The split abdominal wall muscle flap repair for large congenital diaphragmatic hernias on extracorporeal membrane oxygenation.

BACKGROUND: Numerous techniques exist for repairing large congenital diaphragmatic hernias (CDHs) including prosthetic patches, tissue-engineered grafts, and various muscle flaps. A split abdominal wall muscle flap is a simple, durable way to repair a large diaphragmatic hernia. This technique has not gained widespread use, and some have suggested that it would be inappropriate in the setting of extracorporeal membrane oxygenation (ECMO) because of bleeding risk. We present our series of diaphragmatic hernias with a focus on those repaired with the split abdominal wall technique while on ECMO.

Tuberculous abdominal aortic pseudoaneurysm penetrating the left psoas muscle after BCG therapy for bladder cancer.

We describe a case of a 75-year-old man with abdominal aortic and right femoral tuberculous pseudoaneurysms 32 months after intravesical bacillus Calmette-Guerin therapy for bladder cancer. These aneurysms were probably brought on by systemic infection by Mycobacterium bovis. The infrarenal aorta and right common femoral artery were successfully replaced with an in situ expanded polytetrafluoroethylene graft. Tuberculous pseudoaneurysm after bacillus Calmette-Guerin therapy for malignancy is very rare, and we review the related literature.

Management of the abdominal wall defect in the prune belly syndrome by muscle transposition: an 18-year follow-up.

Prune belly syndrome (PBS), an uncommon anomaly, consists of genito-urinary abnormalities and a partial or complete absence of abdominal wall musculature. Although the patient's genito-urinary problems are addressed, the attention currently directed toward the abdominal wall deficiency has been mostly aimed at improving the cosmetic appearance of patients and does little to replace the important functions of the musculature. These functions are the support of the viscera and their compression as well as the movement of the trunk. A case report with an 18-year follow-up is presented where thigh

The automatic activity of abdominal muscles during stable and unstable standing postural tasks in older adults with and without low back pain- A cross-sectional study.

BACKGROUND: The postural control and abdominal muscles' automatic activity were found to be impaired in subjects with low back pain (LBP) during static activities. However, the studies are predominantly conducted on younger adults and a limited number of studies have evaluated abdominal muscles' automatic activity during dynamic standing activities in subjects with LBP. The present study investigated the automatic activity of abdominal muscles during stable and unstable standing postural tasks in older adults with and without LBP. METHODS: Twenty subjects with and 20 subjects without LBP were

Association between abdominal muscle weakness and weaning failure in mechanically ventilated critically ill patients: an ultrasound assessment.

BACKGROUND: Ultrasound evaluation of abdominal muscle activity during cough may provide a practical tool to identify patients at risk of failure of liberation from mechanical ventilation (MV). We aimed to investigate the association between abdominal muscle weakness (AMW), assessed by ultrasound during coughing, and failure of liberation from MV within 48 h after the first liberation attempt, with liberation failure within 7 days as a secondary outcome. We also explored its relationship with diaphragm dysfunction (DD) and ICU-acquired weakness (ICUAW). METHODS: We conducted a prospective coho

PDK4 drives abdominal aortic aneurysm by promoting smooth muscle cell metabolic reprogramming and NLRP3-mediated pyroptosis.

Abdominal aortic aneurysm (AAA) is a progressive dilation of the abdominal aorta that can rupture and cause catastrophic internal bleeding, yet the mechanisms driving AAA remain poorly understood. Here we show that pyruvate dehydrogenase kinase 4 (PDK4), a key metabolic regulator, is upregulated in human and mouse AAA tissues. Deletion of Pdk4 in vascular smooth muscle cells (VSMCs) significantly reduces AAA formation in male mice. Mechanistically, PDK4 promotes metabolic reprogramming in VSMCs, disrupts mitochondrial respiration, and activates the NLRP3 inflammasome and pyroptosis, thereby ex

Comparison of Abdominal Muscle Thickness and Lumbopelvic Control Among Women With and Without Different Severity of Stress Urinary Incontinence.

INTRODUCTION AND HYPOTHESIS: Stress urinary incontinence (SUI) may be related to dysfunction of the core stabilizing system. We hypothesized that women with more severe SUI would demonstrate reduced abdominal-muscle thickness and decreased lumbopelvic control. This study investigated differences in abdominal-muscle thickness and lumbopelvic control among women with different SUI severities compared with continent women. METHODS: This cross-sectional study included 54 women: 21 women with mild SUI, 12 women with moderate SUI, and 21 healthy controls. Abdominal-muscle thickness was measured usi

Visceral fat-to-muscle ratio is associated with inflammatory ileus after excision of intestinal fistula with severe abdominal adhesions.

BACKGROUND: Postoperative inflammatory ileus (PII) is characterised by long-term re-obstruction, following temporary defecation. An intestinal fistula excision (IFE) with severe abdominal adhesions could increase the risk of PII in patients with sarcopenia. On the other hand, the postoperative outcomes are negatively impacted owing to chronic preoperative inflammation, as measured by the visceral fat area (VFA) and the total abdominal muscle area index (TAMAI) ratio. This study aims to evaluate the predictability of VFA/TAMAI for postoperative PII after IFE. METHODS: Four tertiary centres par

External validation of a deep learning model for automatic segmentation of skeletal muscle and adipose tissue on abdominal CT images.

OBJECTIVES: Body composition assessment using CT images at the L3-level is increasingly applied in cancer research and has been shown to be strongly associated with long-term survival. Robust high-throughput automated segmentation is key to assess large patient cohorts and to support implementation of body composition analysis into routine clinical practice. We trained and externally validated a deep learning neural network (DLNN) to automatically segment L3-CT images. METHODS: Expert-drawn segmentations of visceral and subcutaneous adipose tissue (VAT/SAT) and skeletal muscle (SM) of L3-CT-i

Glucose transporter 1 in vascular smooth muscle cells is dispensable for abdominal aortic aneurysm induced by angiotensin II.

Treatment with an inhibitor of glucose use via glucose transporters (GLUT) has been shown to attenuate experimental abdominal aortic aneurysm (AAA) development in mice. Vascular smooth muscle cell (VSMC) signaling seems to be essential for angiotensin II (Ang II)-induced AAA in mice. Accordingly, we have tested a hypothesis that VSMC silencing of the major GLUT, GLUT1, prevents AAA development and rupture in mice treated with Ang II plus β-aminopropionitrile. A mouse model of inducible VSMC GLUT1 deletion was created and aortic GLUT1 silencing was confirmed. Without Ang II plus β-aminopropioni

Low Skeletal Muscle Density Assessed by Abdominal Computerized Tomography Predicts Outcome in Children With Chronic Kidney Disease.

OBJECTIVES: Skeletal muscle loss and abnormal fat distribution are predictors of poor clinical outcomes in adults with chronic kidney disease (CKD). However, the relationship between body composition (muscle mass and adipose tissue) and prognosis in children with CKD has not been well elucidated. METHODS: The retrospective single-center study enrolled children with CKD and healthy group who underwent an abdominal computerized tomography examination and compared the body composition of the third lumbar spine (L3) between the 2 groups. We defined the primary outcome as hemodialysis, peritoneal

[Bilateral Pallidal Deep Brain Stimulation for Dystonic Camptocormia Induced by Repetitive Abdominal Muscle Exercise:A Case Report].

Camptocormia is a rare and disabling movement disorder resulting in forward bending of the trunk. Camptocormia has many etiologies, although it is frequently observed patients with in Parkinson's disease and dystonia. Deep brain stimulation(DBS)of the globus pallidus internus(GPi)and subthalamic nucleus effectively treats camptocormia in Parkinson's disease and dystonia patients. Herein, we report a case of dystonic camptocormia induced by repetitive abdominal muscle exercise in which treatment was administered using bilateral GPi-DBS. A 54-year-old woman developed dystonic camptocormia at 53

Measurement of Uterine and Abdominal Muscle Electromyography in Pregnant Women for Estimation of Expulsive Activities during the 2nd Stage of Labor.

BACKGROUND: The progression of labor and delivery of the fetus is dependent upon uterine contractions and the voluntary effort of abdominal muscle contractions. A good monitor of uterine contractions and pushing is necessary for obstetrical care. Electromyography (EMG) is the underlying basis for contractility of muscle including the myometrium. OBJECTIVES: The aim of this study was to determine the relationship between EMG activity of uterine and abdominal muscles and the duration of the 2nd stage of labor in pregnant women. METHODS: EMG of both uterine and abdominal muscles was simultaneou
