BACKGROUND: Despite technical progress and novel ablation strategies, pulmonary vein (PV) reconnection still occurs in a substantial proportion of patients. The aim of the study was to determine the impact of the elimination of antral low-voltage, fragmented electrograms (LFEGMs) identified by high-density (HD) mapping on the rate of pulmonary vein isolation (PVI) durability.
METHODS: Sixty patients with paroxysmal atrial fibrillation (PAF) were randomly assigned to a verification of PV entrance block and presence of LFEGMs on antral isolation lines with an HD mapping catheter (HD group) or t

BACKGROUND: Centrifugal activation is not always the origin of a focal atrial tachycardia (AT) ("true-focal"), but passive activation from the other structures ("pseudo-focal").
OBJECTIVE: We aimed to establish a method to differentiate true-focal from pseudo-focal.
METHODS: In 49 centrifugal activations in 35 patients with AT, 12-lead electrocardiogram, activation map, atrial global activation histogram (GAH), and local electrograms were analyzed. GAH demonstrates the relation between the activation area and timing through the cycle length, displayed with a normalized value, ranging from 0

BACKGROUND: Fast ventricular tachycardias (VTs) have historically been attributed to shorter path lengths with smaller reentrant circuit dimensions in animal models. The relationship between the dimensions of the reentrant VT circuit and tachycardia cycle length (TCL) has not been examined in humans. This study aimed to analyze the determinants of the rate of human VT with comparison of circuit dimensions and conduction velocity (CV) across a wide range of both stable and unstable VTs delineated by high-resolution mapping.
METHODS: Fifty-four VTs with complete circuit delineation (>90% TCL) b

BACKGROUND: Localized drivers are proposed mechanisms for persistent atrial fibrillation (AF) from optical mapping of human atria and clinical studies of AF, yet are controversial because drivers fluctuate and ablating them may not terminate AF. We used wavefront field mapping to test the hypothesis that AF drivers, if concurrent, may interact to produce fluctuating areas of control to explain their appearance/disappearance and acute impact of ablation.
METHODS: We recruited 54 patients from an international registry in whom persistent AF terminated by targeted ablation. Unipolar AF electrogr

INTRODUCTION: Bipolar voltage is used during electroanatomic mapping to define abnormal myocardium, but the effect of activation rate on bipolar voltage is not known. We hypothesized that bipolar voltage may change in response to activation rate. By examining corresponding unipolar signals we sought to determine the mechanisms of such changes.
METHODS AND RESULTS: LA extrastimulus mapping was performed during CS pacing in 10 patients undergoing first time paroxysmal atrial fibrillation ablation. Bipolar and unipolar electrograms were recorded using a PentaRay catheter (4-4-4 spacing) and indi

A young woman with Epstein's anomaly had a large endocardial scar in the atrialised ventricular myocardium. In patients with significant preoperative ventricular scarring, more information is needed to help appropriately manage the risk of ventricular arrhythmias and sudden death.

PURPOSE: For patients with symptomatic atrial fibrillation (AF), a curvilinear multi-electrode ablation (MEA) catheter has been reported to be successful to achieve pulmonary vein (PV) isolation. However, this approach has not been compared prospectively with conventional PV isolation (CPVI) using a standard circular mapping catheter and 3D electro-anatomic mapping. In this prospective non-randomized study, we compared the efficacy of these two techniques.
METHODS: Of 185 consecutive patients, age 54.6 ± 10.1 years, with symptomatic paroxysmal AF (PAF), 96 patients underwent PV isolation by C

Radiofrequency catheter ablation as a curative method for atrial fibrillation (AF) has become increasingly popular. Patients with paroxysmal AF have been treated by catheter ablation with great success, but so far this treatment has been less effective for patients with persistent AF. Usually there are multiple triggers or substrates during persistent AF and their exact locations are unclear. On the other hand, the non-contact mapping system (Ensite 3000, St Jude Medical) producing thousands of virtual endocardial electrograms, has gradually become accepted as a powerful tool to use on patient

Atrial arrhythmias are a common sequel after surgery for congenital heart disease, and often eventually cause late morbidity and mortality. Nowadays, percutaneous catheter ablation of atrial flutter (AFL) is widely practised, and is highly successful and safe. Patients with a tricuspid valve replacement (TVR) represent a distinct group for whom the appropriate nonpharmacologic therapy for AFL has not yet been established. We report a case of a 55-year-old woman with a history of a total correction of tetralogy of Fallot (ToF) who developed AFL before and after receiving a tricuspid valve prost

BACKGROUND: Multiple remodeling patterns have been observed after catheter ablation of atrial fibrillation (AF).
OBJECTIVE: We aimed to clarify the electrical/structural properties associated with recurrences after ablation of chronic AF.
METHODS: After a stepwise ablation procedure in 120 consecutive patients with persistent/long-lasting persistent AF, 36 had a recurrence of AF (Group 1/Group 2: recurrence with paroxysmal/persistent AF, n = 16/20).
RESULTS: During the first procedure, the left atrial (LA) bipolar voltage did not differ between the 2 groups, and the LA volume was smaller in

Recently, a novel radiofrequency (RF) ablation system has been developed to perform pulmonary vein (PV) isolation. The system consists of a decapolar, steerable, over-the-wire mapping and ablation catheter combined with a multichannel RF generator that delivers energy in a temperature-controlled, power-limited fashion in both uni- and bi-polar modes. Using this technique, long continuous ablation lesions can be created within the left atrial antrum around the PV ostium. Electrical disconnection of PVs can be achieved in 93% of targeted PVs. Medium-term success is reported as 79.5% of patients

AIMS: Areas with complex fractionated electrograms are commonly targeted during ablation of persistent atrial fibrillation (AF). These signals are, however, found in most sampled areas of the left atrium (LA), implying the need for further differentiation.
METHODS AND RESULTS: Electrograms were recorded over 60 s at eight different LA endocardial sites in 10 patients with persistent AF, using a fully automated algorithm. These were analysed in sequential 2 s segments for activity, mean amplitude, continuous activity percentage, and dominant frequency (DF). All three time-domain measures diffe

The congenital long-QT syndrome is commonly associated with a high risk for polymorphic ventricular tachy-cardia and sudden cardiac death. This is probably due to an intensification of the intrinsic heterogeneities present in ventricular myocardium. Increasing the electrophysiological heterogeneities amplifies the dispersion of repolarization which directly affects the morphology of the T wave in the ECG. The aim of this work is to investigate the effects of LQT2, a specific subtype of the long-QT syndrome (LQTS), on the Body Surface Potential Maps (BSPM) and the ECG. In this context a three-d

INTRODUCTION: Characteristics of the 12-lead ECG during VT are used to guide initial placement of mapping catheters in endocardial ventricular tachycardia (VT) ablation. Previously constructed algorithms for guidance in human infarct-related VT are limited to patients known to have anterior or inferior infarcts only. We hypothesized that 12-lead ECG characteristics could be used to determine VT exit site in patients with all types of infarction of unknown location.
METHODS AND RESULTS: From noncontact activation maps of 121 LV VT in 51 patients undergoing catheter ablation, VT exit sites were

BACKGROUND: Inappropriate sinus tachycardia (IST) is characterized by heart rate (HR) increase out of proportion to stress level. Radiofrequency (RF) modification of the sinus node (SN) is an accepted treatment modality for medically refractory IST. We describe a new technique using noncontact mapping and a saline irrigated catheter for SN modification.
METHODS: Seven consecutive patients with medically refractory IST were referred for ablation. Intrinsic heart rate (IHR) was calculated with complete autonomic blockade by atropine and propranolol. Isoproterenol (ISO) 1 mcg/min was initiated a

Cardiac resynchronization therapy (CRT) using biventricular (BiV) pacing has been developed to restore synchrony and improve cardiac performance in patients with heart failure. It has been clinically beneficial in 65% to 70% of patients. Being an electrical (pacing) approach, detailed electrical information during CRT is critical in understanding its mechanism and clinical outcome. However, electrical data from patients have been limited because of the requirement for invasive mapping. Electrocardiographic imaging provides the necessary tool to noninvasively obtain this information. We applied

Recognition of left anterior fascicle block is usually a diagnostic problem. Isochrone maps, displaying results of the body surface potential mapping are helpful in solving this problem. The isochrone maps present changes of heart electric field, especially those concerning a pathway of the depolarization front propagation within the heart conduction system. Body surface potential mapping registrations were performed using a Fukuda Denshi cylindrical system, which enables simultaneous recordings of electrocardiographic signals from the 87 leads. The examined group consisted of 29 patients with
