Global trends in early-onset liver cancer incidence and projections to 2050 based on the global burden of disease study 2021.

BACKGROUND: Early-onset liver cancer (EOLC) affects individuals aged 15–49 and poses a major global health challenge. This study evaluates health inequalities in EOLC burden across regions and sexes to support effective, targeted interventions METHODS: Using Global Burden of Disease (GBD) 2021 data, we analyzed global EOLC incidence, mortality, and disability-adjusted life years (DALYs) from 1990 to 2021. Analyses included Joinpoint regression, decomposition, locally estimated scatterplot smoothing (LOESS), and frontier efficiency assessment. Trends to 2050 were projected via a gender-stratifi

Maternal Serum and Cord Blood Leptin Concentrations in African Newborns: Relationship to Birth Weight and Gender.

BACKGROUND: Leptin, a protein predominantly produced by adipocytes, plays a crucial role in regulating energy balance, inflammation, immunity, and fetal growth. During pregnancy, maternal serum leptin levels increase, peaking in the second trimester, with placental production contributing to this rise. Leptin has also been identified in fetal tissues, and its concentration in umbilical cord blood correlates with birth weight. This study aimed to evaluate serum and umbilical cord blood leptin concentrations in rural Burundian women living in marginal nutritional conditions, and investigate pote

Estimating the economic impact of COVID-19 disruption on access to sexual and reproductive health and rights in Eastern and Southern Africa.

BACKGROUND: The Coronavirus disease 2019 (COVID-19) resulted in the disruption of Sexual and Reproductive Health Rights (SRHR) services in the Eastern and Southern Africa region. To date, studies estimating the impact of COVID-19 disruptions have mainly focused on SRHR services without estimating the economic implication. METHOD: We used national service coverage data on the effectiveness of interventions from the lives saved tool (LiST), a mathematical modeling tool that estimates the effects of service coverage change in mortality. We computed years lost due to COVID-19 disruption on SRHR u

A Landscape Analysis of Prevention of Vertical Transmission Program Data and Interventions From Fiscal Years 2019-2021.

INTRODUCTION: In 2020, an estimated 150,000 infants acquired HIV infection through vertical transmission. With pregnant and breastfeeding women facing numerous social and health system barriers, continuity of care for mother-infant pairs (MIPs) requires prioritized engagement for timely infant HIV testing and linkage to treatment. METHODS: PEPFAR Monitoring, Evaluation, and Reporting indicators were analyzed from across 14 USAID-supported countries across 3 fiscal years (FYs) (October 2018-September 2021): number of HIV-exposed infants (HEIs) with a sample collected for an HIV test by age 2 m

Cesarean section rates and indications in sub-Saharan Africa: a multi-country study from Medecins sans Frontieres.

OBJECTIVES: The World Health Organization considers Cesarean section rates of 5-15% to be the optimal range for targeted provision of this life saving intervention. However, access to safe Cesarean section in resource-limited settings is much lower, estimated at 1-2% reported in sub-Saharan Africa. This study reports Cesarean sections rates and indications in Democratic Republic of Congo, Burundi, and Sierra Leone, and describe the main parameters associated with maternal and early neonatal mortality. METHODS: Women undergoing Cesarean section from August 1 2010 to January 31 2011 were includ

The network structure of posttraumatic stress symptoms in war-affected children and adolescents.

BACKGROUND: It is unclear whether findings from previous network analyses of posttraumatic stress disorder (PTSD) symptoms among children and adolescents are generalizable to youth living in war-torn settings and whether there are differences in the structure and connectivity of symptoms between children and adolescents. This study examined the network structure of PTSD symptoms in a sample of war-affected youth and compared the symptom networks of children and adolescents. METHODS: The overall sample comprised 2007 youth (6-18 years old) living in Burundi, Democratic Republic of Congo, Iraq,

[Evaluation of the surgical treatment of duodenal ulcers. Apropos of 616 patients operated on in Bujumbura].

From 1982 to 1989, 616 patients were operated for duodenal ulcers in Bujumbura. 497 were men and 119 females. The mean age was 38.4. The indication for surgery was the failure of medical treatment for 56.8%. Other complications presented 41.4% and 1.8% of the patients had recurring ulcer after previous operation. The study was divided into two periods: Period I (1982-1985) and period II (1986-1989). During Period I, we observed a rapid drop of frequency in interventions and the indication for complication constituted 58% of the total surgical interventions. In period II, the frequency of inter

Modelling age-heterogeneous Schistosoma haematobium and S. mansoni survey data via alignment factors.

BACKGROUND: Reliable maps of the geographical distribution, number of infected individuals and burden estimates of schistosomiasis are essential tools to plan, monitor and evaluate control programmes. Large-scale disease mapping and prediction efforts rely on compiled historical survey data obtained from the peer-reviewed literature and unpublished reports. Schistosomiasis surveys usually focus on school-aged children, whereas some surveys include entire communities. However, data are often reported for non-standard age groups or entire study populations. Existing geostatistical models ignore

Evidence of multiple introductions of HIV-1 subtype C in Angola.

HIV-1 subtype C is the most prevalent group M clade in southern Africa and some eastern African countries. Subtype C is also the most frequent subtype in Angola (southwestern Africa), with an estimated prevalence of 10-20%. In order to better understand the origin of the HIV-1 subtype C strains circulating in Angola, 31 subtype C pol sequences of Angolan origin were compared with 1950 subtype C pol sequences sampled in other African countries. Phylogenetic analyses reveal that the Angolan subtype C sequences were distributed in 16 different lineages that were widely dispersed among other Afric

Spatially explicit Schistosoma infection risk in eastern Africa using Bayesian geostatistical modelling.

Schistosomiasis remains one of the most prevalent parasitic diseases in the tropics and subtropics, but current statistics are outdated due to demographic and ecological transformations and ongoing control efforts. Reliable risk estimates are important to plan and evaluate interventions in a spatially explicit and cost-effective manner. We analysed a large ensemble of georeferenced survey data derived from an open-access neglected tropical diseases database to create smooth empirical prevalence maps for Schistosoma mansoni and Schistosoma haematobium for a total of 13 countries of eastern Afri

Banana Bunchy Top Disease in Rwanda: Identifying Disease Presence and Potential Areas at Risk.

Banana bunchy top disease (BBTD) has been reported in southwestern Rwanda since the 1980s. Recent sporadic observations of BBTD in western and central regions, made by extension services and the Rwanda Agriculture and Animal Resources Development Board, merited a comprehensive field survey across the country. This survey was performed in 2022 and 2023 to confirm and map the presence of BBTD in suspected areas in Rwanda. Combining the Rwandan survey with BBTD surveys in neighboring countries (eastern DR Congo, Burundi, and Uganda), a BBTD probability model was developed for Rwanda, taking into

Measuring under-5 mortality and fertility through mobile phone surveys: an assessment of selection bias in 34 low-income and middle-income countries.

OBJECTIVES: This study aims to assess sample selection bias in mobile phone survey estimates of fertility and under-5 mortality. DESIGN: With data from the Demographic and Health Surveys, we use logistic regressions to identify sociodemographic correlates of mobile phone ownership and access, and Poisson regressions to estimate the association between mobile phone ownership (or access) and fertility and under-5 mortality estimates. We evaluate the potential reasons why estimates by mobile phone ownership differ using a set of behavioural characteristics. SETTING: 34 low-income and middle-inc

Impact of the COVID-19 pandemic on the food rations of refugees in Rwanda.

The coronavirus disease (COVID-19) has significantly impacted the global economy, by forcing people to stay indoors and creating a 'new normal' of living. Rwanda has made notable efforts to fight the pandemic. However, the impacts of the COVID-19 pandemic on the country's economy are numerous and the refugees residing in Rwanda are not spared these effects. As of December 2020, 164,000 people were granted refugee status in Rwanda according to the United Nations High Commissioner for Refugees (UNHCR). The majority were from neighbouring countries in the Great Lakes regions, including DRC (Democ

Adherence to Iron Supplementation in 22 Sub-Saharan African Countries and Associated Factors among Pregnant Women: A Large Population-Based Study.

BACKGROUND: Iron deficiency anemia during pregnancy is a significant public health problem in sub-Saharan Africa (SSA) and is associated with serious adverse health outcomes. Although it is recommended that all women receive iron supplementation during pregnancy, little research has been conducted to measure overall compliance with this recommendation or variation across SSA countries. OBJECTIVES: To assess prevalence and sociodemographic-economic factors associated with adherence to iron supplementation among pregnant women in SSA. METHODS: This was a weighted population-based cross-section

Cesarean section surgical site infections in sub-Saharan Africa: a multi-country study from Medecins Sans Frontieres.

BACKGROUND: Surgical site infections (SSI) are a significant cause of post-surgical morbidity and mortality and can be an indicator of surgical quality. The objectives of this study were to measure post-operative SSI after cesarean section (CS) at four sites in three sub-Saharan African countries and to describe the associated risk factors in order to improved quality of care in low and middle income surgical programs. METHODS: This study included data from four emergency obstetric programs supported by Medecins sans Frontieres, from Burundi, the Democratic Republic of Congo (DRC), and Sierra

Experiences of gender-based violence among women in sub-Saharan Africa: identifying evidence for intervention and public health priorities.

BACKGROUND: Gender-based violence (GBV) poses a significant global threat to human rights, public health and attaining the Sustainable Development Goals. There is a growing emphasis on researching to identify issues and understand the experiences of women relative to GBV. The increasing demand for policymakers and public health practitioners to adopt evidence-based approaches in addressing GBV highlights the need for research prioritization on experiences of GBV among women in resource-limited settings such as sub-Saharan Africa. This paper explored GBV among women in 25 sub-Saharan African (S

Predicting the effects of climate change on Schistosoma mansoni transmission in eastern Africa.

BACKGROUND: Survival and fitness attributes of free-living and sporocyst schistosome life-stages and their intermediate host snails are sensitive to water temperature. Climate change may alter the geographical distribution of schistosomiasis by affecting the suitability of freshwater bodies for hosting parasite and snail populations. METHODS: We have developed an agent-based model of the temperature-sensitive stages of the Schistosoma mansoni and intermediate host snail lifecycles. The model was run using low, moderate and high warming climate projections over eastern Africa. For each climate

Could there have been substantial declines in sexual risk behavior across sub-Saharan Africa in the mid-1990s?

BACKGROUND: HIV prevalence is decreasing in much of sub-Saharan Africa (SSA), but the drivers of the decline are subject to much dispute. Using mathematical modeling as a tool for hypothesis generation, we demonstrate how the hypothesis that the drop in prevalence reflects declines in sexual risk behavior is self-consistent. We characterize these potential declines in terms of their scale, duration, and timing, and theorize on how small changes in sexual behavior at the individual-level could have driven large declines in HIV prevalence. MATERIALS AND METHODS: A population-level deterministic
