This multice

Bioimpedance cardiography in pregnancy: A longitudinal cohort study on hemodynamic pattern and outcome.

BACKGROUND: Pregnancy associated cardiovascular pathologies have a significant impact on outcome for mother and child. Bioimpedance cardiography may provide additional outcome-relevant information early in pregnancy and may also be used as a predictive instrument for pregnancy-associated diseases. METHODS: We performed a prospective longitudinal cohort trial in an outpatient setting and included 242 pregnant women. Cardiac output and concomitant hemodynamic data were recorded from 11(th)-13(th) week of gestation every 5(th) week as well as at two occasions post partum employing bioimpedance c

Maternal and feto-placental phenotypes of early-onset severe preeclampsia.

OBJECTIVE: To characterize maternal and feto-placental phenotypes of severe preeclampsia that trigger early-onset delivery. METHODS: A retrospective cohort review of pregnant women receiving care from 2000 to 2010. Subjects with early-onset severe preeclampsia delivering between 20 and 32 weeks were identified excluding multiple gestations or major anomalies. We defined indications for delivery as maternal (i.e. severe headache or abnormal laboratory parameters), feto-placental (i.e. non-reassuring tracing) or mixed (i.e. both maternal and feto-placental factors). To characterize the groups,

Do intrauterine growth restricted fetuses of the hypertensive and normotensive mothers differ from each other?

PURPOSE: To investigate if normotensive and hypertensive patients with intrauterine growth restricted (IUGR) fetuses were different with respect to maternal and fetal characteristics and Doppler flow. METHODS: The records of patients with IUGR fetuses who had to be delivered before 34th gestational week because of fetal distress were examined. Early Doppler abnormalities were defined as increased umbilical artery resistance and redistribution of blood flow in the middle cerebral artery while late Doppler abnormalities were defined as the absence or reversal of umbilical artery blood flow and

Decreased expression of the angiogenic regulators CYR61 (CCN1) and NOV (CCN3) in human placenta is associated with pre-eclampsia.

The pregnancy disorder pre-eclampsia (PE) is thought to be caused in part by shallow invasion of the extravillous trophoblast (EVT) leading to uteroplacental insufficiency and hypoxia. Here, we focused on the expressions of cysteine-rich 61 (CYR61, CCN1) and nephroblastoma overexpressed (NOV, CCN3), members of the CCN family of angiogenic regulators, in human placenta during normal pregnancy compared with pre-eclamptic and HELLP placentae using quantitative RT-PCR, western blotting and immunocytochemistry. During normal pregnancy, both proteins showed increasing expression levels and were stro

How I treat thrombocytopenia in pregnancy.

A mild thrombocytopenia is relatively frequent during pregnancy and has generally no consequences for either the mother or the fetus. Although representing no threat in the majority of patients, thrombocytopenia may result from a range of pathologic conditions requiring closer monitoring and possible therapy. Two clinical scenarios are particularly relevant for their prevalence and the issues relating to their management. The first is the presence of isolated thrombocytopenia and the differential diagnosis between primary immune thrombocytopenia and gestational thrombocytopenia. The second is

Preeclampsia and eclampsia: the conceptual evolution of a syndrome.

Preeclampsia, one of the most enigmatic complications of pregnancy, is considered a pregnancy-specific disorder caused by the placenta and cured only by delivery. This article traces the condition from its origins-once thought to be a disease of the central nervous system, recognized by the occurrence of seizures (ie, eclampsia)-to the present time when preeclampsia is conceptualized primarily as a vascular disorder. We review the epidemiologic data that led to the recommendation to use diastolic hypertension and proteinuria as diagnostic criteria, as their combined presence was associated wit

Management of pre-eclampsia: issues for anaesthetists.

Pre-eclampsia is a leading cause of maternal morbidity and mortality. Substandard care is often present and many deaths are preventable. The aim of this review is to summarise the key management issues for anaesthetists in the light of the current literature. A systematic literature search of electronic databases was undertaken including MEDLINE, EMBASE and the Cochrane Library using the key words obstetrics, pregnancy, pregnancy complications, maternal, pre-eclampsia, preeclampsia, cardiac function, haemodynamics, haemolysis, elevated liver enzymes, low platelets (HELLP), eclampsia, anaesthes

Obstetric management of gastroenterologic complications of pregnancy.

Key elements in the management of gastroenterologic complications during pregnancy are knowledge of the natural history of the disease process, understanding the effects of pregnancy on the disease and of the disease on pregnancy, concern about fetal effects of medications, careful antepartum fetal assessment, and appropriate timing of the delivery. Vaginal delivery is preferred in most pregnancies complicated by gastrointestinal disease.

The Impact of Coronary Artery Disease on Outcomes in Patients Hospitalized With Pre-eclampsia.

Introduction Pre-eclampsia leads to long-lasting cardiovascular effects in women in the postpartum period, but prevalence and in-hospital adverse events of coronary artery disease (CAD) in women with pre-eclampsia are poorly understood. The prevalence, outcomes, and mortality risks identified in this study allow for possible routes of clinical intervention of CAD in women with pre-eclampsia. The purpose of this study was to determine the prevalence and outcomes of CAD in women diagnosed with pre-eclampsia compared to those with pre-eclampsia with no history of CAD. Predictors of mortality in p

Prevalence and association of placental lesions with obstetrical features and outcome: data from French prospective study.

PURPOSE: Since the Consensus Statement diffused by the Amsterdam Placental Workshop Group, knowledge of the meaning of placental vascular malperfusion has become essential in the unavoidable analysis of obstetrical history in a patient followed for autoimmune disease or any other maternal comorbidity. We aimed to analyse the prevalence of various placental lesions from a 6-months prospective observational study and to correlate the various placental profiles to obstetrical outcome, maternal diseases and pregnancy treatments. The frequency of foetal vascular malperfusion lesion could be estimat
