Osteopenia in rheumatology practice: pathogenesis and therapy.

Postmenopausal osteoporosis has a multifactorial pathogenesis related to decreases in bone mass, calcium intake, and circulating estrogen levels. Therapy with supplemental calcium, estrogen, fluoride, anabolic steroids, and calcitonin is discussed. Corticosteroid-induced osteopenia is in part related to a decrease in intestinal calcium absorption and therapy with supplemental vitamin D and calcium may favorably alter the outcome. The osteopenia of rheumatoid arthritis appears to be a diffuse process, with increased metabolic bone activity at sites that are remote from the areas of active synov

Comparison of two village primary schools in northern Tanzania affected by fluorosis.

High fluoride levels in drinking water sources are a problem throughout the East African Rift Valley and can lead to dental fluorosis (DF) and skeletal fluorosis (SF) in exposed local populations. Two villages in the Hai District of northern Tanzania in which fluoride has been identified as a problem were investigated in a pilot study. Fluoride levels in drinking water sources were measured and the prevalence of DF and deformities due to SF were assessed in children attending school in the two villages. The assessment also recorded the source of drinking water as well as children's height, wei

Multimodal imaging offers new insights into understanding ion penetration in dentin caries using an animal model.

OBJECTIVE: Dentin being an organic-inorganic composite, understanding how inorganic ions behave and bring about changes in the organic matrix is essential to clarify the pathogenesis of dentin caries. We developed a novel in vivo carious restoration model combining quantitative analysis and multimodal imaging to elucidate the pathology of dental caries and to investigate how multi-ions released from restorative materials promote dentin demineralization. METHODS: Carious lesions in maxillary first molars of Sprague-Dawley rats were restored using a glass ionomer cement. We quantified the eleme

In vitro formation of ethanol in autopsy samples containing fluoride ions.

We present a case of a death of a diabetic man where the concentration of ethanol in post-mortem blood rose from 0.4 g/l 2 days after autopsy to 3.5 g/l 10 days after autopsy. The presence of fluoride ions in this blood sample was determined with ion chromatography and verified that fluoride ions were added to the vials. The concentrations of free fluoride, corresponding to 0.21 and 0.25% w/v potassium fluoride in blood and urine, respectively, were somewhat lower than the recommended 1% w/v. However, the amount of fluoride ions bound to calcium, proteins and other compounds in the samples is

Effect of fluoride-releasing filling materials on underlying dentinal lesions in vitro.

Fluoride-releasing materials placed over carious tissue are assumed to enhance remineralisation of the underlying lesion. This remineralisation, however, also depends on the availability of calcium and phosphate, which may be supplied by the pulpal fluid. The aim of this study was to measure the fluoride release of glass ionomer cements (GICs) into underlying dentin and to measure the effect of the released fluoride on the remineralisation of the underlying dentinal lesions using transversal microradiography. Discs of fluoride-releasing GIC were placed on top of dentinal lesions in an in vitro

[Postmenopausal osteoporosis: are there alternatives to estrogen/gestagen substitution?].

This review on non-estrogen measures against postmenopausal osteoporosis first deals with primary prevention in young women, such as calcium rich nutrition and physical training in sedentary young women. For the case of acute osteoporotic vertebral fracture, analgetic, physiotherapeutic and ergonomic measures are highlighted. A more profound discussion covers those possible measures against established osteoporosis, which have been evaluated in prospective controlled trials: calcium, Vitamin-D, fluorides, bisphosphonates, calcitonin, anabolic steroids and exercise. [Physicochemical quality of drinking water in Southern Algeria: study of excess mineral salts].

The aim of this study was to determine the physicochemical composition of water intended for human consumption in several regions of Southern Algeria. Excess minerals in drinking water, including magnesium, calcium, sulfates and fluorides play a fundamental role in the prevention of urinary calculi, which are formed mainly from calcium oxalate. The ever-increasingly prevalence of this disorder and its recurrence make it a real public health problem in Algeria. The most elementary preventive treatment, recommended to all subjects with lithiasis, is to drink 2 to 3 L water distributed throughout
