Microbiota and enteric nervous system crosstalk in diabetic gastroenteropathy: bridging mechanistic insights to microbiome-based therapies.

Diabetes mellitus has emerged as a global public health crisis, with over half of patients experiencing gastrointestinal (GI) symptoms that exacerbate glucose fluctuations and impair quality of life. While prior research on the pathophysiology of diabetic gastroenteropathy (DGE) focused primarily on autonomic neuropathy, particularly involving the vagus nerve, recent studies have shifted toward the impairment of the enteric nervous system (ENS). As the largest autonomous neural network governing GI motility independent of central control, structural and functional abnormalities of the ENS cons

Blinatumomab after R-CHOP bridging therapy for patients with Richter transformation: a phase 2 multicentre trial.

Richter transformation (RT) is an aggressive lymphoma occurring in patients with chronic lymphocytic leukaemia. Here we investigated the anti-CD3/anti-CD19 T-cell-engager blinatumomab after R-CHOP (i.e. rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisone) in patients with untreated RT of diffuse large B-cell lymphoma histology (NCT03931642). In this multicentre phase 2 study, patients without complete response (CR) after two cycles of R-CHOP were eligible to receive an 8-week blinatumomab induction via continuous vein infusion with stepwise dosing until 112 μg/day. The primar

Donor-Recipient Chimeric Cell Transplantation as the Bridging Therapy for Mitigating Total Body Irradiation-Induced Injury.

In recent years, cell-based therapies have emerged as a promising approach for mitigating radiation-induced injury. Acute radiation syndrome (ARS) results from exposure to high doses of radiation over a short time period. This study aimed to compare the efficacy of donor-recipient chimeric cell (DRCC) therapy in mitigating ARS induced by a total body irradiation (TBI) dose of 10 gray (Gy). Thirty irradiated Lewis rats were employed as ARS models to assess the efficacy of systemic-intraosseous transplantation of different cellular therapies in five experimental groups (n = 6/group): saline cont

Stem cell therapy for degenerative disc disease: Bridging the gap between preclinical promise and clinical potential.

Stem cell therapy has gained attention in the field of regenerative medicine due to its potential to restore damaged tissue. This article focuses on the application of stem cell therapy for treating spinal pathologies, particularly intervertebral disc degeneration. Disc degeneration is a major cause of low back pain and is characterized by changes in the matrix and inflammation. Animal studies have demonstrated that the implantation of mesenchymal stem cells (MSCs) yields promising results, including increased disc height, improved hydration, and reduced inflammation. However, the number of cl

Reinforced antimyeloma therapy via dual-lymphoid activation mediated by a panel of antibodies armed with bridging-BiTE.

Immunotherapy using bispecific antibodies including bispecific T-cell engager (BiTE) has the potential to enhance the efficacy of treatment for relapsed/refractory multiple myeloma. However, myeloma may still recur after treatment because of downregulation of a target antigen and/or myeloma cell heterogeneity. To strengthen immunotherapy for myeloma while overcoming its characteristics, we have newly developed a BiTE-based modality, referred to as bridging-BiTE (B-BiTE). B-BiTE was able to bind to both a human immunoglobulin G-Fc domain and the CD3 molecule. Clinically available monoclonal ant

Successful Bridging Therapy in a 103-Year-Old Woman with Acute Terminal Internal Carotid Artery Occlusion.

The efficacy of intravenous thrombolysis and endovascular therapy and their favorable treatment outcomes have been established in clinical trials irrespective of age. Current guidelines do not recommend an age limit in selecting eligible patients for reperfusion treatment as long as other criteria are satisfied. A 103-year-old woman was admitted at our hospital within 1 h of stroke onset secondary to a left internal carotid artery terminus occlusion. On admission, her National Institutes of Health Stroke Scale (NIHSS) score was 30, with a small left thalamic diffusion restriction lesion on MRI

Suspected intraoperative formation of left atrial thrombus in a patient with atrial fibrillation receiving bridging anticoagulation therapy.

We present a patient with atrial fibrillation (AF) in whom a left atrial (LA) thrombus might have formed during laparotomy despite bridging anticoagulation therapy. No evidence of thrombus was detected by transesophageal echocardiography (TEE) at the start of surgery; however, a thrombus measuring 13 × 10 mm was found in the LA appendage by the end of the procedure, suggesting that thrombus might develop intraoperatively in patients with AF even when bridging anticoagulation is properly established. Intraoperative TEE can assist in detecting intracardiac thrombus in patients with AF regardless

Mobilized progenitor cells as a bridging therapy for radiation casualties: a brief review of tocopherol succinate-based approaches.

Nuclear detonation through either military or terrorist action would most likely lead to a mass-casualty scenario involving victims with varying degrees of exposure to ionizing radiation. As a result of radiation injury to the hematopoietic system, victims would suffer from a lack of red blood cells that deliver oxygen, immune cells that detect and eliminate infectious agents, and blood platelets that promote blood clot formation. In part, these symptoms are generally referred to as acute radiation syndrome (ARS). While some victims of moderate to high levels of radiation will be beyond saving

Cell and Gene Therapy for Patients Suffering from Xerostomia (Dry Mouth): Positioning Extracellular Vesicles as the Bridge Between Biomarker Discovery and Regenerative Therapy in Xerostomia-A Scoping Review.

Xerostomia is a common and debilitating condition caused by salivary gland dysfunction, frequently associated with primary Sjögren's syndrome and head and neck radiotherapy. Current management is largely symptomatic and does not address underlying glandular injury. Extracellular vesicles (EVs), including exosomes, have emerged as candidate mediators of intercellular communication that have been proposed for diagnostic and therapeutic applications; however, their translational relevance to xerostomia remains uncertain and is currently supported only by exploratory evidence. This scoping review

Cyclosporine A and Romiplostim as a Bridging Therapy to Curative Surgery in Aplastic Anemia Complicated by Gastric Cancer: A Case Report.

We present a case of aplastic anemia (AA) complicated with gastric cancer that was successfully treated with cyclosporine A (CsA) and romiplostim (ROMI) as a bridging therapy before curative surgery. This patient had untreated gastric cancer at the time of AA diagnosis, and surgery was difficult because of thrombocytopenia. However, CsA and ROMI were effective, resulting in a rapid increase in the platelet count, thereby making surgery for gastric cancer possible. CsA and ROMI as a bridging therapy to curative surgery may therefore represent a feasible treatment option in patients with AA comp

Continuous Subcutaneous Apomorphine as a Temporary Bridging Therapy for Dopamine Dysregulation Syndrome in Patients with Deep Brain Stimulation.

BACKGROUND: Dopamine dysregulation syndrome (DDS) is a challenging complication of dopaminergic therapy in Parkinson's disease and can persist despite subthalamic deep brain stimulation (STN-DBS). We evaluated a temporary bridging concept in which oral dopaminergic medication was replaced by continuous subcutaneous apomorphine infusion while maintaining DBS. CASES: Six patients with STN-DBS and DDS were temporarily transitioned from oral dopaminergic medication to continuous subcutaneous apomorphine infusion. Treatment success was defined as reduction of DDS below a predefined threshold (<3 c

Bridging exercise biology and immunometabolism: A novel irisin pathway toward next-generation metabolic and neurodegenerative therapies.

Recent work by Mu et al. identifying irisin as a modulator of adipose tissue IL-33 and regulatory T cells introduces a new paradigm in immunometabolic biology, shifting attention from thermogenesis alone toward immune-stromal crosstalk as a determinant of metabolic health. By inducing IL-33 production in adipose mesenchymal stromal cells, irisin preserves ST2+ regulatory T cells (Tregs) in visceral adipose tissue, thereby restraining inflammation, improving insulin sensitivity, and promoting metabolic homeostasis. This mechanism expands the concept of exercise-induced metabolic protection by h

Bridging Conservative Management With Injectable Orthobiologic Therapies: Proceedings of the International Consensus Meetings on Cartilage Repair of the Ankle.

BackgroundThe management of osteochondral lesions of the talus (OLT) is challenging, with 2 main options being conservative and surgical management. Orthobiologic therapies come to bridge the gap between these 2 options; however, inconsistencies in indication, composition, and heterogeneous evidence limit clinical decision-making. To address these challenges, 3 International Congress Meetings on Cartilage Repair of the Ankle were convened in Pittsburgh, USA (2017), Dublin, Ireland (2019), and New York, USA (2025) to establish agreement on the role of conservative management and injectable orth

Bridging the "Valley of Death" in Antifungal Therapy: Next-Generation Biomimetic and Exosome-Inspired Nanocarriers for Invasive Candidiasis.

Invasive candidiasis, predominantly driven by multidrug-resistant Candida species and intractable biofilms, represents an escalating global health crisis with mortality rates rivaling major infectious diseases. The clinical efficacy of conventional antifungal agents-azoles, polyenes, and echinocandins-is severely compromised by poor tissue penetration, dose-limiting systemic toxicity, and the rapid evolution of complex resistance mechanisms. Here, we review the two-decade structural evolution of nanotechnological interventions designed to overcome these pharmacological and biological barriers.

Bridging the gap between experimental evidence and clinical applications of antipseudomonal phage therapy against Pseudomonas aeruginosa.

Pseudomonas aeruginosa is one of the World Health Organization's (WHO) priority pathogens requiring urgent intervention because of its remarkable ability to develop antimicrobial resistance. The rapid emergence and global dissemination of multidrug-resistant P. aeruginosa have made the treatment of these infections increasingly challenging, posing a major threat to public health worldwide. Consequently, there is an urgent need for alternative therapeutic strategies. Bacteriophage (phage) therapy has re-emerged as a promising antimicrobial approach for combating drug-resistant bacterial infecti

Redox-Responsive Tellurium-Bridged Covalent Organic Frameworks/PEG Composites for Targeted Therapy of Diabetic Cardiomyopathy.

Diabetic cardiomyopathy, a major complication of diabetes, is strongly associated with elevated levels of glycated hemoglobin (HbA1c) and reactive oxygen species (ROS). However, effective clinical strategies to simultaneously lower HbA1c and ROS levels remain elusive, primarily due to the lack of therapeutic agents that can efficiently and synergistically interact with both biological macromolecules and small reactive molecules. To tackle this challenge, a redox-responsive tellurium-bridged covalent organic framework (Te-COF) is developed whose surface is functionalized with hydrazine-bonded p

Transformative potential of plant-based nanoparticles in cancer diagnosis and treatment: bridging traditional medicine and modern therapy.

Cancer is a primary global health concern, with an estimated 35.3 million cancer cases expected worldwide, representing a 76.6% increase in 2022, and 20 million by 2050, resulting from genetic mutation and environmental factors that cause uncontrolled cell growth. Other factors including smoking, unhealthy diets, physical inactivity, exposure to carcinogens, UV radiation, and aging increase DNA damage. Current cancer treatments like chemotherapy, radiation therapy, immunotherapy, and surgery are effective, but those have significant effects like lack of specificity, development of drug resista
