PURPOSE OF REVIEW: There has been increasing use of multimodality imaging in the evaluation of cardiomyopathies.
RECENT FINDINGS: Echocardiography, cardiac magnetic resonance (CMR), cardiac nuclear imaging, and cardiac computed tomography (CCT) play an important role in the diagnosis, risk stratification, and management of patients with cardiomyopathies. Echocardiography is essential in the initial assessment of suspected cardiomyopathy, but a multimodality approach can improve diagnostics and management. CMR allows for accurate measurement of volumes and function, and can easily detect uniqu

Cardiac arrest is a rare but reported complication during breast augmentation surgery. It is even more rare in a reportedly healthy patient without preexisting cardiac disease. The authors present the case of a healthy 34-year-old female who underwent elective bilateral augmentation mammaplasty and experienced unanticipated asystolic cardiac arrest intraoperatively following general anesthesia supplemented by a regional pectoral (pec I) nerve block. The performing plastic surgeon provided cardiopulmonary resuscitation while the anesthesiologist initiated a rescue protocol per Advanced Cardiac

Cardiovascular disease (CVD) remains the leading cause of death worldwide and is a major driver of critical illness, resulting in admission to cardiac intensive care units (CICUs). Patients with acute, life-threatening CVD often experience episodes of acute decompensation characterized by high symptom burden that may portend progressive loss of function, significantly impair quality of life (QOL), and involve high-intensity, technologically advanced therapies to sustain life. Despite these advances, up to 30% of patients die during CICU hospitalizations, and those who survive may subsequently

Acute caffeine intoxication has become a rare but life-threatening clinical and forensic concern, particularly with the increasing availability of highly concentrated pure caffeine preparations. This report presents a fatal case following the intentional ingestion of a high dose of powdered pure caffeine, evaluated through an integrated assessment of the clinical course, autopsy findings, histopathological examination, and postmortem toxicological analyses. A man in his late twenties presented to the emergency department with nausea, vomiting, and tachycardia. Although transient clinical impro

Extracorporeal membrane oxygenation was first successfully achieved in 1975 in a neonate with meconium aspiration. Neonatal extracorporeal membrane oxygenation has expanded to include hemodynamic support in cardiovascular collapse before and after cardiac surgery, medical heart disease, and rescue therapy for cardiac arrest. Advances in pump technology, circuit biocompatibility, and oxygenators efficiency have allowed extracorporeal membrane oxygenation to support neonates with increasingly complex pathophysiology. Contraindications include extreme prematurity, extremely low birth weight, leth

BACKGROUND: We have reported the epidemiology and survival rate of out-of-hospital cardiac arrest (OOH-CA) in a north-east region of Italy previously, the Friuli-Venezia-Giulia Arrest Cooperative Study (FACS). We present the results of a second observational, prospective, multicentre study on OOH-CA victims in a local area in the same geographical Italian region.
METHODS AND RESULTS: The area investigated, Pordenone province, is representative of the entire region studied in 1994. In the 1994 FACS study, the heterogeneous ambulance personnel, ranging from volunteers to registered nurses and p

UNLABELLED: The survival to discharge rate after unwitnessed, non-cardiac out-of-hospital cardiac arrest (OHCA) is dismal. We report the successful use of therapeutic hypothermia in a 26-year old woman with OHCA due to intentional poisoning with heroin, amphetamine and insulin. The cardiac arrest was not witnessed, no bystander CPR was initiated, the time interval from the call to ambulance arrival was 9 minutes and the initial cardiac rhythm was asystole. Eight minutes of advanced cardiac life support resulted in ROSC.Upon hospital admission, the patient's pupils were dilated. Her arterial lac

The effect of prehospital advanced airway management (AAM) in patients with out-of-hospital cardiac arrest requiring extracorporeal cardiopulmonary resuscitation remains unclear. In this retrospective study, we analysed data from the Study of Advanced Cardiac Life Support for Ventricular Fibrillation with Extracorporeal Circulation in Japan II, encompassing 36 institutions in Japan (2013-2018). Patients were divided into two groups: those who received prehospital AAM and those who did not. The primary outcome was a favourable neurological outcome at discharge, and secondary outcomes included 3

Adult congenital heart disease (ACHD) is a growing population that requires life-long care due to advances in pediatric care and surgical or catheter procedures. Despite this, drug therapy in ACHD remains largely empiric due to the lack of clinical data, and formalized guidelines on drug therapy are currently lacking. The aging ACHD population has led to an increase in late cardiovascular complications such as heart failure, arrhythmias, and pulmonary hypertension. Pharmacotherapy, with few exceptions, in ACHD is largely supportive, whereas significant structural abnormalities usually require

Perioperative right ventricular dysfunction can be life-threatening. Anesthetic management of elderly patients with hip fractures and advanced cardiac disease poses considerable perioperative risks. The problem-based learning discussion examines the management of an 87-year-old female with severe pulmonary hypertension, right ventricular dysfunction, and recent anticoagulation scheduled for urgent hip fracture repair. A tailored management approach included peripheral nerve blocks, intraoperative transesophageal echocardiographic monitoring, goal-directed hemodynamic support, and management of

Cardiogenic shock (CS) is a life-threatening condition characterized by acute end-organ hypoperfusion due to inadequate cardiac output that can result in multiorgan failure, which may lead to death. The diminished cardiac output in CS leads to systemic hypoperfusion and maladaptive cycles of ischemia, inflammation, vasoconstriction, and volume overload. Obviously, the optimal management of CS needs to be readjusted in view of the predominant dysfunction, which may be guided by hemodynamic monitoring. Hemodynamic monitoring enables (1) characterization of the type of cardiac dysfunction and the

Background: Seasonal influenza is typically a self-limiting illness in healthy young adults, but severe and potentially life-threatening complications can occur. Influenza A infection may predispose patients to secondary bacterial infections, including Staphylococcus aureus pneumonia and bacteremia, which can rapidly progress to necrotizing pneumonia, acute respiratory distress syndrome (ARDS), and cardiovascular dysfunction. Case Presentation: We report the case of a young adult with influenza A(H3) infection complicated by Staphylococcus aureus bacteremia, necrotizing pneumonia, ARDS, and bi

Myocarditis is an inflammatory condition of the myocardium and is usually categorised as acute nonfulminant and acute fulminant myocarditis. Myocardial injury can result via viral infections, direct injury or immune responses. Fulminant myocarditis can be characterised by severe and sudden cardiac inflammation that may result from cardiogenic shock, ventricular arrhythmias or multi-organ system failure. Extracorporeal membrane oxygenation (ECMO), also known as extracorporeal life support, is an effective technique for patients with fulminant myocarditis, providing heart and lung support and ad

Severe hypokalemia is a rare but potentially reversible cause of malignant arrhythmias and cardiac arrest. Early recognition of significant fluid, electrolyte, and acid-base disturbances in patients with intestinal urinary diversion may be life-saving. We present the clinical course of a 55-year-old woman who developed severe hyperchloremic metabolic acidosis and extreme hypokalemia following prolonged vomiting and high-output fluid loss through a Bricker ureteroileocutaneostomy. The patient was admitted to the intensive care unit in a peri-arrest condition, where cardiac arrest occurred short

Sudden unexpected death in epilepsy (SUDEP) poses a significant risk to life expectancy for individuals with epilepsy. Mechanistic insight, while incomplete, has advanced through clinical observational studies and animal models. Yet we lack preventative therapies, which will depend on understanding SUDEP mechanisms. Recurrent convulsive seizures are the major SUDEP risk factor. Cardiorespiratory dysfunction precedes SUDEP, but whether cardiac arrhythmias are major proximate culprits for SUDEP remains to be determined. Here, we highlight recent data from mouse models and clinical studies that p

Life expectancy of patients with congenital heart disease (CHD) continues to increase dramatically over the last few decades, primarily due to significant medical and surgical advancements in the field. Adult congenital heart disease (ACHD) patients are now living longer but continue to face morbidity and reduced long-term survival when compared to the general population. We briefly describe the growth of ACHD with a focus on surgical procedures and outcomes in the more complex lesions including Tetralogy of Fallot, Ebsteins Anomaly of the tricuspid valve, Coarctation of the Aorta, Transpositi

Fulminant myocarditis (FM) is an uncommon syndrome characterized by sudden and severe diffuse cardiac inflammation often leading to death resulting from cardiogenic shock, ventricular arrhythmias, or multiorgan system failure. Historically, FM was almost exclusively diagnosed at autopsy. By definition, all patients with FM will need some form of inotropic or mechanical circulatory support to maintain end-organ perfusion until transplantation or recovery. Specific subtypes of FM may respond to immunomodulatory therapy in addition to guideline-directed medical care. Despite the increasing availa

We report on recent methodological advances at the Life Science X-ray Scattering (LiX) beamline of the National Synchrotron Light Source II (NSLS-II) to support small-angle X-ray scattering experiments on striated muscles. These experiments measure sarcomeric protein organization in healthy and diseased muscle and provide direct molecular evidence for these proteins' functional roles and dynamics. Many recent advances in sarcomeric protein research have relied on diffraction data; for example, structural and functional roles of cMyBP-C and titin, and conformational/energetic states of actomyos

Accidents are a leading cause of death for children in several nations. Motor vehicle accidents are among the most common causes of cardiac arrest. Other causes include drowning, burns, gunshot wounds, poisoning, smoke inhalation, and airway obstruction caused by asphyxiation from foreign bodies. Approximately 50% to 65% of children who require cardiopulmonary resuscitation are younger than 1 year of age, with most being younger than 6 months of age. To prevent loss and improve quality of life, it is imperative to initiate measures to improve oxygenation and treat cardiac dysfunction. Guidelin

A 75-year-old female presented for left total hip reimplantation and suffered pulseless electrical activity arrest upon lateral positioning and administering vancomycin. Resuscitation was achieved according to Advanced Cardiac Life Support protocol. Post-event echocardiography showed hypertrophic cardiomyopathy with asymmetrical septal thickening, an under-filled left ventricle, dynamic left ventricular outflow obstruction, and severe mitral regurgitation related to systolic anterior motion of the mitral valve. Laboratory analysis showed a tryptase level of 209 ng/mL. After multispecialty eval
