Effect of shunt surgery on idiopathic normal pressure hydrocephalus with incomplete Gerstmann syndrome: A CARE-compliant case report.

RATIONALE: Idiopathic normal pressure hydrocephalus (iNPH) occasionally co-exists with neurodegenerative disease, but its concurrence with Gerstmann syndrome (GS) has not been reported, leaving the reversibility of GS-like deficits after cerebrospinal fluid diversion unknown. PATIENT CONCERNS: A 77-year-old woman experienced a 1-year progressive decline in memory, object naming, and spatial orientation, eventually requiring institutional care. DIAGNOSES: Neurological examination revealed severe cognitive impairment (mini-mental state examination [MMSE] 4/30) with acalculia, agraphia, finger

Ubiquitin-positive frontotemporal lobar degeneration presenting with progressive Gogi (word-meaning) aphasia. A neuropsychological, radiological and pathological evaluation of a Japanese semantic dementia patient.

A patient with progressive anomia and alexia with agraphia for kanji (Japanese morphograms) is described. The patient showed a deficit in single-word comprehension and on-reading (a type of reading that conveys phonetic value) dominance in kanji reading, i.e. on-preceding (pronouncing first with on-reading, irrespective of its preferred reading) and kun-deletion (inability to recall and recognize kun-reading [another type of reading that conveys meaning]) when reading a single-character kanji. These features were due to loss of lexico-semantic information and thus the patient was regarded as h

Speech and Language Presentations of FTLD-TDP Type B Neuropathology.

Four right-handed patients who presented with an isolated impairment of speech or language had transactive response DNA-binding protein of 43 kDa (TDP-43) type B pathology. Comportment and pyramidal motor function were preserved at presentation. Three of the cases developed axial rigidity and oculomotor findings late in their course with no additional pyramidal or lower motor neuron impairments. However, in all 4 cases, postmortem examination disclosed some degree of upper and lower motor neuron disease (MND) pathology in motor cortex, brainstem, and spinal cord. Although TDP-43 type B patholo

Auditory processing skills and phonological representation in dyslexic children.

It is now well-established that there is a causal connection between children's phonological skills and their acquisition of reading and spelling. Here we study low-level auditory processes that may underpin the development of phonological representations in children. Dyslexic and control children were given a battery of phonological tasks, reading and spelling tasks and auditory processing tasks. Potential relations between deficits in dyslexic performance in the auditory processing tasks and phonological awareness were explored. It was found that individual differences in auditory tasks requ

Do the reading disabilities of children with cleft fit into current models of developmental dyslexia?

OBJECTIVE: The purpose of this study was to determine whether the reading problems of children with cleft fit models of developmental dyslexia. DESIGN: The study compared children with nonsyndromic cleft lip and palate who had reading disability (n = 46) with those who did not have reading disability (n = 46). The children were matched for age, sex, and grade. The two groups were compared (t tests) on measures of verbal expression, phonemic awareness, and rapid naming. Also, regression analyses compared the relative relationships of these variables to reading disability. PARTICIPANTS: Childr

Apraxia is not associated to a disproportionate naming impairment for manipulable objects.

The relevance of gesture knowledge for the semantic representation of manipulable objects was investigated in a series of 15 patients with a focal left-hemisphere lesion. The patients were classified according to the presence/absence of ideomotor and ideational apraxia. We investigated picture naming and word-picture matching (pointing to a picture on verbal command) with stimuli including items from three categories: manipulable objects, non-manipulable objects, and animals. The analysis was performed at group level and at single-patient level. Nine patients were affected by ideational apraxi

Can recognition of living things be selectively impaired?

Brain damage sometimes seems to impair recognition of living things, despite relatively preserved recognition of nonliving things. The most straightforward interpretation of this dissociation is that the recognition of living things depends on some specialized mechanisms that are not needed for the recognition of nonliving things. However, there are alternative interpretations of the dissociation in terms of the greater complexity or inter-item similarity of living things, or the more specific, within-category identifications that are usually required for living things. Surprisingly, the relev

Implicit access to semantic information.

Three experiments investigating the patient M.S.'s semantic memory are reported. Experiments 1 and 2 involved a category-membership decision task, in which M.S. was asked to determine whether a noun was a member of a specified semantic category. His performance in Experiment 1 was impaired for nouns from living categories in comparison with nouns from nonliving categories, and this impairment was especially marked for nouns of low typicality. Experiment 2 demonstrated an equivalent pattern of very poor performance to nouns of low familiarity from living categories. In Experiment 3 the effect o

An approach to analyzing a single subject's scores obtained in a standardized test with application to the Aachen Aphasia Test (AAT).

Methods for the analysis of a single subject's test profile(s) proposed by Huber (1973) are applied to the Aachen Aphasia Test (AAT). The procedures are based on the classical test theory model (Lord & Novick, 1968) and are suited for any (achievement) test with standard norms from a large standardization sample and satisfactory reliability estimates. Two test profiles of a Wernicke's aphasic, obtained before and after a 3-month period of speech therapy, are analyzed using inferential comparisons between (groups of) subtest scores on one test application and between two test administrations fo

Naming difficulties in children with dyslexia: application of the tip-of-the-tongue paradigm.

The present study used the tip-of-the-tongue (TOT) experimental paradigm in a picture naming task to explore the source of the naming deficits of children with dyslexia. Compared with a control group of typically developing readers, the children with dyslexia showed fewer correct responses and spontaneous recalls, more don't know (DK) and TOT responses, and less accurate feeling of knowing (FOK) judgments. When they failed to retrieve a target word, the children with dyslexia did not differ from the control group in the partial semantic information they provided, but they gave less valid and m

Refractory semantics in global aphasia: on semantic organisation and the access-storage distinction in neuropsychology.

A single case study is reported of a global aphasic patient, JM, with impaired access to semantic information which was particularly severe for the class of proper names. JM's ability to perform matching tasks with printed words and pictures to auditory words deteriorated when items were repeated, especially when the response-stimulus interval was short. Performance was also inconsistent across items. The effect of repeated testing on items generalised to other, previously untested members of the same category. Despite this, JM was able to access general semantic information about stimuli from

Category-specificity and modality-specificity in semantic memory.

Studies of agnosia have revealed two apparently orthogonal dimensions along which knowledge may break down. In some cases, knowledge of specific categories (such as living things) seems lost, regardless of the modality being tested. In other cases, knowledge in specific modalities (such as vision) seems lost, regardless of the category of stimuli being tested. These different sets of phenomena suggest different organizations for knowledge in the brain, the first by category and the second by modality. Unfortunately, possible confoundings between category, modality, and difficulty level in the

Naming performance in bilinguals with Alzheimer's disease and mild cognitive impairment.

BackgroundThere is consistent evidence of a disadvantage in bilinguals' speech production compared to monolinguals in healthy individuals, but studies investigating this phenomenon in clinical populations such as mild cognitive impairment (MCI) and Alzheimer's disease (AD) are scarce. Given that both clinical groups are characterized by word-finding difficulties, understanding how bilingualism influences speech production in these populations is essential.ObjectiveTo investigate the effect of bilingualism on dominant-language speech production in individuals with MCI and AD relative to cogniti

Clinical and neuroimaging investigations of language disturbance in frontotemporal dementia-motor neuron disease patients.

This study systematically investigated the neuropsychological profile of language disturbance in frontotemporal dementia-motor neuron disease (FTD-MND) using a data-driven approach. Neuroanatomical correlates of language profiles were also examined. Patients with FTD-MND (N = 26), pure motor neuron disease (N = 34), progressive non-fluent aphasia (N = 30), semantic dementia (N = 17), and controls (N = 31) underwent comprehensive language assessments. Clinical assessments were complemented with the Sydney Language Battery (SYDBAT), to assess semantic abilities, and the Test for Reception of Gra

The cognitive and neural expression of semantic memory impairment in mild cognitive impairment and early Alzheimer's disease.

Semantic deficits in Alzheimer's disease have been widely documented, but little is known about the integrity of semantic memory in the prodromal stage of the illness. The aims of the present study were to: (i) investigate naming abilities and semantic memory in amnestic mild cognitive impairment (aMCI), early Alzheimer's disease (AD) compared to healthy older subjects; (ii) investigate the association between naming and semantic knowledge in aMCI and AD; (iii) examine if the semantic impairment was present in different modalities; and (iv) study the relationship between semantic performance a

[Diagnostic criteria for fronto-temporal lobe degeneration].

Circumscribed atrophy of the frontal and temporal lobes (frontotemporal lobar degeneration) accounts for about one fifth of cases of primary degenerative dementia occurring before the age of 65. It produces three prototypical clinical syndromes. The most common is frontotemporal dementia, characterized by personality change and profound alteration in social conduct and associated with bilateral atrophy of the frontal and anterior temporal lobes. Progressive non-fluent aphasia is characterized by difficulty in verbal expression, anomia and phonemic errors in the presence of relative preservatio

Chromosome 14 linked familial Alzheimer's disease. A clinico-pathological study of a single pedigree.

The clinical features of three affected members of a British pedigree with familial Alzheimer's disease are presented. This pedigree is one of six included in an earlier study which demonstrated linkage to chromosome 14. The individuals were investigated clinically and neuropsychologically, using both PET and MRI over a 4-year period. Further information from three deceased individuals was obtained, including histopathological confirmation of Alzheimer's disease in one case which came to autopsy. The mean age at onset for this family was 43 years. Neurological examination revealed myoclonic je

We compared the writing ability between eyes open (looking at one's writing) and eyes closed (without looking) in a case of pure alexia. The patient is a 84-year-old right handed man who developed pure alexia following an infarction in the distribution of the left posterior cerebral artery. He showed right homonymous hemianopsia, slight memory disorder and slight anomia. He could not read any kinds of letters and words at all though he could categorize letters: Kana (phonograms), Kanji (morphograms) and Arabic numbers, and he could distinguish the real letters from false ones. He could achieve
