Severe cutaneous adverse reactions induced by targeted anticancer therapies and immunotherapies.

With the increasing use of targeted anticancer drugs and immunotherapies, there have been a substantial number of reports concerning life-threatening severe cutaneous adverse reactions (SCARs), including Stevens-Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), drug rash with eosinophilia and systemic symptoms, drug-induced hypersensitivity syndrome, and acute generalized exanthematous pustulosis. Although the potential risks and characteristics for targeted anticancer agent- and immunotherapy-induced SCAR were not well understood, these serious adverse reactions usually result in morb

Abrupt generalized pustules in patients with rheumatoid arthritis and interstitial lung disease.

We report a case of a 30-year-old Chinese woman with rheumatoid arthritis and interstitial lung disease who abruptly developed generalized pustules and a high fever for 10 days. She had been taking oral prednisone, iguratimod and total glucosides of peony regularly for 5 months prior. In addition, she had taken metronidazole for 3 days 20 days prior which she had used before with no adverse reaction. She had no history of similar lesions and psoriasis. A biopsy of a pustule on the back showed spongiform pustule of Kogoj. She was suspected of having generalized pustular psoriasis or acute gener

Celecoxib: Not So Harmless?

Toxic epidermal necrolysis (TEN) is a severe and potentially fatal cutaneous reaction secondary to the use of drugs, including celecoxib. Celecoxib is a nonsteroidal anti-inflammatory drug (NSAID) that is widely prescribed to relieve arthralgia because of its better gastrointestinal tolerance. However, it may also be responsible for skin reactions, most of which are benign. Serious cutaneous adverse reactions have been reported in the form of acute generalized exanthematous pustulosis (AGEP). TEN secondary to celecoxib is poorly documented. Discontinuation of the suspected or causative drug(s)

Severe cutaneous adverse reactions associated with systemic antibiotics in acne vulgaris: a systematic review.

BACKGROUND: Acne vulgaris (AV) is an inflammatory disorder of the pilosebaceous unit commonly managed with systemic antibiotics. Since antibiotics are known triggers of severe cutaneous adverse drug reaction (SCARs), patients with AV receiving prolonged antibiotics are at potential risk. This study aims to describe reported cases of SCARs occurring among AV patients treated with systemic antibiotics. METHODS: A literature search was conducted in November 2025 using PubMed, Web of Science, ClinicalKey, Ovid and Cochrane library. Eligible studies included reports of AV patients who developed SC

T cell-mediated acute localized exanthematous pustulosis caused by finasteride.

A 21-year-old man presented with multiple erythematous nonfollicular papules partially confluent to plaques on his breast and lower abdomen that had been present for 1 month. Grouped pustules were present under the right breast. The patient had been taking finasteride over the past 3 months for androgenetic alopecia. His medical history was negative for psoriasis. Our initial differential diagnosis included dyskeratosis follicularis Darier, allergic contact dermatitis, infectious folliculitis, varicella zoster virus infection, fixed drug eruption, and IgA pemphigus. The white blood cell count

Line-field confocal optical coherence tomography: a new tool for non-invasive differential diagnosis of pustular skin disorders.

BACKGROUND: The spectrum of pustular skin disorders (PSD) is large and particularly challenging, including inflammatory, infectious and amicrobial diseases. Moreover, although pustules represent the unifying clinical feature, they can be absent or not fully developed in the early stage of the disease. The line-field confocal optical coherence tomography (LC-OCT) is a recently developed imaging technique able to perform a non-invasive, in vivo, examination of the epidermis and upper dermis, reaching very high image resolution and virtual histology. OBJECTIVES: We aimed to investigate the poten

BACKGROUND: Besides reactions of the IgE-mediated immediate type, medicamentous therapies can cause a variety of different mucocutaneous adverse events. Exanthematous manifestations require a fast and certain diagnosis due to their extent, sometimes rapid progression, and mucous membrane or organ involvement. OBJECTIVES: The spectrum of non-IgE-mediated exanthematic drug reactions is covered. MATERIAL AND METHODS: The most relevant reactions are portrayed clinically and histopathologically. RESULTS: Displayed are classical maculo-papular drug eruption, lichenoid drug reaction, acute general

Iatrogenic skin injury in hospitalized patients.

Iatrogenic skin injuries in hospitalized patients range from drug-related complications to those related to procedures. Common drug complications include drug reaction with eosinophilia and systemic symptoms (DRESS), linear immunoglobulin (Ig) A bullous dermatosis, Stevens-Johnson syndrome/toxic epidermal necrolysis, and acute generalized exanthematous pustulosis. Contact dermatitis can result from surgical preparations of chlorhexidine and povidone-iodine, medical adhesives, topical postsurgical ointments, most commonly neomycin and bacitracin, and internal prostheses, including coronary sten

Treatments for Severe Cutaneous Adverse Reactions.

Severe cutaneous adverse reaction (SCAR) is life-threatening. It consists of Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN), drug reaction with eosinophilia and systemic symptoms (DRESS), acute generalized exanthematous pustulosis (AGEP), and generalized bullous fixed drug eruptions (GBFDE). In the past years, emerging studies have provided better understandings regarding the pathogenesis of these diseases. These diseases have unique presentations and distinct pathomechanisms. Therefore, theoretically, the options of treatments might be different among various SCARs. However, du

Skin tests in the work-up of cutaneous adverse drug reactions: A review and update.

Skin tests, including patch tests (PTs), prick tests, and intradermal tests (IDTs), are useful in identifying the culprits of cutaneous adverse drug reactions (CADRs), and determining safer, alternative drugs. PTs have a low sensitivity but are valuable in investigating maculopapular exanthema (MPE), as well as severe CADR, including toxic epidermal necrolysis (TEN), Stevens-Johnson syndrome (SJS), and in particular, acute generalized exanthematous pustulosis (AGEP), and drug reaction with eosinophilia and systemic symptoms (DRESS). To ensure their specificity, at least 10 control tests should

Novel IL36RN mutation in a Japanese case of early onset generalized pustular psoriasis.

Generalized pustular psoriasis is a distinct type of psoriasis characterized by recurrent febrile attacks with disseminated subcorneal pustules on generalized skin rashes. Recently, homozygous and compound heterozygous mutations of the IL36RN gene, which encodes the anti-inflammatory cytokine interleukin (IL)-36 receptor antagonist, were identified in familial and sporadic cases of various ethnicities with generalized pustular psoriasis. Here we report a 39-year-old Japanese male patient who had suffered from repeated attacks of generalized pustular psoriasis since infancy with intervals of se

Post-Streptococcal Neutrophilic Pustular Dermatosis: Expanding the Histopathological Paradigm of Reactive Neutrophilic Dermatoses.

Post-streptococcal pustulosis was first described in 1974 by Tan et al., and was only clearly distinguished from acute generalized exanthematous pustulosis in 1995. It is a rare neutrophilic dermatosis which usually arises following group A streptococcal infections, and is self-limited in time, typically resolving with 1 month. Clinical presentation includes a roughly symmetrical sterile pustular eruption arising in normal skin, most often affecting acral areas, like palms, soles, and legs. Up to this date, only 26 cases have been documented worldwide, making it a rare phenomenon. Differential

[Drug rash with eosinophilia and systemic symptoms (DRESS syndrome)].

Adverse cutaneous reactions to drugs are frequent, affecting from 2% to 3% of all hospitalized patients. But only about 2% of these cutaneous reactions are severe and seldom are fatal. The term drug hypersensitivity syndrome refers to a specific severe drug reaction, including skin rash, fever, lymph node enlargement, and single or multiple organ involvement. The cutaneous rash is usually morbilliform. The drugs associated with the syndrome are: anticonvulsants, ACE inhibitors, Beta-blockers, allopurinol and sulphonamides. The differential diagnosis includes maculopapular rash, exfoliative der

A recent update of pharmacogenomics in drug-induced severe skin reactions.

In some adverse drug reactions (ADRs), genetic predisposition plays a significant role in pathogenesis, and the skin is the most frequently reported target. These severe cutaneous ADRs include bullous fixed drug eruptions (FDE), acute generalized exanthematous pustulosis (AGEP), drug-induced hypersensitivity syndrome (HSS), Stevens-Johnson syndrome (SJS), and toxic epidermal necrolysis (TEN). The putative contribution of individual effector cells in drug hypersensitivity is briefly mentioned. To trigger these drug hypersensitivities, certain class I HLA alleles (e.g., HLA-A and HLA-B alleles)
