Leprosy, a chronic infectious disease caused by Mycobacterium leprae complex, remains a significant global health concern despite being curable with multidrug therapy. Delayed diagnosis is common, particularly in non-endemic regions or when presenting with atypical symptoms. This can lead to missed opportunities for early intervention, potential disabilities, and increased transmission. Misdiagnosis is often compounded by leprosy's ability to mimic other conditions, as illustrated in this case report. We present a 43-year-old Filipino woman residing in Eastern Saudi Arabia, who presented to a

BACKGROUND: Tuberculosis (TB) otitis media is an uncommon site of extrapulmonary TB and can primarily present as a complicated TB mastoiditis (TBM). This complication is rare in children, even in TB endemic areas but necessitates early identification as delays can lead to severe morbidities. We describe the clinical characteristics as a case series to raise awareness of the condition, and highlight fundamentals related to diagnosis and management.
METHODS: A retrospective chart review of clinical and radiological information of five children with TBM seen at the Red Cross War Memorial Childre

INTRODUCTION: Chronic immune activation due to ongoing HIV replication may lead to impaired immune responses against opportunistic infections such as tuberculosis (TB). We studied the role of HIV replication as a risk factor for incident TB after starting antiretroviral therapy (ART).
METHODS: We included all HIV-positive adult patients (≥16 years) in care between 2000 and 2014 at three ART programmes in South Africa. Patients with previous TB were excluded. Missing CD4 cell counts and HIV-RNA viral loads at ART start (baseline) and during follow-up were imputed. We used parametric survival m

BACKGROUND: Early mortality among HIV-positive adults starting antiretroviral therapy (ART) remains high in resource-limited settings, with tuberculosis (TB) the leading cause of death. However, current methods to estimate TB-related deaths are inadequate and most autopsy studies do not adequately represent those attending primary health clinics (PHCs). This study aimed to determine the autopsy prevalence of TB and other infections in adults enrolled at South African PHCs in the context of a pragmatic trial of empiric TB treatment ("TB Fast Track").
METHODS AND FINDINGS: Adults with CD4 ≤150

BACKGROUND: Mycobacterium tuberculosis (TB) infection of the liver, known as hepatic TB, is an extrapulmonary manifestation of TB. Hepatic TB has become more prevalent, likely as a result of the human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) epidemic. We sought to review case series to characterize the epidemiology, pathophysiology, clinical features, diagnosis, and treatment of hepatic TB and to comment on the impact of HIV co-infection on these characteristics.
METHODS: We conducted a systematic literature search in PubMed and ScienceDirect for articles pertainin

BACKGROUND: The incidence of tuberculosis (TB) in developed countries has decreased since the 1990s, reflecting worldwide efforts to identify and treat TB according to WHO recommendations. However TB remains an important public health problem in industrialized countries with a high proportion of cases occurring among subjects originating from high prevalence countries. The aim of this study was to describe clinical and social characteristics of patients with TB and their outcome in a low incidence area with a high immigration rate.

BACKGROUND: Tuberculous lymphadenopathy (TB-LAP) is the most common form of extrapulmonary TB and remains diagnostically challenging due to its paucibacillary nature. We evaluated the diagnostic performance of droplet digital PCR (ddPCR) for detecting Mycobacterium tuberculosis (Mtb) DNA in formalin-fixed paraffin-embedded (FFPE) lymph node specimens.
METHODS: We retrospectively analysed 102 lymph node FFPE specimens with microbiologically confirmed or pathology-suspected TB-LAP. We performed ddPCR targeting the IS6110 gene and correlated it with pathological findings and mycobacterial cultur

BACKGROUND: Tuberculosis (TB) remains a major global public health challenge, significantly impacting morbidity and mortality. Assessing the patient-reported Health-Related Quality of Life (HRQoL) is crucial for understanding the comprehensive burden of TB. This study aimed to evaluate HRQoL and its associated factors among adult TB patients in the Kembata Zone, Southern Ethiopia. METHODS: A concurrent triangulation mixed-methods study was conducted from December 1 to 30, 2023. A multi-stage sampling technique was used to select 597 participants from health facilities across the zone. Quantita

INTRODUCTION: Bedaquiline (BDQ) has revolutionized multidrug-resistant tuberculosis (MDR-TB) management in the Indian population with a high MDR-TB burden. However, its potential cardiotoxicity in the form of QTc prolongation warrants careful monitoring. This study aims to evaluate the prevalence, severity, and risk factors of BDQ-related QTc prolongation in MDR/rifampicin-resistant (RR)-TB patients. Given the genetic variability and diverse environmental factors, extrapolating foreign data to Indian patients is challenging; thus, local evidence is crucial.
METHODS: A prospective analytical s

BACKGROUND: Tuberculous pleurisy represents a prevalent form of extrapulmonary tuberculosis and constitutes a significant diagnostic challenge in clinical practice, particularly in endemic regions where it accounts for approximately 20-30% of all PEs. The nonspecific clinical presentation and the limitations of conventional diagnostic methods-including the low sensitivity and prolonged turnaround time of pleural fluid culture-often result in delayed diagnosis and treatment, potentially compromising patient outcomes.
OBJECTIVE: To develop and validate a comprehensive, evidence-based predictive

Background: Tuberculosis (TB) remains a major cause of morbidity and mortality among children worldwide, with approximately one million new pediatric cases annually. The conventional treatment for drug-susceptible TB has long relied on a 6-month multidrug regimen, which is highly effective but associated with challenges in adherence, toxicity, and healthcare burden. Objectives: To evaluate whether short-course therapy is an appropriate regimen for children and young adolescents with drug-susceptible TB, with particular focus on its efficacy, safety, and applicability in different clinical cont

BACKGROUND: Genitourinary tuberculosis (GUTB) is a common form of extrapulmonary TB (EPTB) in children. An example of GUTB is epididymal TB, which usually presents unspecific chronic clinical manifestations. Definitive diagnosis can be conducted based on bacteriologic confirmation and histopathologic results, but this is challenging due to the paucibacillary nature of EPTB. Therefore, we reported the challenges in diagnosing isolated epididymal TB in an adolescent male.
