Quality of recovery and safety of deep intravenous sedation compared to general anesthesia in facial plastic surgery: A prospective cohort study.

BACKGROUND: Facial plastic surgical procedures are performed under either general anesthesia (GA) or sedation. GA is often associated with post-operative nausea and longer recovery, while deep sedation is thought to greatly facilitate perioperative patient comfort and expedite recovery. The objective of this study was to compare these two anesthetic techniques in a relatively healthy patient population undergoing facial plastic surgery and to discuss optimizing patient safety with a deep sedation technique. METHODS: A non-randomized, prospective cohort study was conducted to evaluate patients

Comparison of peribulbar block and general anaesthesia in mechanical vitrectomy: a prospective observational study.

INTRODUCTION: Vitrectomy surgery is a common procedure for the treatment of several types of ophthalmologic conditions. It can be performed under regional anaesthesia with peribulbar block (PB) or general anaesthesia (GA). There are no evidence-based recommendations on the optimal anaesthesia strategy for this procedure. The aim of this study was to compare the advantages of PB and GA for vitrectomy surgery. MATERIALS AND METHODS: A prospective observational study was conducted on adults submitted for mechanical vitrectomy between January 2017 and December 2017. Demographic and perioperative

Reduced Recovery Times with Total Intravenous Anesthesia in Patients with Obstructive Sleep Apnea.

OBJECTIVES/HYPOTHESIS: There is currently no standard of care in terms of anesthesia modality for patients receiving upper airway surgery with comorbid obstructive sleep apnea (OSA). Although both total intravenous anesthesia (TIVA) and volatile gas anesthesia are commonly utilized in ambulatory otolaryngology surgery, it is currently unclear if there are any advantages with one modality over the other. We hypothesize that patients receiving upper airway surgery with comorbid OSA will have quicker recovery times with TIVA. STUDY DESIGN: Retrospective chart review from January 2019 to December

Intraoperative clonidine to prevent postoperative emergence delirium following sevoflurane anesthesia in pediatric patients: a randomized clinical trial.

INTRODUCTION AND OBJECTIVE: Emergence Delirium (ED), particularly in children, is characterized by mental confusion, irritability, disorientation, and inconsolable crying. ED prolongs the time required in the Post-Anesthesia Care Unit (PACU) and increases concern and anxiety in parents. The present study aimed to determine the effectiveness and safety of low-dose clonidine in preventing ED in children receiving sevoflurane anesthesia for tonsillectomy/adenotonsillectomy. METHODS: A randomized, double-blind clinical trial was conducted between November 2013 and January 2014. Sixty-two children

[Case of delayed awakening after remifentanil leakage out of a vein].

We report a case of delayed awakening with characteristic repeated loss of consciousness after remifentanil infusion complicated by leakage from an intravenous catheter. A 30-year-old male underwent microlaryngeal surgery for a vocal cord polyp. During anesthetic induction, infiltration from an intravenous (IV) line in the left forearm was observed 10 min after initiating a continuous infusion of remifentanil 0.5 microg x kg-1 x min -1. A second peripheral IV catheter was placed in the right forearm and general anesthesia was induced with remifentanil infusion at 0.3 microg x kg-1 x min- 1, pr

Repetitive generalized seizure-like activity during emergence from sevoflurane anesthesia.

PURPOSE: Sevoflurane-induced seizures are most often described during mask induction of anesthesia when high concentrations of the volatile agent are administered concomitant with alveolar hyperventilation. The occurrence of seizure-like activity during emergence from sevoflurane anesthesia has been rarely reported. CLINICAL FEATURES: We describe a patient who developed several episodes of generalized tonic-clonic seizure-like activity during and immediately after emergence from sevoflurane anesthesia and discuss the potential factors that contributed to this unusual complication. The patient

Recovery characteristics following maintenance of anaesthesia with sevoflurane or isoflurane in dogs premedicated with acepromazine.

A standard anaesthetic protocol was used to anaesthetise 40 dogs for intravenous urography and a retrograde urethrogram or vaginourethrogram. The dogs were allocated by blocked randomisation to receive either isoflurane or sevoflurane for maintenance of anaesthesia after they had been premedicated with acepromazine and pethidine, and anaesthesia induced with propofol. An observer who was unaware of which agent had been used assessed ataxia 30 and 60 minutes after discontinuation of administration of the anaesthetic and assigned an overall recovery score. No complications occurred during anaest

Anaesthesia in the older patient.

PURPOSE OF REVIEW: Clinical anaesthesia and analgesia address a growing number of elderly surgical patients. Ageing modifies physiology, pharmacokinetics and pharmacodynamics, and comorbidity is a common occurrence in the elderly. Therefore, based on recent information regarding perioperative outcome, indications and techniques should be individualized. RECENT FINDINGS: Clinical studies have highlighted the occurrence of postoperative cognitive dysfunction in elderly patients, and have given some information on its risk factors. As pain was found to be one of the most important of these, this

Neurological examination in patients recovering from general anesthesia.

We performed serial neurological evaluations on 86 patients who underwent surgery for herniated lumbar disk during the first 3 hours after the end of anesthesia (isoflurane-N20-02 for 25 patients, halothane-N20-02 for 38, fentanyl-droperidol-N20-02 for 23). At time 0, the idiomuscular response to percussion of the extensor carpi muscle was present in every patient, while tendon reflexes were always absent. Hyperreflexia was as frequent as shivering, but it took place earlier; we could determine no correlation between these two phenomena. There was no correlation between shivering and rectal te

[Anesthesia induction in children: propofol in comparison with thiopental following premedication with midazolam].

Propofol provides smooth and rapid induction of anesthesia in adults and guarantees rapid recovery. The use of propofol in adults is frequently associated with pain on injection, but this can be reduced by: (1) injection into the relatively large veins in the forearm or the antecubital fossa: (2) addition of lignocaine to the propofol; or (3) injection of an opioid (alfentanil) before propofol. Compared with experience in adults, there is very little experience with propofol in pediatric anesthesia. The aim of this random prospective study was to compare the induction characteristics of propof

Volatile versus intravenous anesthesia and postoperative neurocognition: A meta-analysis and trial-sequential analysis.

BACKGROUND: Postoperative delirium and delayed neurocognitive recovery are common and consequential complications of anesthesia and surgery. Maintenance of general anesthesia with propofol-based intravenous drugs may reduce postoperative neurocognitive complications compared with volatile anesthesia. METHODS: Major databases were searched through November 2025. The primary outcome was postoperative delirium within 7 postoperative days. Secondary outcomes included delayed neurocognitive recovery (formerly termed postoperative cognitive decline) and Mini-Mental State Examination scores on posto

Evaluation of a lidocaine with epinephrine infusion for reducing postoperative pain and complications in elective cat castrations.

This prospective, randomized, blinded clinical trial evaluated whether infusing lidocaine with epinephrine (LE) into spermatic cords during elective feline surgical castrations improved postoperative pain control and reduced acute postoperative complications. Five hundred one apparently healthy male cats presenting for elective surgical gonadectomy to a non-for-profit high-volume spay and neuter practice were entered into the study. Cats were a combination of feral, shelter-owned, and client-owned. Cats were anesthetized using a standardized, multimodal protocol that included buprenorphine, ke

Effect of Esketamine-Based Opioid-Sparing Anesthesia Protocol on the Quality of Early Recovery After Urological Surgery: A Randomized Clinical Trial.

PURPOSE: The quality of postoperative recovery under enhanced recovery after surgery protocols has always been the focus of anesthesiologists. It has been proven that esketamine application during the perioperative period can reduce the use of opioid drugs and improve the quality of postoperative recovery. The present study explored the effect of the esketamine-based opioid-sparing anesthesia protocol on the quality of postoperative recovery in patients undergoing elective urological surgery. PATIENTS AND METHODS: A randomized, double-blind, controlled clinical trial was adopted. Patients age

A Retrospective Study on Pre- and Intraoperative Predictors on the Recovery Quality of Horses After General Anesthesia.

Equine anesthesia is related to high morbidity and mortality rates and recent studies suggested that the period of recovery remains the phase associated with the greatest risk of mortality in horses. Intraoperative hypotension, hypoxemia, and hypercapnia are recognized as potential determinants of recovery quality. This study, conducted at the Equine University Hospital of Liege, aimed to explore how these factors influence recovery outcomes and compare complications between non-emergency and emergency procedures. We analyzed data from 1057 horses, with a mean weight of 498 kg (ranging from 15

Electroencephalogram Correlates of Delayed Emergence After Remimazolam-Induced Anesthesia Compared to Propofol.

BACKGROUND: Delayed emergence from anesthesia presents clinical challenges, including prolonged stays in the postanesthesia care unit (PACU). The neurobiological mechanisms underlying delayed emergence, particularly in remimazolam-induced anesthesia, remain poorly understood. This study aimed to explore patterns of brain electrical activity of delayed emergence in remimazolam-induced anesthesia by comparing dynamic changes in electroencephalogram (EEG) activity under various anesthesia states of remimazolam and propofol, focusing on the prefrontal region. METHODS: Forty-eight patients (age >1

Discharge one hour sooner: Comparing post-operative metrics between spinal and general anesthetics among male infants undergoing circumcision.

INTRODUCTION AND BACKGROUND: The aim of this study is to determine if spinal anesthesia (SA) for infant surgical circumcision results in shorter postoperative times compared to general anesthesia (GA). We hypothesized that, relative to GA, SA would have significantly shorter post-operative recovery times without increasing operative time or postoperative complications relative to GA. MATERIAL AND METHODS: A retrospective cohort study was conducted among infants receiving an outpatient surgical circumcision in the operating room (OR). The exposure was undergoing spinal anesthesia (n = 88) rela

Effects of sevoflurane versus other general anaesthesia on emergence agitation in children.

BACKGROUND: Sevoflurane is an inhaled volatile anaesthetic that is widely used in paediatric anaesthetic practice. Since its introduction, postoperative behavioural disturbance known as emergence agitation (EA) or emergence delirium (ED) has been recognized as a problem that may occur during recovery from sevoflurane anaesthesia. For the purpose of this systematic review, EA has been used to describe this clinical entity. A child with EA may be restless, may cause self-injury or may disrupt the dressing, surgical site or indwelling devices, leading to the potential for parents to be dissatisfi

[Use of sugammadex in patients undergoing caesarean section using general anesthesia with rocuronium].

Recently, rocuronium is being used in patients for caesarean section undergoing general anesthesia instead of suxamethonium. An increased dose of rocuronium improves intubating conditions but prolongs neuromuscular blockade. Sugammadex reverses rapidly and predictably even profound rocuronium-induced neuromuscular blockade. We experienced 13 cases of caesarean section patients undergoing general anesthesia with thiopental (3.5 mg x kg(-1)) and rocuronium (0.9 mg x kg(-1)). At the end of surgery, sugammadex (2 mg x kg(-1)) was administered every 3 minutes repeatedly until TOF>0.9. In two patien
