Outcome of pancreaticoduodenectomy with pylorus preservation or with antrectomy in the treatment of chronic pancreatitis.

OBJECTIVE: To compare the short- and long-term results of pancreaticoduodenectomy with pylorus preservation (PPPD) or with antrectomy (Whipple procedure) in the treatment of selected patients with chronic pancreatitis. BACKGROUND: PPPD may be preferred over Whipple because of its purported nutritional advantages and the reduced likelihood of postgastrectomy syndromes. METHODS: A retrospective review was performed of 72 consecutive patients undergoing pancreaticoduodenectomy for chronic pancreatitis between 1991 and 1997. RESULTS: PPPD was performed in 39 patients and Whipple in 33. The two

Can biliary-cyst communication be predicted before surgery for hepatic hydatid disease: does size matter?

BACKGROUND: The aim of this study was to determine if there is any predictive factor indicating the risk of bile leakage before surgery for hepatic hydatid disease in clinically asymptomatic patients. METHODS: The data of 116 patients who underwent surgery for hepatic hydatid disease were reviewed retrospectively. There were 43 men (37%) and 73 women (63%) with a mean age of 45 +/- 15 years. Because of high preoperative serum bilirubin and liver function test levels, 12 patients were excluded from the study. These patients underwent preoperative endoscopic retrograde cholangiopancreatography.

Development of intrapulmonary arteriovenous shunting in postoperative biliary atresia: evaluation by contrast-enhanced echocardiography.

PURPOSE: This report presents biliary atresia (BA) patients with intrapulmonary arteriovenous shunting (IPS), which was evaluated suitably by contrast-enhanced echocardiography (CEC). METHODS: Of 88 BA patients seen in the last 20 years, 8 (9.1%) had IPS at 8 months to 16 years of age. Two were associated with polysplenia syndrome, 1 had persistent jaundice after hepatic portoenterostomy, and 2 underwent splenorenal shunt. According to the comparison between microbubbles in left atrium (LA) and in right atrium (RA) detected by CEC, IPS was classified as grade I, mild (LA << RA); grade II, mod

Pancreatectomy with reconstruction of the right and left hepatic arteries for locally advanced pancreatic cancer.

BACKGROUND/PURPOSE: The resectability of locally advanced pancreatic cancer depends upon, before anything else, the relationship between the tumor and the adjacent arterial structure. Pancreatic cancer that has developed at the caudal side of the pancreas can invade the common hepatic artery (CHA). Pancreatic cancers with CHA involvement can become candidates for surgery in selected cases. Pancreatic cancer arising at the caudal side of the pancreas head may sometimes invade the right and left hepatic arteries (RLHA) as well as the CHA. Pancreatic cancer with RLHA involvement may be assessed a

Multiphase helical CT findings after percutaneous ablation procedures for hepatocellular carcinoma.

BACKGROUND: Multiple-phase helical computed tomography (CT) has been regarded as the method of choice in the evaluation of patients with hepatocellular carcinoma (HCC) treated by nonsurgical procedures. The aim of this article was to report our experience in the assessment of nodular and parenchymal changes recognizable after various percutaneous ablation therapies. METHODS: We reviewed the studies of 116 consecutive patients with HCC treated with multisession percutaneous ethanol injection (56 patients, 98 nodules), single-session percutaneous ethanol injection (14 patients, 31 nodules), rad

Surgical technique and results of intrapancreatic bile duct resection for hilar malignancy (with video).

BACKGROUND: Hilar malignancy can occasionally be associated with high grade dysplasia (HGD) adjacent to invasive malignancy. For patients with HGD extending into the intrapancreatic bile duct, the authors adopted intrapancreatic bile duct resection (IP-BDR). The aims of this study were to compare the incidence of clinically relevant postoperative pancreatic fistula (CR-POPF), distal R0 resection and local recurrence within the distal bile duct remnant for patients undergoing extrahepatic bile duct resection without pancreaticoduodenectomy (with or without IP-BDR). METHODS: Patients who presen

Indocyanine green fluorescence-guided surgery in acute settings: state-of-the art.

BACKGROUND: Operating in acute surgical settings is significantly different from elective surgery. One aspect is the complexity of acutely ill or injured patients with abnormal physiology, unoptimized comorbidities, and unplanned interventions. Traditional visual and tactile assessments of the surgical field anatomy and tissue viability have documented limitations. The need for integrated technology to augment such assessments is described. Indocyanine green fluorescence-guided surgery (ICG-FGS) is a promising tool, given its history, safety, low cost, and accumulated experience in elective se

Routine abdominal drainage versus no drainage after elective hepatectomy: A systematic review and meta-analysis.

INTRODUCTION: The value of routine abdominal drainage after hepatectomy remains controversial. Although drains have traditionally been used to detect bile leakage or postoperative bleeding and to evacuate intra-abdominal collections, their routine use after elective liver resection has increasingly been questioned. MATERIALS AND METHODS: A systematic review and metaanalysis of randomized controlled trials comparing routine abdominal drainage with no drainage after elective hepatectomy was performed. PubMed/MEDLINE, Embase, Web of Science, and the Cochrane Library were searched from inception

Microbiological colonization of the pancreatic tumor affects postoperative complications and outcome after pancreatic surgery.

BACKGROUND: The patient´s microbiome has become a focal point in cancer research. Even for pancreatic cancer, alterations in the microbiome appear to influence cancer formation and progression. The aim of our single-center analysis was the examination of microbiological colonization of pancreas tissue at the time of surgery and its potential influence on complications and outcome. METHODS: We prospectively evaluated patients undergoing pancreatic surgery over a three-year period from June 2018 to June 2021. We focused on the microbiological colonization of pancreatic tissue which was acquired

Italian survey about intraperitoneal drain use in distal pancreatectomy.

Intraperitoneal prophylactic drain (IPD) use in distal pancreatectomy (DP) is still controversial. A survey was carried out through the Italian community of pancreatic surgeons using institutional emails, Twitter, and Facebook accounts of the Italian Association for the Study of the Pancreas (AISP) and the Italian Association of Hepato-biliary-pancreatic Surgery (AICEP). The survey was structured to learn surgeons' practice in using IPD through questions and one clinical vignette. Respondents were asked to report their regrets for omission and commission regarding the IPD use for the clinical

Radiological Management of Chyle Leaks Following Oesophagectomy: A Systematic Review.

A chyle leak is a rare but potentially life-threatening complication following oesophagectomy. If untreated, it can result in hypovolaemia, malnutrition, electrolyte imbalance, and immunosuppression. Traditionally managed with surgical ligation, radiological interventions have emerged as less-invasive alternatives, though their comparative efficacy remains uncertain. This paper aims to systematically review the literature on the efficacy, safety, and clinical outcomes of radiological interventions for managing chyle leaks following oesophagectomy. An electronic search of PubMed (Medline) was p

Comparison of the clinical effects of dual-modality endoscopy and traditional laparotomy for the treatment of intra- and extrahepatic bile duct stones.

BACKGROUND: Bile duct stones (BDSs) may cause patients to develop liver cirrhosis or even liver cancer. Currently, the success rate of surgical treatment for intrahepatic and extrahepatic BDSs is not satisfactory, and there is a risk of postoperative complications. AIM: To compare the clinical effects of dual-modality endoscopy (duodenoscopy and laparoscopy) with those of traditional laparotomy in the treatment of intra- and extrahepatic BDSs.
