A markedly elevated prothrombin time (PT) and activated partial thromboplastin time (APTT) were observed in a 24-year-old man who was admitted with a history of ethylene glycol ingestion. Further laboratory evaluation suggested that the coagulopathy was related to acquired factor deficiencies. The PT and APTT improved transiently on usual doses of vitamin K, but rapidly became abnormal again. The coagulopathy was controlled only after large doses of vitamin K for at least 37 days. A series of coumarins incorporating hydroxy-, chloro- and/or chloromethyl-moieties in positions 3-, 4-, 6- and 7- of the heterocyclic ring were investigated for the inhibition of the zinc enzyme carbonic anhydrase (CA, EC 4.2.1.1). Fish are exposed to residues of anticoagulant rodenticides in the environment, likely due to secondary poisoning or direct consumption of fragments of bait. Based on detection of hepatic residues, scavenging and predatory non-target raptors are widely exposed to second generation anticoagulant rodenticides (SGARs). The authors present a case of chronic hepatitis presumably due to Syncumar treatment. The patient should be maintained on anticoagulant therapy due to combined prosthetic valve insertion. Chromatin activation of blood lymphocytes was investigated to justify the drug induced liver injury. In this test system Syncumar and other anticoagulant derivates gave a positive reaction to a different extent. The increased serum bilirubin and transaminase levels, which indicated liver injury showed regression in the course of the treatment and after changing Syncumar to Marcoumar (Phenprocoumon).

Voluntary ingestion of concentrated anticoagulant rodenticides leads to prolonged hypocoagulability sometimes accompanied by haemorrhage. The authors report 11 cases referred to the Paris Anti-Poisons Centre. The products implicated were chlorophacinone, bromadiolone, and warfarin. A case report of a 35-year old woman is presented who continued a coumarin therapy unconscious of an early pregnancy up to day 61. At term she delivered a baby with tetralogy of Fallot, up to now not mentioned in the symptom complex of malformations due to a coumarin therapy. In the case presented the typical symptoms of a warfarin embryopathy including a chondrodysplasia punctata and other morphological changes were lacking. Although some cases of cardiac malformations have been reported in literature following coumarin therapy in early pregnancy, a causal connection could not be confirmed.
