Alcoholism among homeless men in Munich, Germany.

Probably in association with changes in the economic structure and high unemployment rates in western industrialized countries, homelessness is becoming more apparent publicly and receiving increased media attention. More studies on the issue of mental illness and homelessness have been performed in recent years in North America while hardly any representative and reliable data exists concerning Germany and some other European countries. The aim of our study was 1) to assess alcohol abuse and dependency as well as other mental disorders in a representative sample of homeless men in Munich usin

Research strategies to assess pharmacotherapies for alcoholism.

1. The development and implementation of standardized basic and clinical research strategies is necessary for the discovery and testing of new and effective pharmacotherapies for alcohol dependence. While many animal studies have indicated the involvement of central serotonin, endogenous opioids and dopamine, their exact mechanisms of action have not been fully discerned. One problem is the use of various animal models, making it difficult to compare or replicate results from different laboratories. 2. Once a drug appears to decrease alcohol intake in animals, assessing its effects in humans c

Co-morbidity of alcoholism and the paraphilias.

Alcoholism has been related to a wide variety of crimes. Various authors have studied the prevalence of alcoholism in relationship to incest, rape and pedophilia. This study evaluates the co-morbidity of alcoholism and the specific paraphilias including sexual sadism, fetishism, incest, pedophilia, exhibitionism and transvestism. Rapists were included although by definition it is not strictly a paraphilic disorder. Seven hundred and twenty eight paraphilic individuals were evaluated. More than 50% of sexual sadists were alcoholics. Alcoholism in association with sexual sadists was statisticall

Disulfiram (antabuse) contracts in treatment of alcoholism.

Although studies repeatedly demonstrate that alcoholics who consistently take disulfiram experience more favorable drinking outcomes, serious problems with compliance among the majority of alcoholic patients have reduced the effectiveness of disulfiram as a therapeutic adjunct. In general, alcoholism counseling with disulfiram simply prescribed seems no more effective than counseling without disulfiram (Fuller et al. 1986). Problems with compliance as well as problems with acceptance by patients and likely by clinical staff reduce the utility of disulfiram in the treatment of alcoholism. Impla

[Alcoholism: from the molecular to public health].

The current knowledge on the interactions between alcohol and the central nervous system moves very quickly. Moreover the scientific advances follow the three axes of our understanding, biological, psychological and sociological, but this implies some dispersion. The present paper focuses on recent advances in neurobiology of alcoholism and shows new perspectives in pharmacological interventions, with new drugs. In the same time, the psychodynamic and systemic understandings are evoked, in a purpose of integration and of public health policy.

Multiple risk factors predict suicide in alcoholism.

From a study of the lives of 50 alcoholics who committed suicide, seven nonacute clinical/historical features were identified post hoc that appear to be intimately linked to suicide. They included continued drinking, major depressive episode, suicidal communication, poor social support, serious medical illness, unemployment, and living alone. This study examined whether these features are common to alcoholics generally and/or to nonalcoholics who commit suicide or if they may be specifically predictive of suicide in alcoholics. The frequencies of these factors were compared with those in an ea

[Fetal alcoholism syndrome: apropos of a case].

Fetal alcoholic syndrome (F.A.S.) is a well-recognized entity for over 20 years; it associates mental retardation, delayed growth and anomalies of the cranium in children born from alcoholic mothers. The syndrome has been described primarily in young children. By contrast, its evolution is not well known. We report the case of a 15-year old youngster, who exhibited the characteristic features of F.A.S. We also review the data related to the clinical and pathophysiologic aspects of the syndrome.

Pharmacotherapy of alcoholism: the American current status.

This paper is a brief review which deals with research findings, clinical issues, and strategies in the pharmacotherapy of alcoholism. The pharmacotherapy is presented according to different clinical phases of the alcoholic process. The acute intoxicated patient receives supportive treatment, to be clinically observed to prevent severe respiratory depression, aspiration of vomitus, and severe alcohol withdrawal syndrome. Benzodiazepine therapy is the mainstay in treating alcohol withdrawal syndrome. Disulfiram is the only chemical used in the United States to deter the alcoholic patient from f

[Value and justification of screening for alcoholism].

The Swiss are heavy alcohol consumers. Since alcohol consumption is a risk-associated behavior, it is not surprising that alcohol-related medical and psychosocial morbidities are frequently observed. Related costs are therefore also substantial. To reduce alcohol-associated morbidities and related costs, it is essential that the physician's role include both health education counseling to patients (to reduce alcohol consumption) and systematic alcohol abuse and dependence screening. This study reviews current knowledge of alcoholism screening. It emphasizes the validity of some screening instr

CSF studies on alcoholism and related behaviours.

1. A defect in central serotonin metabolism may manifest itself in poor impulse control leading to attempts at suicide, violence towards others, and Type II alcohol abuse. 2. Studies reporting negative associations between low CSF levels of the serotonin metabolite 5-HIAA and personality traits of aggressiveness and hostility are reviewed. 3. We also studied pathological gamblers. They showed significantly increased central noradrenaline metabolism, perhaps related to sensation seeking.

[Conradi-H:unermann chondrodysplasia punctata and fetal alcoholism].

Fetal alcoholism induces an extremely wide spectrum of embryopathies. In addition to the classical fetal alcohol syndrome, alcohol is also the cause of numerous fetal malformations. A case of Conradi-Hünermann type chondrodysplasia punctata is reported. Maternal alcohol ingestion was reported during gestation.

Depression and alcoholism: clinical considerations in management.

When alcohol abuse occurs with depression, both the substance abuse and the mood disorder necessitate treatment. These conditions may have some similar manifestations, making differential diagnosis difficult. Depressed alcoholics report more previous treatment for substance abuse, withdrawal symptoms, and marital problems than those without depression. They also incur greater loneliness, unemployment, and social ineptness. Depressive symptoms found commonly in this group include work inhibition, guilt, self-disgust, dissatisfaction, and social disinterest. A history of depression among relativ

Osteoporotic correlates of alcoholism in young males.

A group of 22 white males (ages, 28 to 40 years) with histories of chronic alcohol abuse ranging from 1 to 21 years were evaluated for trabecular bone mineral density in the left femoral neck area. A group of age and weight-matched white males with no history of alcoholism served as controls. All participants completed medical history questionnaires regarding fracture history, dietary habits, medications, and physical activity. Singh femoral trabecular indices also were measured for the alcoholic participants. Analysis of bone mineral data as measured by dual photon absorptiometry revealed no

[Chronic alcoholism--alcohol sequelae--causes of death].

In this study 444 chronic alcoholic patients, hospitalized at the beginning of this study, were followed up for 4 to 7 years (31). During this time 101 patients died (23.2%), with whom severeness of disease as well as the extent of social depravation could be identified as factors influencing mortality. Beginning in January of 1982 we investigated mortality of the population of a village with a similar socio-cultural background (working situation, rural population, wine-growing area). As it was possible to cooperate with the local general practitioner, who has been living and working in this v

Arylsulfatase A variants in patients with alcoholism.

Leukocytes from 200 mentally ill patients and 100 normal controls were analyzed for electrophoretic variants of arylsulfatase A. Four different variant forms were found in 15 subjects. There is a relatively high occurrence of the arylsulfatase A variants in patients with alcoholism. Twenty-one per cent (12/56) of patients with alcoholism have a variant enzyme. Only one of the 100 normal controls has a variant enzyme. (This single subject was considered normal by the criteria of the study, namely, a self-report of no current medical problem or psychiatric history. However, upon further testing,

Measuring the success of occupational alcoholism programs.

The research on the success of occupational alcoholism programs (OAPs) was reviewed. One measure of the success of OAPs has been a change in drinking behavior. This may seem like the most relevant measure but there are questions about its reliability because researchers usually depend on subjects' self-reports and do not define their criteria for successful outcome well. In addition, attempts by employers to dictate the drinking habits of their employees may be difficult to justify. Varying rates of success on this measure have been reported. Subjects have generally been able to control drinki
