Long-term habituation and sensitization of the acoustic startle response in the normal adult human.

The development and independence of short-term and long-term habituation (and short-term sensitization) of the acoustic startle response to repetitive stimuli has been repeatedly demonstrated in the experimental animal. Although short-term habituation (and sensitization) of acoustic startle has been studied in humans, neither long-term habituation nor sensitization has been demonstrated. In this study, long-term habituation (response amplitude decrement) of the blink component of the acoustic startle response occurred in normal men across five consecutive daily sessions of repetitive acoustic

Acoustic middle ear muscle reflex protection against magnetic coil impulse noise.

Electromagnetic stimulation (EMS) of the brain and the intracranial portion of the facial nerve has become a widely used clinical technique. The high intensity impulse noise acoustic artifact generated by some magnetic coils used in EMS has been shown to cause severe cochlear damage in experimental animals. This damage results in permanent threshold shifts throughout the auditory spectrum of the rabbit. As with other impulse and impact noise signals, the duration of the coil impulse noise is too short to be influenced by the normally protective acoustic middle ear muscle reflex. Artificially a

Speech Biomarkers for Quantifying Effects of Subthalamic Deep Brain Stimulation in Parkinson's Disease.

OBJECTIVE: Dysarthria is one of the most common and disabling side effects of subthalamic nucleus deep brain stimulation (STN-DBS) in Parkinson's disease (PD). Stimulation often exacerbates speech dysfunction beyond the effects of PD progression, likely because of current spread to structures surrounding the STN. This study aimed to develop speech biomarkers sensitive to DBS-induced dysarthria by isolating stimulation side effects and mapping their emergence across incrementally increased amplitudes. METHODS: Twenty-four PD patients with bilateral STN-DBS completed a standardized speech asses

Pressure-induced modifications of the acoustic nerve. Part II: Auditory brain stem responses.

The auditory brain stem response is a diagnostic tool commonly used in the evaluation of patients suspected of having an acoustic neuroma. The mechanism by which the auditory brain stem response is altered in the presence of an acoustic neuroma is unclear. In this report, an experimental model is presented as an investigative tool for future studies of this mechanism: acute compression of the nerves of the internal auditory canal in cats stimulates the changes observed in patients with acoustic neuromas.

Sleep Spindles and Slow Oscillations as Potential Biomarkers and Therapeutic Targets in Mild-to-Moderate Alzheimer's Disease.

PURPOSE OF REVIEW: This review critically evaluates evidence on sleep spindles (SP), slow oscillations (SOs), and their coordination in mild-to-moderate Alzheimer's disease (AD), focusing on their relationships with cognition, AD biomarkers, and their therapeutic potential. RECENT FINDINGS: Longitudinal studies associate greater SP and/or SO activity with better cognition and favourable amyloid and tau profiles after clinical onset. Spindle and SO coupling may index network integrity. Preserved NREM sleep oscillations are associated with lower orexin concentrations and weaker adverse relation

Is cochlear implantation possible after acoustic tumor removal?

HYPOTHESIS: This study aimed to assess the possibility of hearing restoration after acoustic tumor removal. BACKGROUND: Hearing restoration surgery may be indicated after surgery of bilateral acoustic tumors or surgery of acoustic tumor in the only hearing ear. The choice is between cochlear implantation and brainstem implantation. METHODS: From the temporal bone collection at the House Ear Institute, Los Angeles, the author histologically examined eight temporal bones from seven patients who had undergone acoustic tumor removal during their lifetime. Special emphasis was put on examining th

Hearing preservation at low frequencies by insulin-like growth factor 1 in a guinea pig model of cochlear implantation.

The hybrid or electric-acoustic stimulation cochlear implant is indicated in patients with a residual hearing at low frequencies. It provides electric and acoustic stimulation for compensating for high- and low-frequency sounds, respectively. However, the implantation procedure damages the cochlea, resulting in loss of the residual-hearing and diminished effects of the acoustic-hearing in several patients. To prevent hearing loss after implantation, corticosteroids have been used clinically although their effects are limited. As an alternative to corticosteroids, insulin-like growth factor 1 (

Stapedius reflexes to electrical stimulation in the rabbit for the assessment of hearing.

The possibility of using electrically induced stapedius reflexes as a means of objective hearing evaluation was investigated in the rabbit as an animal model. The contralateral stapedius reflex to acoustic and electrical stimulation was measured in anesthetized rabbits. For electrical stimulation, the middle ear was opened surgically and stimulation was applied by a monopolar electrode placed at or into the round window. Contraction of the middle ear muscle was monitored by digital sampling and averaging of the impedance changes in the contralateral ear. Acoustically evoked reflexes were recor

Intracranial Electrical Stimulation of the Superior Temporal Gyrus Evokes Rapid Responses in Human Visual Cortex.

Sounds can alter visual processing even at the earliest stages of the cortical pathway. However, our understanding of how acoustic signals are relayed to visual cortex is limited, particularly with respect to the involvement of different regions of auditory cortex in the most rapid stages of this communication across distinct visual areas. Here, we address this question by examining occipital intracranial responses to direct electrical stimulation of the superior temporal gyrus (STG) in 23 patients with epilepsy. Stimulation of mid- and posterior-STG elicited responses in visual cortex as earl

Transmission of Binaural Cues by Bilateral Cochlear Implants: A Comparison of Spectral Peak-Picking and Interleaved Stimulation Strategies.

Cochlear implants (CIs) effectively restore speech understanding in quiet for many recipients. However, bilateral CI listeners continue to perform poorer than acoustic-hearing listeners on spatial-hearing tasks such as sound localization. One explanation is that stimulation strategies and the lack of coordination across sound processors degrade the fidelity of binaural cues. To provide further insight on this issue, this study compared the binaural cues transmitted by two CI stimulation strategies: a spectral peak-picking stimulation strategy and a constantly stimulating strategy. Two clinical

Rabbit model with vocal fold hyperadduction.

OBJECTIVE: Adductor spasmodic dysphonia (AdSD) is caused by hyperadduction of the vocal folds during phonation, resulting in a strained voice. Animal models are not yet used to elucidate this intractable disease because AdSD has a difficult pathology without a definitive origin. For the first step, we established an animal model with vocal fold hyperadduction and evaluated its validity by assessing laryngeal function. METHODS: In this experimental animal study, three adult Japanese 20-week-old rabbits were used. The models were created using a combination of cricothyroid approximation, forced

Short-term effects of transcranial direct current stimulation on motor speech in Parkinson's disease: a pilot study.

INTRODUCTION: Hypokinetic dysarthria (HD) is a common motor speech symptom of Parkinson's disease (PD) which does not respond well to PD treatments. We investigated short-term effects of transcranial direct current stimulation (tDCS) on HD in PD using acoustic analysis of speech. Based on our previous studies we focused on stimulation of the right superior temporal gyrus (STG) - an auditory feedback area. METHODS: In 14 PD patients with HD, we applied anodal, cathodal and sham tDCS to the right STG using a cross-over design. A protocol consisting of speech tasks was performed prior to and imm

On the influence of the middle ear muscles upon changes in sound transmission.

The influence of artificially induced impedance changes on the frequency response of the middle ear has been investigated in three experienced listeners. The tensor-reflex has been simulated by application of positive and negative air-pressure to the outer ear canal. In this test-situation, the thresholdshift ("l) obtained is greatest in the surrounding of the first resonance of the middle ear ("l = 8 dB at 500 Hz). For bone-conduction, the effect is weaker by about 2 dB than for air-conduction. The change in sound transmission is constant for all SPL's and is mainly due to the impedance chang

Fast Periodic Auditory Stimulation Reveals a Robust Categorical Response to Voices in the Human Brain.

Voices are arguably among the most relevant sounds in humans' everyday life, and several studies have suggested the existence of voice-selective regions in the human brain. Despite two decades of research, defining the human brain regions supporting voice recognition remains challenging. Moreover, whether neural selectivity to voices is merely driven by acoustic properties specific to human voices (e.g., spectrogram, harmonicity), or whether it also reflects a higher-level categorization response is still under debate. Here, we objectively measured rapid automatic categorization responses to h

Auditory-nerve integrity after middle-fossa acoustic-tumor removal.

We sought to investigate the functional integrity of the auditory nerve in patients with postoperative hearing loss after middle cranial fossa acoustic-tumor removal in a case-series descriptive study. The study setting was a tertiary referral center, a private otologic practice. The study population comprised seven patients who underwent a middle-cranial-fossa approach for unilateral acoustic-tumor resection and sustained postoperative anacusis with an anatomically intact auditory nerve. Four were men and three women; they ranged in age from 30 to 60 years; all underwent surgery between 1990

Changes in Acoustic Absorbance Pre- and Post-Cochlear Implantation.

PURPOSE: Until recently, there has been little investigation on the effects of cochlear implantation on the transmission of acoustic stimuli through the middle-ear system. Recent studies have shown that cochlear implantation decreases low-frequency acoustic absorbance, consistent with a stiffer middle-ear system postsurgery. The objectives of this study are (a) to investigate the time course of changes in acoustic absorbance post-cochlear implantation in the implanted ear and (b) to compare changes in acoustic absorbance between implanted and nonimplanted ears over time. METHOD: Seventeen adu

[Smartphone app-supported approaches to tinnitus therapy].

In recent years, applications (apps) for medical purposes have been developed and introduced, including apps that perform diagnostics and documentation for hearing loss and tinnitus in combination with smartphones. Even therapeutic apps, particularly for chronic tinnitus, have been launched. This review discusses the existing scientific literature for these smartphone applications. For tinnitus, Internet-based cognitive behavioral therapy has been developed and evaluated. For tinnitus therapy, introduced apps combine acoustic stimulation and music, or serve acoustic stimulation of cortical reg

A corticosteroid-responsive transcription factor, promyelocytic leukemia zinc finger protein, mediates protection of the cochlea from acoustic trauma.

Animals can be induced to resist cochlear damage associated with acoustic trauma by exposure to a variety of "conditioning" stimuli, including restraint stress, moderate level sound, heat stress, hypoxia, and corticosteroids. Here we identify in mice a corticosteroid-responsive transcription factor, PLZF (promyelocytic leukemia zinc finger protein), which mediates conditioned protection of the cochlea from acoustic trauma. PLZF mRNA levels in the cochlea are increased following conditioning stimuli, including restraint stress, dexamethasone administration, and moderate-to-high level acoustic s
