B-type natriuretic peptide in Parkinson's disease: a novel biomarker of dysautonomia.

BACKGROUND: The presence of cardiac diseases in Parkinson's disease (PD) patients opens the debate on the identification of early biomarkers of heart damage. N-terminal pro-brain natriuretic peptide (NT-proBNP) may be involved in cardiac autonomic denervation. OBJECTIVES: i) to investigate whether there is a relationship between NT-proBNP and measures of cardiovascular autonomic function in PD patients; ii) to elucidate the discrimination power of NT-proBNP levels to identify PD patients with dysautonomia. METHODS: Thirty-one consecutive PD patients were enrolled. NT-proBNP levels were measu

Hemodynamic and ventilatory response to different levels of hypoxia and hypercapnia in carotid body-denervated rats.

OBJECTIVE: Chemoreceptors play an important role in the autonomic modulation of circulatory and ventilatory responses to changes in arterial O(2) and/or CO(2). However, studies evaluating hemodynamic responses to hypoxia and hypercapnia in rats have shown inconsistent results. Our aim was to evaluate hemodynamic and respiratory responses to different levels of hypoxia and hypercapnia in conscious intact or carotid body-denervated rats. METHODS: Male Wistar rats were submitted to bilateral ligature of carotid body arteries (or sham-operation) and received catheters into the left femoral artery

Topical apraclonidine testing discloses pupillary sympathetic denervation in diabetic patients.

BACKGROUND: Autonomic denervation is common in diabetes mellitus (DM). Pupillary sympathetic denervation (PSD) has been found in Horner syndrome following instillation of apraclonidine 0.5%. We have applied this technique to investigate the prevalence of PSD in DM. METHODS: Apraclonidine 0.5% was instilled in the eyes of 50 patients with DM and 30 age-matched and gender-matched subjects without DM (control subjects). Pupil diameters (PD) were measured before and 60 minutes after instillation. The duration of DM and the degree of diabetic retinopathy (DR) were recorded for each patient. RESUL

Changes in autonomic nervous activity after catheter ablation of atrial tachycardia arising from the atrioventricular annulus.

Radiofrequency (RF) catheter ablation of supraventricular tachycardias causes local parasympathetic denervation. This study used heart rate variability (HRV) to evaluate the effects of ablation of atrial tachycardia (AT) arising from the atrioventricular annulus (AVAT) on autonomic function. Ten patients with AVAT were referred for ablation (group AT) and compared with 8 patients with paroxysmal atrial fibrillation who underwent PV isolation (group Paf), and 13 patients with idiopathic ventricular tachycardia successfully treated by ablation (group VT). Time and frequency domain analysis of HR

Pterygopalatine Ganglion Cell Death as an Underrecognized Mechanism in Dry Eye Syndrome: A Hypothesis with Special Attention to Diabetic Etiology.

PURPOSE: To propose that progressive neuronal and satellite glial cell death within the pterygopalatine ganglion (PPG) is an underrecognized upstream mechanism in chronic aqueous-deficient dry eye, with particular relevance to diabetes mellitus, and to examine whether this ganglionic lesion also unifies broader secretory and cerebrovascular abnormalities into a single "diabetic secretory denervation syndrome". METHODS: A narrative, hypothesis-driven synthesis was performed across ocular surface disease, autonomic neuroscience, diabetic neuropathy, regulated cell death, surgical denervation mo

Selection of patients with symptomatic vagal-induced sinus node dysfunction: Who will be the best candidate for cardioneuroablation?

Sinus node dysfunction is a multifaceted disorder with variable manifestations, the prevalence of which increases with age. In a specific group of patients, excessive vagal activity may be the sole cause for this condition. These patients are characterized as having recurrent daytime symptoms attributed to bradyarrhythmia, no evidence of organic sinus node lesions, cardiac vagal overactivation, and are non-elderly. For sinus node dysfunction patients, a permanent pacemaker implantation appears to be the ultimate solution, although it is not an etiological treatment. Cardioneuroablation is a pr

The trigeminal (V), an oft-neglected nerve.

Corneal denervation is one of the most serious insults that can occur to an eye; disease symptoms can be suppressed, and epithelial breakdown is likely to occur. In the cases presented, failure to test corneal sensation delayed diagnosis and endangered the cornea. Conditions affecting the trigeminal sensory pathway, including those causing a pain-anesthesia paradox, are described. Corneal sensation must be tested in all instances of suspicious head pain or paresthesia, head trauma, cranial nerve defect, cranial autonomic dysfunction, or unexplained corneal epithelial defect. The trigeminal ner

Shy-Drager syndrome. A review and a description of the anaesthetic management.

Autonomic failure in patients with the Shy-Drager syndrome may produce cardiovascular instability during anaesthesia and surgery. The syndrome is reviewed and the anaesthetic management of a case is described. The choice between general and regional anaesthesia seems to be less important than adequate cardiovascular monitoring and the maintenance of blood pressure with intravenous fluids. Sympathomimetic drugs, if used at all, should be administered in very dilute solutions to avoid hypertension from denervation hypersensitivity. In the postoperative period, symptoms from orthostatic hypotensi

A rat study of the use of end-to-side peripheral nerve repair as a "babysitting" technique to reduce the deleterious effect of chronic denervation.

OBJECTIVE: Functional recovery is disappointing after surgical repair of nerves that are injured far from their target organs and/or after delayed repair. In the former case, a nerve transfer that transects a distal nerve fascicle to innervate denervated targets is one strategy to promote nerve regeneration and functional recovery. An alternate strategy tested in this study is to perform an end-to-side neurorrhaphy to "babysit" (protect) the denervated distal nerve stump at the time of nerve repair and reduce the deleterious effect of chronic denervation on nerve regeneration. METHODS: In the

Control of the nasal mucosa by the tonic activities of the autonomic nervous system in dogs.

We studied the effects of stimulation and acute denervation of the cervical sympathetic nerve and the vidian nerve on the nasal vascular tone, as measured by intranasal balloon pressure. Significant vasoconstriction was found during electrical stimulation of the cervical sympathetic nerve. When the cervical sympathetic nerve was sectioned, causing a transient vasoconstriction due to the stimulatory effect of the nerve injury, then significant vasodilation was found in 23 out of 30 experiments. Significant vasodilation during electrical stimulation of the vidian nerve and slight but significant

Sympathetic Nervous System Contributions to Hypertension: Updates and Therapeutic Relevance.

The sympathetic nervous system plays a pivotal role in the long-term regulation of arterial blood pressure through the ability of the central nervous system to integrate neurohumoral signals and differentially regulate sympathetic neural input to specific end organs. Part 1 of this review will discuss neural mechanisms of salt-sensitive hypertension, obesity-induced hypertension, and the ability of prior experiences to sensitize autonomic networks. Part 2 of this review focuses on new therapeutic advances to treat resistant hypertension including renal denervation and carotid baroactivation. B

Is renal denervation still a treatment option in cardiovascular disease?

The role of renal sympathetic denervation (RDN) has been the topic of ongoing debate ever since the impressive initial results. The rationale of RDN is strong and supported by non-clinical studies, which lies in uncoupling the autonomic nervous crosstalk between the kidneys and the central nervous system. Since we know that cardiovascular diseases, such as hypertension, atrial, ventricular arrhythmias and heart failure (HF) are related to sympathetic (over)activity, modulation of the renal nerve activity appears to be a reasonable and attractive therapeutic target in these patients. This revie

Denervation or stimulation? Role of sympatho-vagal imbalance in HFpEF with hypertension.

Heart failure (HF) in the elderly is an increasingly large and complex problem in modern society. Notably, the cause of HF with preserved ejection fraction (HFpEF) is multifactorial and its pathophysiology is not fully understood. Among these, hypertension has emerged as a pivotal factor in the pathophysiology and therapeutic targets of HFpEF. Neuronal elements distributed throughout the cardiac autonomic nervous system, from the level of the central autonomic network including the insular cortex to the intrinsic cardiac nervous system, regulate the human cardiovascular system. Specifically, i

Sympathetic neuroimaging.

Sympathetic neuroimaging provides an important supplement to physiological, neurochemical, and neuropharmacological approaches in the evaluation of patients with clinical autonomic disorders. Almost all sympathetic neuroimaging to date has involved visualization of noradrenergic innervation in the left ventricular myocardium. Single-photon emission computed tomography (SPECT) scanning after injection of the sympathomimetic amine (123)I-metaiodobenzylguanidine ((123)I-MIBG) constitutes by far the most commonly used means worldwide to assess cardiac sympathetic innervation. Based on heart:medias

[Cold pressor test in the evaluation of autonomic neuropathy].

Neurogenic orthostatic hypotension is due to interruption of the baroreceptor reflex. To optimize the pharmacological therapy in this clinical setting the lesion should be carefully pinpointed and the degree of denervation established. To this purpose 7 patients with autonomic failure underwent ambulatory intraarterial monitoring, which showed the reversal of the normal 24-hour blood pressure (BP) pattern. During daytime BP gradually rose from its lowest point early in the morning to a peak during late evening. No BP fall was observed during sleep. Tilting test, Valsalva maneuver, hyperventila

The sympathetic nervous system in diabetic neuropathy. A clinical and pathological study.

Tests of autonomic function were performed on 16 subjects with diabetic neuropathy. Abnormal sweating occurred in 10/10 (100 per cent), postural hypotension in 7/16 (44 per cent), an abnormal Valsalva ratio in 7/11 (64 per cent), and denervation hypersensitivity to phenylephrine in 2/8 (25 per cent) of patients tested. A quantitative assessment of baroreceptor function was made. In diabetics, there was a reduced resting heart period, heart period range and mean gain. Quantitative histological studies were performed on the greater splanchnic nerves removed at autopsy from 9 control subjects and

Reduced myocardial 123-iodine metaiodobenzylguanidine uptake: a prognostic marker in familial amyloid polyneuropathy.

BACKGROUND: Transthyretin familial amyloid polyneuropathy is a hereditary form of amyloidosis characterized by sensorimotor and autonomic neuropathy, cardiac conduction defects, and infiltrative cardiomyopathy. Previous studies have suggested that myocardial sympathetic denervation assessed by 123-iodine metaiodobenzylguanidine (MIBG) imaging occurs early in disease progression. However, its prognostic significance was never evaluated. We aimed to study the long-term prognostic value of myocardial sympathetic denervation detected by MIBG imaging in transthyretin familial amyloid polyneuropathy

Olfactory dysfunction in pure autonomic failure: Implications for the pathogenesis of Lewy body diseases.

BACKGROUND: Pure autonomic failure (PAF) and Parkinson disease (PD) both are Lewy body diseases, and both entail substantia nigra dopaminergic, locus ceruleus noradrenergic, and cardiac sympathetic denervation. Multiple system atrophy (MSA) is a non-Lewy body disease in which alpha-synuclein accumulates in glial cells, with central catecholamine deficiency but preserved cardiac sympathetic innervation in most patients. PD is associated with more severe and consistent olfactory dysfunction than in MSA; whether PAF entails olfactory dysfunction has been unknown. In this study we assessed olfacto
