Conus medullaris injury following both tetracaine and lidocaine spinal anesthesia.

Multiple reports of cauda equina syndrome and transient radicular nerve root irritation have suggested that lidocaine spinal anesthesia may be responsible. In this case report, a patient with a preexisting diabetic neuropathy received a partial block following a tetracaine spinal, which was followed by a lidocaine spinal. Following block resolution, a new conus medullaris syndrome was diagnosed. Because of the close proximity of the cauda equina and the conus medullaris, differentiation between these syndromes can be difficult. The preexisting diabetic neuropathy may have predisposed this pati

Metoclopramide decreases emesis after spinal anesthesia supplemented with subarachnoid morphine.

BACKGROUND AND OBJECTIVES: The authors studied whether metoclopramide decreases the incidence of emesis after spinal anesthesia supplemented with subarachnoid morphine. METHODS: Patients underwent total hip arthroplasty under spinal anesthesia using tetracaine with norepinephrine and morphine 0.2 mg. Forty-eight patients were included in the study conducted with a paired design, using sequential analysis. Patients were allocated randomly and double-blinded to the treatment with metoclopramide or to the control group. In one group, metoclopramide 20 mg intramuscularly was given before and afte

[Accidental spinal anesthesia in obstetrics. Limits of epidural test-dose].

Two cases of accidental spinal anaesthesia occurring in obstetrical patients are reported and discussed. Epidural anaesthesia had been asked for by the women, both being free from any significant medical history. A Tuohy needle was inserted in the midline between L3 and L4 with the patient sitting. The extradural space was identified by the loss of resistance using saline. The test-dose (2 ml and 4 ml of 1% lignocaine respectively) was administered five minutes before changing the patient to the supine position. In the first case, after a test-dose had remained without any effect, 8 ml of 0.25

Hearing loss after spinal anesthesia is related to needle size.

Audiograms were performed preoperatively and 2 days postoperatively in 28 patients given spinal anesthesia for transurethral resection of the prostate. In 14 patients 22-gauge and in 14 patients 26-gauge spinal needles were used. Hearing loss of 10 dB or more at any frequency was observed in 13 of 14 patients in the 22-gauge group and in 4 of 14 patients in the 26-gauge group. There was a statistically significant reduction in hearing level in the low-frequency range in patients in whom the 22-gauge needle was used. Hearing loss was unilateral at five frequencies and bilateral at one frequency

Fourth cranial nerve palsy following spinal anesthesia. A case report.

Fourth nerve palsy has been rarely seen following lumbar puncture, myelogram, or spinal anesthesia. We report a case of 4th nerve and 6th nerve palsies following spinal anesthesia. The 4th nerve palsy was best detected by using a Maddox rod. If all 6th nerve palsies occurring after spinal anesthesia were examined with a Maddox rod, more cyclovertical palsies might be discovered.

Suspected Lignocaine Allergy: Safe Conduct of Spinal Anesthesia with Bupivacaine.

Local anesthetics (LAs) are integral to surgical and diagnostic procedures, with allergic reactions being rare yet potentially life-threatening. We report a case of a 69-year-old male with chronic obstructive pulmonary disease and suspected lignocaine allergy, posted for transurethral resection of the prostate. Intradermal testing revealed a positive reaction to lignocaine but a negative response to preservative-free bupivacaine heavy. Based on this, spinal anesthesia was planned with bupivacaine heavy and fentanyl, along with prophylactic corticosteroids and antihistamines. The surgery procee

Low-dose spinal anesthesia in cesarean section: a narrative review.

Spinal anesthesia is the technique of choice for cesarean delivery, offering rapid onset and reliable anesthesia, but conventional intrathecal doses of hyperbaric bupivacaine frequently cause significant hypotension and delayed recovery. Low-dose spinal anesthesia (LDSA), generally defined as ≤ 8 mg of hyperbaric bupivacaine, often combined with intrathecal opioids, has been investigated as a strategy to mitigate these effects. Current evidence indicates that LDSA can attenuate hemodynamic fluctuations and reduce intraoperative nausea without compromising neonatal outcomes; however, excessivel

Intracranial Acute Subdural Hematoma Following Spinal Anesthesia for Cesarean Delivery.

Intracranial subdural haematoma is a rare but serious complication of spinal anesthesia in obstetric patients undergoing Cesarean Delivery (CD). In this case series, we analyzed 3 patients who developed acute intracranial subdural hematoma following spinal anesthesia for CD. Clinical presentation with headache of 3-4 weeks duration that worsened over 2-3 days prior to admission for neurosurgical intervention was evident in all the patients. Brain CT-Scan was the imaging modality used to confirm the diagnosis of acute intracranial subdural hematoma in all of them, and Burr hole was done to evac

Parental Decision-Making for Infant Spinal Anesthesia - A Qualitative Study.

BACKGROUND: Spinal anesthesia is a viable alternative to general anesthesia for infant surgical procedures below the umbilicus. However, choosing spinal anesthesia over general anesthesia is a complex medical decision for parents. Little is known about how parents perceive infant spinal anesthesia as an alternative to general anesthesia and how they make this decision for their children. AIMS: The aim of this research study was to generate a holistic understanding of the parental decision-making process for spinal anesthesia in infants. METHODS: A qualitative research study was conducted at

Supraventricular Tachycardia Associated With Repeat Cesarean Section Under Spinal Anesthesia.

Supraventricular tachycardia (SVT) is the most common tachyarrhythmia of pregnancy. Catecholamine surges, the use of vasoactive agents during delivery, and increased cardiac output during pregnancy are the most common contributing factors to developing SVT. SVT is usually benign in presentation but can lead to more serious arrhythmias in patients with a history of mitral stenosis secondary to rheumatic heart disease. When an SVT is detected, organic heart causes should be ruled out first. Symptoms of SVT include shortness of breath, palpitations, syncope, sweating, chest pain, and dizziness. I

Suspected Septic Sacroiliitis Following Recent Cesarean Section Under Spinal Anesthesia.

Septic sacroiliitis is sacroiliac (SI) joint inflammation secondary to microbial invasion of the synovial space characterized by tenderness over the sacroiliac joint, difficulty walking, and lower back pain that can radiate to the buttocks. Clinicians can easily overlook septic sacroiliitis as a potential diagnosis due to its rare occurrence and non-specific symptoms. A 30-year-old female (G2P2A0) who presented acutely to the ED nine days after an uncomplicated Cesarean section performed under spinal anesthesia. The patient experienced progressive lancinating, electric-shock pain originating f

Spinal Anesthesia for Pediatric Laparoscopic Hernia Repair: A Case Report.

Spinal anesthesia is practical and safe in infants and toddlers and prevents perioperative complications including cardiorespiratory instability and apnea. Other perioperative advantages include reduced time from surgery completion to operating room exit and first feed. This article presents the case of an ex-premature infant who underwent laparoscopic inguinal hernia repair under spinal anesthesia. The infant remained calm and stable throughout the uneventful procedure and postoperative period. Spinal anesthesia may be feasible for laparoscopic herniotomy when pneumoperitoneum pressure and op

Transforaminal endoscopic decompression for thoracic spinal stenosis under local anesthesia.

BACKGROUND: Thoracic spinal stenosis is a common vertebral degenerative disease, and treatment remains challenging. In recent years, transforaminal endoscopic decompression has been widely used for treating lumbar degenerative diseases. However, the efficacy of this procedure for thoracic spinal stenosis has yet to be established. Herein, we report a case of thoracic spinal stenosis treated with transforaminal endoscopic decompression under local anesthesia. CASE REPORT: An 88-year-old man presented with a 1-month history of progressive paralysis and dysesthesia in the bilateral lower extremi

Intrathecal Dexmedetomidine Alleviates Shivering during Cesarean Delivery under Spinal Anesthesia.

Shivering associated with spinal anesthesia during Cesarean delivery is an uncomfortable experience for the parturient, which may also cause adverse effects. In this prospective, randomized, double-blind, placebo-controlled study, we sought to evaluate the effect of intrathecal dexmedetomidine, administered as an adjunct to hyperbaric bupivacaine for Cesarean delivery, on the incidence and severity of shivering associated with spinal anesthesia. Patients undergoing Cesarean delivery were randomly allocated to three groups of 30 patients each. Experimental treatments were added to hyperbaric bu

Segmental thoracic spinal anesthesia in patient with Byssinosis undergoing nephrectomy.

Byssinosis is an occupational disease occurring commonly in cotton mill workers; it usually presents with features of chronic obstructive pulmonary disease (COPD). The management of patients with COPD presents a significant challenges to the anesthetist. Regional anesthesia is preferred in most of these patients to avoid perioperative and postoperative complications related to general anesthesia. We report a known case of Byssinosis who underwent nephrectomy under segmental spinal anesthesia at the low thoracic level.

Combined spinal epidural anesthesia in achondroplastic dwarf for femur surgery.

Achondroplasia is the commonest form of short-limbed dwarfism and occurs in 1:26,000-40,000 live births. This is an autosomal dominant disorder with abnormal endochondral ossification whereas periosteal and intramembranous ossification are normal. The basic abnormality is a disturbance of cartilage formation mainly at the epiphyseal growth plates and at the base of the skull. The anesthetic management of achondroplastic dwarfs is a challenge to the anesthesiologist. Both regional as well as general anesthesia have their individual risks and consequences. We report a case of an achondroplastic
