Predicting fetal asphyxia in intrahepatic cholestasis of pregnancy.

PURPOSE: Intrahepatic cholestasis of pregnancy (ICP) is a disease associated with high-perinatal morbidity and mortality rates. It is important to have parameters that aid in predicting fetal outcomes. Certain parameters affecting asphyxia in newborns to mothers with ICP are evaluated in this study. METHODS: One hundred eighty-seven cholestatic pregnancies were analyzed, retrospectively. Fetal asphyxia was defined as an APGAR score of less than 7 at 5 min postpartum. Predictors of asphyxia in ICP were analyzed by binary multivariate logistic regression analysis. RESULTS: Thirty-six of the ch

Autopsy findings in asphyxia in medical bed rails.

Lethal asphyxial entrapment between bedrails and mattresses is a well-recognized clinical event although there are few descriptions of autopsy findings. This convenience sample series of 29 deaths shows the infrequency of petechiae and laryngeal crush injuries and subtlety of soft-tissue trauma findings in such cases. Petechiae, plethora, and venous congestion may be noted in extremities that are pinned by rails in a manner that obstructs venous return. Medical examiners must mainly rely on eyewitness descriptions of the how the person was found and try to correlate this position with scanty s

Safety of deep hypothermia in treating neonatal asphyxia.

BACKGROUND: Several studies have demonstrated the efficiency and safety of mild hypothermia (33 degrees C) used for treating moderate encephalopathy. In animal models, deep hypothermia proved to be neuroprotective. OBJECTIVES: To determine the safety of whole-body deep hypothermia between 30 and 33 degrees C in moderate-severe hypoxic-ischemic encephalopathy in newborn term infants. METHODS: Mortality rates, incidence of brain damage detected by magnetic resonance imaging (MRI) and neurological outcomes of 39 term asphyxiated infants were retrospectively compared. A first group of patients (

Perinatal asphyxia pathophysiology in pig and human: a review.

In utero fetuses are evidently exposed to several factors that cause an interruption of the oxygen flow through the umbilical cord causing asphyxia leading to hypoxia and metabolic acidosis. These conditions are important causes of intra-partum and neonatal mortality. The main objective of this review is to provide current information regarding the pathophysiology of asphyxia in piglets around parturition; the physiological mechanisms invoked by affected piglets to compensate perinatal hypoxemia are discussed. This review also addresses some similarities and differences of asphyxia between pig

[Effects of hypothermia on cardiac function in neonates with asphyxia].

OBJECTIVE: To study the effects of hypothermia on cardiac function in neonates after birth asphyxia. METHODS: Fifty term newborns with Apgar score < 5 at 5 minutes were randomly divided into no cooling group (normothermia group, NG; rectal temperature = 36.5 degrees C +/- 0.5 degrees C, n = 27) and cooling group (hypothermia group, HG; rectal temperature = 34.5 degrees C +/- 0.3 degrees C, nasopharyngeal temperature = 34.0 degrees C +/- 0.5 degrees C, n = 23). The selective head cooling was applied to maintain nasopharyngeal temperature at 34 degrees C for 72 h in hypothermia group. Systolic

[Neurological prognosis of term infants with perinatal asphyxia].

Neonatal encephalopathies following birth asphyxia are the first features of cerebral insult. They never miss when asphyxia is directly involved in cerebral impairment. Mild encephalopathies have constantly a good prognosis. Conversely, moderate and severe encephalopathies are associated with poor outcome (death or severe handicap) in 25% to 100% of cases. Prognosis of these moderate and severe encephalopathies can be assessed during the first ten days of life by 3 complementary ways: clinical exam, electrophysiology and imaging. The most information is obtained from the EEG and MRI which toge

Effect of asphyxia on the pupils of brain dead subjects.

The pupils dilate following cardiac arrest but the mechanism is unknown. If pupillary dilation represents inadequate blood supply to the midbrain, pupil size might be a rough guide to the adequacy of the resuscitation effort. The brain dead organ harvest patient presents a unique opportunity to study pupillary activity in the absence of an intact midbrain and to examine the effects of asphyxia on the pupil. Because the midbrain is dead in these subjects, the pupil has no supraspinal autonomic control and following aortic cross clamp, no blood can be delivered to the orbit. Ten brain dead patie

Energy metabolism in graded perinatal asphyxia of the rat.

Although information on energy metabolism during hypoxemic-ischemic states is abundant, data on perinatal asphyxia (PA) are limited. As results from hypoxia-ischemia cannot be directly extrapolated to PA, a clinical entity characterized by acidosis, hypoxemia and hypercapnia, we decided to use a rat model of graded PA during delivery. Cesarean section was performed at the 21st day of gestation and the pups, still in the uterus horns, were asphyxiated from 0 to 20 minutes. In this model survival decreases with the length of asphyxia. Early changes of energy-rich phosphates in brain, heart and k

Cerebrospinal nerve growth factor--a marker of asphyxia?

Asphyxia in neonates is characterized by different degrees of hypoxia-ischemia, with the outcome depending on the severity of the underlying brain cell damage. Neurotrophic factors rescue neurons from cell death after injury and promote neuronal survival during development. The authors have used enzyme-linked immunosorbent assay to study levels of nerve growth factor in the cerebrospinal fluid of children with asphyxia at birth (n = 10) and of controls (n = 23). Compared with reference groups the children who had had severe asphyxia had lower or negligible levels of cerebrospinal fluid nerve g

The use of hypothermia: a role in the treatment of neonatal asphyxia?

Perinatal asphyxia remains one of the most devastating neurologic processes. Although the understanding of the pathophysiology after perinatal asphyxia is extensive, there are few therapeutic interventions available to prevent or even mitigate the devastating process that unfolds after injury. The search for a safe and efficacious therapy has prompted scientists and clinicians to consider various promising therapies. One such therapy is therapeutic hypothermia. On the basis of adult, pediatric, and animal research, there is increasing evidence to suggest that therapeutic hypothermia may be an

Admission test as predictor of intrauterine fetal asphyxia.

This study determined the accuracy of the admission test in predicting intrauterine fetal asphyxia. A total of 229 subjects were included. The admission test, a short continuous electronic fetal monitoring recording, was made immediately on admission, on all patients in labor, and was categorized as "reassuring", "equivocal", or "ominous". Intrauterine fetal asphyxia was considered to be present when the umbilical cord blood pH was 7.2 and/or the Apgar score was 7 at 1 minute; an "ominous" admission test has a high accuracy (LR = 8.54) in predicting intrauterine fetal asphyxia, while a "reassu

Asphyxia due to oxygen deficiency by gaseous substances.

The determination of the cause of death in asphyxiation gas cases is very difficult because of the variation in circumstances surrounding such deaths. To clarify the cause of death and to identify the factors involved in asphyxia, the symptoms during asphyxia, the concentration of gases at the respiratory arrest, the time to death and the concentration of the gaseous substances in the tissues were studied using rats and six gases. Three inhalations were used: (1) rapid asphyxia (2-3 min) in the exposure chamber in which the oxygen was depleted completely, (2) prolonged asphyxia (20-25 min) by

The fetal brain: metabolic and circulatory responses to asphyxia.

The fetal environment is well suited for normal brain growth and development with oxygen availability well in excess of oxidative needs. With impairments in blood gas exchange, cerebral oxidative metabolism is initially maintained by an increase in cerebral blood flow, thus protecting the 'oxygen margin of safety', and, when this response becomes limited, by an increase in the brain's fractional extraction of oxygen. Additional adaptive mechanisms involve substrate alterations and a decrease in energy consuming processes, including growth restriction and behavioural state alterations. Although

Some animal models for the study of perinatal asphyxia.

Much of our current understanding concerning the pathophysiology of perinatal brain disorders has evolved from animal studies over the past three decades. Fetal and neonatal nonhuman primate, pregnant sheep, lamb, puppy, piglet and immature rodents, all have been important animal models for perinatal brain research. Although no model can be considered 'perfect' in reflecting the variety and complexity of human brain pathology, the investigator must assess the merits and limitations of each model within the framework of the research questions being asked. A variety of experimental designs have

Perinatal features of birth asphyxia and neurologic outcome.

The perinatal events of 40 term newborn infants who had severe birth asphyxia were reviewed and correlated with the neurological outcome at a mean age of 2 1/2 years. Severe birth asphyxia was defined as an Apgar Score of less than or equal to 3 at 1 min. The majority (68.5%) of the babies had an abnormal fetal heart pattern before delivery. Persistent fetal bradycardia was most frequently associated with poor outcome. The neurological staging, the need for assisted ventilation for more than 24 hours and severe renal impairment were the best predictors of neonatal death or subsequent poor neur

A case of autoerotic asphyxia associated with multiplex paraphilia.

During the past 20 years, the sensational aspects of autoerotic fatalities have captured the attention of medical examiners, psychiatrists, law enforcement agents, and the public, as well as the individuals themselves who engage in these dangerous practices. Reports of deaths related to sexual asphyxia have been presented numerous times at national and international meetings and have been the topic of discussion on television talk shows and in the press. Autoerotic fatalities and all the sexual curiosities related to these activities have prompted death scene investigators to publish case repo

Use of lithium carbonate in the treatment of autoerotic asphyxia.

We report the successful use of lithium carbonate to promptly extinguish life-threatening behavior in a case of autoerotic asphyxia occurring in a 26-year-old male. In addition to his paraphilia, the patient met the DSM-III-R criteria for dysthymia, and had an avoidant personality disorder secondary to verbal, physical and sexual abuse inflicted during childhood and adolescence.
