We report a case of a patient with HELLP syndrome, hemorrhagic shock due to liver rupture and late postpartum eclampsia superimposed on lupus nephropathy and chronic hypertension. This patient was delivered at 26 weeks by C-section. Aggressive and complex surgical and medical treatments were necessary to achieve hemostasis and stabilize the patient. She recovered and was discharged to home on the 24th postpartum day in good condition. The premature baby was discharged home in fair condition on the 131st hospital day.

We report on the progress of an ongoing prospective non-randomized trial evaluating carotid artery stenting (CAS) with adjuvant cerebral protection in patients who are considered high risk for surgery. The 62 patients (34M/28F) in this study underwent CAS with interruption or reversal of flow in the internal carotid artery to protect the brain from embolization of particulate debris that may otherwise egress to the brain resulting in a stroke. There is an important association between testicular microlithiasis and germ cell tumors. Ultrasound findings of 1-3 mm echogenic foci within the testicular parenchyma may be noted. Testicular microlithiasis in the general population without germ cell tumors or cryptorchidism has been estimated between 0.05 and 0.6%. Subclavian artery injury secondary to blunt trauma is uncommon, accounting for only 3% of traumatic injuries of the upper extremity. Associated injuries of nerve, bone and joint are common. Early recognition and appropriate surgical intervention are essential to reduce disability. Long-term limb dysfunction usually results from concomitant nerve injury. This article describes our experiences treating four patients at two Level II trauma centers in Huntington, W.Va., and reviews the literature on the subject of subclavian artery injury secondary to blunt trauma.

A thirty-eight-year old immunosuppressed woman presented with respiratory distress and was diagnosed with Pneumocystis Carinii Pneumonia. Pulmonary embolism was also suspected on clinical grounds. A Ventilation-Perfusion Scan was indeterminate. Contrast-enhanced spiral computed tomography of the chest confirmed the presence of a central pulmonary embolus and helped to avoid pulmonary angiography in this critically ill patient.

Since there are few studies examining gastroesophageal reflux (GER) in healthy children beyond infancy, we report our experiences treating children older than two with this condition. GER was diagnosed by either an abnormal extended intraesophageal pH monitoring (pH study) or presence of histological esophagitis.
