A metallic hemiarthroplasty resurfacing prosthesis for the hallux metatarsophalangeal joint.

Two-hundred seventy-nine arthritic hallux metatarsophalangeal joints treated surgically with a metallic resurfacing hemiarthroplasty over a 40-year period were reviewed. The implant, which is made available in three evenly graded sizes, is designed to replace only the articular surface of the proximal phalanx, with minimal resection of bone stock. The pathologic indications for surgery included classical hallux rigidus, rheumatoid arthritis, and degenerative changes associated with hallux valgus and bunion deformity. Follow-up at 8 months to 33 years after surgery revealed good or excellent cl

Revisional surgery for hallux valgus with serial osteotomies at two levels.

The aetiology and form of hallux valgus (HV) is varied with many corrective procedures described. We report a 39-year-old woman, previously treated with a Chevron osteotomy, who presented with recurrent right HV, metatarsus primus varus, and associated bunion. Osteotomies were performed at two levels as a revisional procedure. This report highlights (1) limitations of the Chevron osteotomy and (2) the revisional procedure of the two level osteotomies: (i) proximal opening-wedge basal osteotomy and (ii) distal short Scarf with medial closing wedges. If a Chevron osteotomy is used inappropriatel

Mitchell osteotomy for adolescent hallux valgus.

Sixty-nine Mitchell osteotomies augmented with smooth-pin fixation and a trapezoidal step-off osteotomy to maintain ray length were performed on 46 adolescent patients with a painful hallux valgus deformity. Average follow-up time was 6 years. By using radiographic, clinical, and subjective patient data, we found an overall excellent or good result in 91% of cases. Nine feet were found to be cosmetically unsatisfactory, 11 required special shoewear, 10 had mild residual pain with high heels or strenuous running, and three had pain severe enough to restrict activities. Range of motion was norma

Early results of the modified Simmonds-Menelaus procedure for adolescent hallux valgus.

Adolescent hallux valgus is a common problem, and there is no agreement about the best surgical technique to use to correct this deformity. Excellent results have been reported with a distal soft tissue procedure and an associated osteotomy at the base of the first metatarsal. The current study reports the early results of using an incomplete osteotomy at the base of the first metatarsal. No hardware was used to fix the osteotomy, and the postoperative immobilization was shorter. The study included 20 feet in 11 female patients. All osteotomies healed primarily without complications. The avera

Foot and ankle problems in dancers.

Lower extremity problems, specifically in the feet and ankles, are common in dancers. This is not surprising considering the repetitive and acute stresses applied to lower limbs during this rigorous performing arts activity. When evaluating a dancer with lower extremity complaints, the nature of these demands must be appreciated. In addition to routine physical examination, static and dynamic biomechanical evaluation is paramount to analysis and treatment of leg, foot, and ankle conditions. Although a particular problem, such as painful bunion, may seem localized, it is often related to other

The Learning Curve of Third-Generation Percutaneous Chevron and Akin Osteotomy (PECA) for Hallux Valgus.

The learning curve to reach technical proficiency for third-generation percutaneous or minimally invasive chevron and Akin osteotomies (PECA/MICA) is recognized to be steep however it is poorly defined in the literature. This study is a retrospective review of the first 58 consecutive PECA cases of a single surgeon. The primary outcome was the number of cases required to reach technical proficiency as defined by the operation time. Secondary outcomes included radiation exposure, radiographic deformity correction, and complication rates. Between November 2017 and March 2019, 61 consecutive PECA

Distributional patterns of subchondral bone density and histopathological features of the first tarsometatarsal joint in hallux valgus feet.

BACKGROUND: Hypermobility of the first tarsometatarsal (TMT) joint is frequently identified in patients with hallux valgus (HV); however, its association with the development of osteoarthritis in the first TMT joint in such patients remains unknown. The purpose of this study was to clarify the distribution of subchondral bone density of the first TMT joint via computed tomography (CT) using Hounsfield units (HU). METHODS: Patients were divided into three groups: the osteotomy (20 feet; 20 women, mean age: 61.8 years), arthrodesis (23 feet; two men, 21 women, 71.2 years), and control group (pa

Relationship between foot morphology, muscle strength, and physical performance test in women aged 65 years and older: a cross-sectional study.

BACKGROUND: Hallux valgus is a foot deformity that may affect gait, thus increasing the risk of falls among older people. We investigated the relationship between foot morphology, muscle strength, and physical performance. METHODS: In this study, community-dwelling older people aged ≥65 years were included. A three-dimensional footprint automatic measurement apparatus was used to measure the hallux valgus angle, arch height ratio, and heel-floor angle. Furthermore, the toe flexor strength and ankle plantar flexion strength were measured. Physical performance tests included the five-repetition

Pronation on weightbearing radiographs does not correlate with pronation from weightbearing CT scans.

INTRODUCTION: In hallux valgus (HV), first metatarsal pronation is increasingly recognized as an important aspect of the deformity. The purpose of this study was to compare pronation in HV patients determined from the shape of the lateral head of the first metatarsal on AP weightbearing radiographs with pronation calculated from weightbearing CT (WBCT) scans. METHODS: Patients were included in this study if they had preoperative and 5-month postoperative WBCT scans and corresponding weightbearing AP radiographs of the affected foot. Pronation of the first metatarsal on WBCT scans was measured

The first metatarsophalangeal joint meniscus and its relation to hallux valgus deformity--an anatomical and clinical study.

BACKGROUND: This study describes the anatomy and incidence of the metatarsophalangeal (MTP) joint meniscus, a structure not mentioned before in literature. METHODS: An anatomical cadaver study on 102 feet was performed with special attention to the first MTP joint anatomy. These results were compared with the per-operative findings in a clinical prospective study on 100 consecutive hallux valgus surgeries. RESULTS: On cadavers this meniscus is more common in patients with hallux valgus. Clinically, in patients with a mild hallux valgus the meniscus is found in more than half of cases during

Arthrodesis of the first metatarsocuneiform joint: a comparative study of fixation methods.

BACKGROUND: The Lapidus bunionectomy is a popular procedure for severe bunion deformity where metatarsus primus varus is equal to or exceeds 15 degrees. We evaluated a new locking compression plate which may improve outcomes with the Lapidus procedure. METHODS: Ten matched pairs of cadaver feet were used to compare the standard crossed 4.0-mm compression screw method of fixation to the LPS Lapidus plate. After performing the matched operations the cadaver constructs were stressed to failure using the INSTRON and Wavemaker software. RESULTS: The LPS Lapidus plate load to failure was 108 Nm wi

Idiopathic adult hallux varus.

Adult hallux varus is an uncommon clinical entity usually caused by an inflammatory arthropathy or overcorrection during bunion reconstruction. We present five cases of unexplained spontaneous hallux varus. Clinically, all patients initially were found to have flexible deformities, and no evidence of underlying inflammatory disease or history of trauma. Symptoms were easily relieved with shoe wear modifications. Two of the patients developed more rigid deformities and subsequent medial callusing of the first toe and varus deformities of the lesser toes. These two patients eventually required s

Correction of juvenile hallux valgus deformity associated with metatarsus primus adductus using epiphysiodesis technique.

A 4.5-year follow-up retrospective study on the use of epiphysiodesis procedure for juvenile hallux valgus deformity and metatarsus primus adductus deformity shows a good reduction of deformity in nine patients with minimal change in one patient that can be explained due to the timing of the procedure. This is a minor operation for juvenile bunion deformities, using epiphyseal arrest techniques. The only complication to this procedure was one case with a noted metatarsus primus elevatus due to incomplete epiphyseal arrest from dorsal to plantar. This procedure has been found to be a safe and e

[Deformities of the forefoot in patients with rheumatoid arthritis--results of surgical treatment].

PURPOSE OF THE STUDY: Rheumatoid arthritis affects the foot very frequently. The transversal arch of the foot gradually declines due to the inflammation, the metatarsal heads protrude in the sole of the foot which is accompanied by painful bunions and sometimes also skin necrosis. The great toe turns into a valgus position and pronation. Walking is very painful. MATERIAL: The authors evaluate a group of patients operated on for the deformities of the forefoot between 1995 and 2002. METHODS: In the deformities of the forefoot the authors use resection of the heads of II-V metatarsals. They us

Silicone implant arthroplasty of the great toe. A review of single stem and flexible hinge implants.

One hundred sixty-five feet were reconstructed with the single stem silicone implant, including patients with hallux regidus, hallux valgus due to degenerative and osteoarthritic joint changes, hallux valgus secondary to rheumatoid arthritis and patients with previous unsuccessful "bunion" procedures. Preoperative pain and intolerance to shoe wear were relieved in the great majority of patients. The amount of correction of the hallux valgus angle was directly related to the preoperative deformity. Infection occurred in 2 patients, and inflammatory reaction to the implant occurred in one patien
