Machine learning-based prediction of mortality and multidrug-resistant infection risks in ICU patients with suspected infection: a prospective national multicenter cohort study.

BACKGROUND: Suspected infection can progress to sepsis or septic shock, contributing to a high mortality rate among patients admitted to ICU. However, the characteristics of suspected infection remain incompletely defined. We aimed to develop and validate predictive models to identify independent risk factors for mortality and multidrug-resistant infection in patients with suspected infection upon ICU admission in mainland China. METHODS: We prospectively collected medical data from patients with suspected infection admitted to ICUs across mainland China between July 2021 and December 2022. Pa

Description of a nationwide structure for monitoring nosocomial outbreaks of (highly resistant) microorganisms in the Netherlands: characteristics of outbreaks in 2012-2021.

BACKGROUND: Before 2012, established national surveillance systems in the Netherlands were not able to provide a timely, comprehensive epidemiological view on nosocomial outbreaks. The Healthcare-associated Infections and AntiMicrobial Resistance Monitoring Group (SO-ZI/AMR) was initiated in 2012 for timely national nosocomial outbreak monitoring and risk assessment. This paper aims to describe the achievements of the SO-ZI/AMR by presenting characteristics of outbreaks reported in 2012-2021. METHODS: Hospitals and, since 2015, long-term care facilities (LTCF) were requested to report outbrea

Fluorescence High-Throughput Screening for Inhibitors of TonB Action.

Gram-negative bacteria acquire ferric siderophores through TonB-dependent outer membrane transporters (TBDT). By fluorescence spectroscopic hgh-throughput screening (FLHTS), we identified inhibitors of TonB-dependent ferric enterobactin (FeEnt) uptake through Escherichia coli FepA (EcoFepA). Among 165 inhibitors found in a primary screen of 17,441 compounds, we evaluated 20 in secondary tests: TonB-dependent ferric siderophore uptake and colicin killing and proton motive force-dependent lactose transport. Six of 20 primary hits inhibited TonB-dependent activity in all tests. Comparison of thei

The antimicrobial efficacy of silver on antibiotic-resistant bacteria isolated from burn wounds.

The antibiotic-resistant bacteria are a major concern to wound care because of their ability to resist many of the antibiotics used today to treat infections. Consequently, other antimicrobials, in particular ionic silver, are considered ideal topical agents for effectively helping to manage and prevent local infections. Little is known about the antimicrobial efficacy of ionic silver on antibiotic-resistant bacteria at different pH values. Consequently, in this study our aim was to evaluate the effect of pH on the antimicrobial efficacy of a silver alginate (SA) and a silver carboxymethyl cel

A simulation model to quantify the efficacy of dry cleaning interventions on a contaminated milk powder line.

UNLABELLED: Outbreaks of Salmonella in low moisture foods have been caused by cross-contamination from the processing environment into product. We used Monte Carlo simulations to model the impact of hypothetical cross-contamination scenarios of Salmonella from production equipment into milk powder. Model outputs included the quantity and extent of the contaminated product from a production line. Outputs were used to compare the efficacy of cleaning interventions. Cross-contamination of potential dry cleaning surrogates was also modeled. Input parameters for the model included log reductions fr

[New treatment option for gram-positive infections in critically ill patients - overview over linezolid].

INTRODUCTION: During the last decade, there has been an emergence of multiresistant Gram-positive bacteria in intensive care units. Oxazolidinones are a new class of antibiotics without cross-resistance to other antimicrobial agents, and linezolid was recently introduced as first oxazolidinone on the German market. In this overview, we summarize important data and discuss possible indications for linezolid in the treatment of infections in critically ill patients. ANTIMICROBIAL ACTIVITY AND PHARMACOLOGY: The antimicrobial spectrum comprises mainly Gram-positive bacteria, especially Staphylococ

In Vitro Persistence Level Reflects In Vivo Antibiotic Survival of Natural Pseudomonas aeruginosa Isolates in a Murine Lung Infection Model.

Clinicians are increasingly confronted with the limitations of antibiotics to clear bacterial infections in patients. It has long been assumed that only antibiotic resistance plays a pivotal role in this phenomenon. Indeed, the worldwide emergence of antibiotic resistance is considered one of the major health threats of the 21st century. However, the presence of persister cells also has a significant influence on treatment outcomes. These antibiotic-tolerant cells are present in every bacterial population and are the result of the phenotypic switching of normal, antibiotic-sensitive cells. Per

A change for the antibacterial treatment policy to decrease carbapenem consumption at a haematopoietic stem cell transplantation centre.

After experiencing a high rate of carbapenem-resistant Gram-negative bacilli infections in febrile neutropenic patients, a two-stage intervention was introduced in the haematopoietic stem cell transplantation (HSCT) centre. During the first eight months of 2014, carbapenems remained the first choice for the empirical treatment of febrile neutropenia while the use of piperacillin/tazobactam (TZP) was encouraged in patients with stable clinical condition. When blood cultures were reported as negative and the patient was clinically stable the carbapenem/TZP treatment was stopped regardless of con

Housefly (Musca domestica) and Blow Fly (Protophormia terraenovae) as Vectors of Bacteria Carrying Colistin Resistance Genes.

Flies have the capacity to transfer pathogens between different environments, acting as one of the most important vectors of human diseases worldwide. In this study, we trapped flies on a university campus and tested them for mobile resistance genes against colistin, a last-resort antibiotic in human medicine for treating clinical infections caused by multidrug-resistant Gram-negative bacteria. Quantitative PCR assays we developed showed that 34.1% of Musca domestica (86/252) and 51.1% of Protophormia terraenovae (23/45) isolates were positive for the mcr-1 gene, 1.2% of M. domestica (3/252) a
