Charcoal haemoperfusion in amitriptyline poisoning: experience in 20 children.

AIM: Tricyclic antidepressant (TCA) toxicity is common among children and adults due to widespread use. Amitriptyline (AT) is one of the most commonly prescribed TCAs. Current guidelines do not recommend charcoal haemoperfusion (HP) for AT overdose due to high protein binding and large volume of distribution. However evidence regarding the efficacy of charcoal HP in addition to supportive measures is accumulating in the published reports. METHODS: Here we report our experience in 20 children (15 girls, 5 boys) with acute AT overdose aged between 1.5 and 15 years, successfully managed with HP

Toxicological and clinical aspects of cyanide metabolism.

This contribution deals with the occurrence of cyanide and its biological pathways in the body. Especially possibilities of detoxification are pointed out. Intoxications are caused by acute and chronical cyanide uptake. Tobacco amblyopia, retrobulbar neuritis in pernicious anaemia, Leber's optic atrophy, Nigerian nutritional neuropathy, and sterility in female heavy smokers are attributed to cyanide intoxication. Various methods for treating acute and chronic cyanide intoxication are discussed.

A large pediatric cohort of colchicine intoxication: prognostic factors and experience with rapid sequence apheresis.

INTRODUCTION: Severe colchicine intoxication often leads to multi-organ failure and death. With no specific antidote and ineffective conventional elimination methods, we hypothesized that rapid sequence apheresis could reduce the intravascular colchicine reservoir. This study describes pediatric colchicine poisoning, focusing on dose-related outcomes and the clinical role of rapid sequence apheresis. METHODS: This retrospective observational analysis included children admitted to a tertiary pediatric intensive care unit for colchicine poisoning (2011-2025). Clinical data, ingested dose, organ

Malic Enzyme 1 Limits Acetaminophen-Induced Liver Injury by Sustaining Redox and Bioenergetic Homeostasis.

Background: Acetaminophen (APAP) overdose remains a major cause of acute liver injury. Although N-acetylcysteine (NAC) is the clinically established antidote for APAP toxicity, its efficacy is greatest when administered early, and additional therapeutic strategies are still needed for patients with delayed presentation or progressive injury. Because APAP hepatotoxicity involves coupled disturbances in redox control, mitochondrial performance, and cellular metabolism, metabolic enzymes that sustain NADPH availability may critically influence disease severity. Malic enzyme 1 (ME1), a cytosolic N

Serum benzylpenicillin levels during treatment for amatoxin poisoning: pharmacokinetic support for OATP1B3 inhibition.

INTRODUCTION: Benzylpenicillin has long been included in treatment protocols for amatoxin poisoning based on experimental evidence suggesting inhibition of hepatocellular amatoxin uptake via OATP1B3. However, no human pharmacokinetic data have confirmed whether therapeutic regimens achieve serum concentrations sufficient to inhibit this transporter in vivo. METHODS: We retrospectively analyzed serum benzylpenicillin concentrations in 16 patients with confirmed amatoxin poisoning treated at the Florence Poison Control Center between 2020 and 2024. All patients received continuous intravenous b

Generation and distribution of cyanide in rats and humans following sodium cyanoborohydride exposure.

Sodium cyanoborohydride (NaBH3CN) is a commonly used reducing agent. However, its metabolism and mechanism of toxicity are still unclear. In this study, we first demonstrated that NaBH3CN underwent rapid hydrolysis in aqueous solution and produced hydrocyanic acid. Cyanide was detected in the gastric contents of rats following oral administration of NaBH₃CN, and cyanide poisoning symptoms such as tachypnea, limb convulsions, and opisthotonos were observed. Combined with a gas chromatography-mass spectrometry (GC-MS) method and rat poisoning models, we systematically investigated the metabolism

Acute rotenone poisoning: A scoping review.

CONTEXT: Rotenone is a toxic chemical found in various plants, including some used as food. Rotenone poisoning can be fatal and there is no antidote. Mechanistically, rotenone inhibits mitochondrial complex I, leading to reduced ATP production, compensatory glycolytic upregulation and secondary lactate production, and oxidative stress. Our literature review examined acute rotenone poisoning in humans, including exposure scenarios, clinical presentations, and treatments. METHODS: We searched five databases for relevant literature from database inception through the search date: July 12, 2022,

Aminopyridines Restore Ventilation and Reverse Respiratory Acidosis at Late Stages of Botulism in Mice.

Botulinum neurotoxin (BoNT) is a potent protein toxin that causes muscle paralysis and death by asphyxiation. Treatments for symptomatic botulism are intubation and supportive care until respiratory function recovers. Aminopyridines have recently emerged as potential treatments for botulism. The clinically approved drug 3,4-diaminopyridine (3,4-DAP) rapidly reverses toxic signs of botulism and has antidotal effects when continuously administered in rodent models of lethal botulism. Although the therapeutic effects of 3,4-DAP likely result from the reversal of diaphragm paralysis, the correspon

A study protocol for a European, mixed methods, prospective, cohort study of the effectiveness of naloxone administration by community members, in reversing opioid overdose: NalPORS.

BACKGROUND: Worldwide, opioid use causes more than 100,000 overdose deaths annually. Naloxone has proven efficacy in reversing opioid overdoses and is approved as an emergency antidote to opioid overdose. Take home naloxone (THN) programmes have been introduced to provide 'community members', who are likely to observe opioid overdoses, with naloxone kits and train them to recognise an overdose and administer naloxone. The acceptability and feasibility of THN programmes has been demonstrated, but the real-life effectiveness of naloxone administration by community members is not known. In recent

Russell’s viper venom induced nephrotoxicity, myotoxicity, and hepatotoxicity—Neutralization with gold nanoparticle conjugated 2-hydroxy-4-methoxy benzoic acid in vivo.

Snakebite is one of the major neglected tropical diseases and health hazard that leads to significant mortality, particularly in rural populations of tropical and subtropical countries including India. Antisnake venom serum (ASVS) is the only specific treatment against snake envenomation. Available treatment i.e. ASVS have many limitations not only low efficiency but also considerable side effects. Search for alternative ASVS is a major domain in toxinology research. Targeted drug therapy using nanoparticles, an emerging area of nanotechnology, is one such alternative. Here, we studied neutral

Invasive Lionfish (Pterosis volitans) Pose Public Health Threats.

The lionfish, Pterosis volitans, a native of Indo-Pacific oceans, is a popular saltwater aquarium fish despite venomous spines on its fins. Lionfish were inadvertently introduced into the western Atlantic from Florida in the early 1990s and have overpopulated and dispersed widely into the Caribbean Sea and Gulf of Mexico. Initiatives to control lionfish populations were launched, including the National Oceanographic and Atmospheric Administration (NOAA)-sponsored "Lionfish as Food Campaign".2 Recently, scientists from the Food and Drug Administration (FDA) reported that lionfish caught off the

Dabigatran and atrial fibrillation: the alternative to warfarin for selected patients.

For patients with atrial fibrillation and a high risk of thrombosis, the standard prophylaxis is warfarin, an anticoagulant, at a dose adjusted to the INR. Warfarin and aspirin are both reasonable choices for patients with a moderate risk of thrombosis. Dabigatran, an oral anticoagulant that inhibits thrombin, has been authorised for patients with atrial fibrillation and a moderate or high risk of thrombosis, without associated valvular abnormalities. Clinical evaluation of dabigatran is based on a randomised "non-inferiority" trial comparing two doses of dabigatran (110 mg and 150 mg) taken t

N-acetylcysteine pretreatment ameliorates mercuric chloride-induced oxidative renal damage in rats.

The effectiveness of the antioxidant thiol, N-acetylcysteine (NAC), in enhancing methylmercury (CH3HgCl) excretion and its utility as a possible antidote in CH3HgCl poisoning has been reported. NAC, however, has been reported to be ineffective in accelerating excretion of divalent toxic metals, including inorganic mercury, Hg2+. In this study, we evaluated the possible protective effect of short-term pretreatment with NAC against mercuric chloride (HgCl2) toxicity in rat model. This is aimed at determining its chemopreventive or prophylactic benefit in situations of high risk exposure (occupat

[Aromatic trees and herbs that connect Heaven and earth].

It has been known for a long time that aromatic substances (essential oils) contained in plants often exert psychological effects ranging from sedative to excitatory actions. Medicinal effects have also been confirmed through numerous experiences. In ancient times, aromatic trees and herbs were offered to deities, mostly as incenses that were believed to carry people's wishes, such as requests to cure sick people, to Heaven. In the medieval periods, their deep and subtle aromas elevated aromatics to so-called "treasures of the world," while their various medicinal activities including the psyc

Liver.

The liver's unique metabolism and relationship to the gastrointestinal tract make it an important target of the toxicity of drugs and xenobiotics. The developmental changes that occur in the liver's metabolic activity from birth to adolescence contribute to the varied sensitivity to toxins seen in the pediatric population. Hepatic drug metabolism, often with an imbalance between the generation of toxic metabolites and detoxification processes, can influence the degree of hepatotoxicity. The decreased capacity of the neonatal liver to metabolize, detoxify, and excrete xenobiotics explains the p
