Absence of Collaterals is Associated with Larger Infarct Volume and Worse Outcome in Patients with Large Vessel Occlusion and Mild Symptoms.

BACKGROUND: Mechanical thrombectomy is the standard of care for patients with large vessel occlusion (LVO) presenting with severe symptoms; however, little is known about the best treatment for patients with LVO and mild symptoms. The absence of good collaterals has been associated with a worse outcome in patients with LVO. In this study, we aim to assess the use of collateral score to identify patients with LVO and mild symptoms that might benefit from mechanical thrombectomy (MT). METHODS: A retrospective review of prospectively collected data on patients presenting with mild ischemic strok

Factors associated with the timing of the first prenatal ultrasound in Canada.

BACKGROUND: The aim of this study was to investigate the factors associated with the timing of the first prenatal ultrasound in Canada. METHODS: This was a secondary data analysis of the Maternity Experiences Survey, a cross-sectional survey covering different aspects of pregnancy, labour, birth and the post-partum period. Bivariate and multivariate multinomial logistic regressions were performed to assess the relationship between timing of first prenatal ultrasound and different independent variables. RESULTS: 68.4% of Canadian women received an optimally timed first prenatal ultrasound, 27

Glycemic Control Status After Percutaneous Coronary Intervention and Long-Term Clinical Outcomes in Patients With Type 2 Diabetes Mellitus.

BACKGROUND: Data on the association between glycemic control after percutaneous coronary intervention and clinical outcomes are limited and controversial in diabetic patients. METHODS AND RESULTS: We studied 980 patients with type 2 diabetes mellitus undergoing percutaneous coronary intervention using drug-eluting stents. Based on 2-year glycosylated hemoglobin A (HbA1c) levels, we divided patients into 2 groups of HbA1c<7.0 (n=489) and HbA1c≥7.0 (n=491). Propensity score-matched analysis was performed in 322 pairs. The primary outcome was major adverse cardiac and cerebrovascular events (MAC

Vascular Parkinsonism in a Tertiary Care Stroke Prevention Clinic and the Development of a New Screening Strategy.

OBJECTIVES: To determine the prevalence of vascular parkinsonism (VasP) in a stroke prevention clinic (SPC). BACKGROUND: VasP can be defined by an onset of parkinsonism with prominent gait problems occurring within 1 year of stroke. METHODS: We created a screening strategy based on the Tanner Questionnaire (TQ), a validated scale for parkinsonism, and the creation of a 4-point Five-Minute Assessment Scale (FMAS) operationalizing Zijlmans' criteria for the diagnosis of VasP. Consecutive stroke patients were screened over a 12-month period using the TQ and the FMAS. SAS statistical software wa

Surgical practice variation and outcomes regarding single-level posterior approach for degenerative lumbar spondylolisthesis: a Canadian Spine Outcomes Research Network (CSORN) study.

BACKGROUND CONTEXT: Symptomatic degenerative lumbar spondylolisthesis (DLS) is a common pathology for spinal surgery. Due to the high surgical volume and variability in management approaches, analyzing and understanding practice patterns is essential for improving patient care. PURPOSE: To investigate the extent of surgical practice variation in the treatment of Meyerding Grade I DLS among Canadian spine centers and compare outcomes between decompression alone and decompression with instrumented fusion. STUDY DESIGN: We conducted a multicenter retrospective review of collected data on consec

Outcomes of Implementing a Webinar-Based Strategy to Improve Spinal Cord Injury Knowledge and Community Building: Convergent Mixed Methods Study.

BACKGROUND: COVID-19 disrupted services received by persons with spinal cord injury (SCI) worldwide. The International Disability Alliance declared the need for a disability-inclusive response to the COVID-19 crisis, as decreased access to health care services for individuals living with varying levels of function was unacceptable. As a result, an SCI community in Canada created a novel webinar-based strategy aimed at improving access to self-management information for people living with SCI and other stakeholders. However, although telehealth practices have previously been used effectively in

Endovascular Therapy for Acute Stroke with a Large Ischemic Region.

BACKGROUND: Endovascular therapy for acute ischemic stroke is generally avoided when the infarction is large, but the effect of endovascular therapy with medical care as compared with medical care alone for large strokes has not been well studied. METHODS: We conducted a multicenter, open-label, randomized clinical trial in Japan involving patients with occlusion of large cerebral vessels and sizable strokes on imaging, as indicated by an Alberta Stroke Program Early Computed Tomographic Score (ASPECTS) value of 3 to 5 (on a scale from 0 to 10, with lower values indicating larger infarction).

The Prognostic Value of CT Angiography and CT Perfusion in Acute Ischemic Stroke.

BACKGROUND: CT angiography (CTA) and CT perfusion (CTP) are important diagnostic tools in acute ischemic stroke. We investigated the prognostic value of CTA and CTP for clinical outcome and determined whether they have additional prognostic value over patient characteristics and non-contrast CT (NCCT). METHODS: We included 1,374 patients with suspected acute ischemic stroke in the prospective multicenter Dutch acute stroke study. Sixty percent of the cohort was used for deriving the predictors and the remaining 40% for validating them. We calculated the predictive values of CTA and CTP predic

Long-term outcomes of endovascular thrombectomy vs. medical care in patients with large ischemic stroke: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Several randomized controlled trials (RCTs) have shown endovascular thrombectomy (EVT) to be superior to standard medical care (MC) for anterior circulation large vessel occlusion (LVO)-related large ischemic infarcts at the 90-day follow-up, but long-term (≥90 days) safety and effectiveness evidence is lacking. OBJECTIVES: This meta-analysis used high-quality RCTs (1-year follow-up) to assess EVT's clinical benefits in this patient group and compare short- vs. long-term prognostic changes. SEARCH METHODS: Literature searches were conducted in PubMed/MEDLINE, Scopus, and Web of S

Endovascular Thrombectomy in Patients With Largest Baseline Infarcts (ASPECTS 0-2): An Ancillary Analysis of the LASTE Trial.

BACKGROUND: Endovascular thrombectomy (EVT) has demonstrated benefits in patients with ischemic stroke and large-vessel occlusions, but its efficacy and safety in patients with the largest baseline infarcts (Alberta Stroke Program Early Computed Tomography Score [ASPECTS] 0-2), remain controversial. This study aimed to evaluate the efficacy and safety of EVT in early presenting patients with ASPECTS 0 to 2 compared with medical care alone. METHODS: This post hoc analysis of the multicenter, randomized LASTE trial ([Large Stroke Therapy Evaluation]; patients ≤80 years, ASPECTS 0-5, proximal an

Intracranial and Peripheral Inflammatory Biomarkers Predict Futile Recanalization After Endovascular Treatment.

BACKGROUND: Futile recanalization (absence of functional independence despite successful endovascular treatment [EVT]) remains a limitation in the treatment of large vessel occlusion stroke. Inflammation has emerged as a key contributor to brain reperfusion injury. Leveraging EVT, we quantified a large panel of immunologic biomarkers in the intracranial and peri-procedural peripheral blood samples and probed their association with futile recanalization. METHODS: We conducted a prospective single-center cohort study of patients with anterior-circulation large vessel occlusion, successful EVT (

The role of music therapy and music-based interventions in male mental health: a narrative review of mechanisms, barriers, and clinical implications.

BACKGROUND/OBJECTIVES: Men bear a disproportionate burden of undertreated mental health problems globally yet consistently underutilize formal psychological services, primarily due to traditional masculine norms that frame help-seeking as incompatible with masculine identity. Music therapy and music-based interventions offer a promising pathway that may circumvent these barriers by providing masculinity-congruent, non-clinical contexts for emotional expression and social connection. This narrative review synthesizes evidence on the mechanisms, outcomes, and clinical implications of music thera

Functional Connectivity of Hippocampal Circuits and Visual Memory Function in Children and Adolescents With Perinatal Stroke.

Perinatal stroke is a vascular injury occurring early in life, often resulting in motor deficits (hemiplegic cerebral palsy/HCP). Comorbidities may also include poor neuropsychological outcomes, such as deficits in memory. Previous studies have used resting state functional MRI (fMRI) to demonstrate that functional connectivity (FC) within hippocampal circuits is associated with memory function in typically developing controls (TDC) and in adults after stroke, but this is unexplored in perinatal stroke. Investigating links with visual memory function has the potential to inform prognosis and p

Arthroscopic anterior glenoid reconstruction using distal tibial allograft with suture button fixation: 2-year postoperative clinical outcomes.

BACKGROUND: Anterior glenohumeral instability is a common cause of shoulder instability. Bony reconstruction with distal tibial allograft (DTA) is a relatively new technique with promising results. Screw fixation of the graft comes with certain risks including hardware complications requiring revision surgery, so a novel technique, arthroscopic DTA with double suture-button fixation, has been proposed. The purpose of our study was therefore to report the outcomes of arthroscopic DTA anterior glenoid reconstruction with double suture-button fixation. METHODS: A single-surgeon, single-center, m
