Treatment of Medication-Related Osteonecrosis of the Jaw: Controversies in Causality and Therapy.

Medication-related osteonecrosis of the jaw (MRONJ) is a most interesting, complex and "elusive" condition seen by the oral health care provider. It is plagued by controversy and although a wealth of research has created clinical treatment databases, there is no "gold standard" algorithm to be applied in a universal fashion. The purpose of this article is to explore several controversies associated with the etiology(s), staging, treatments, and long-term resolution of MRONJ in patients who are treated by the oral health care provider. Controversies for optimizing prevention, and disease contro

The hypothalamus may mediate migraine and ictal photophobia: evidence from Granger causality analysis.

BACKGROUND: The hypothalamus plays a central role in the pathophysiology of migraine and is considered to be the "migraine generator." It participates in initiating a migraine attack through its connectivity to regions of the brain involved in processing and modulating pain. However, the underlying mechanisms of hypothalamic effective functional connectivity that bring about migraines remain unclear. This study investigated the hypothalamus-based directional effective connectivity in migraine without aura patients and assessed associations among the clinical characteristics. METHODS: Seven pa

Tamsulosin and risk of priapism: A causality assessment using Austin Bradford Hill Criteria.

Tamsulosin hydrochloride, a selective alpha-adrenergic blocking agent has been previously associated with priapism. Priapism is a medically serious condition that, if not intervened, can cause permanent erectile dysfunction. This study was conducted to investigate whether the association of tamsulosin and priapism is causal. All currently available evidence such as experimental, biological, toxicological, published studies, and safety data mined from the WHO global pharmacovigilance database was systematically organized into the Austin Bradford Hill causality assessment framework. Mendelian randomization to assess causality between uromodulin, blood pressure and chronic kidney disease.

UMOD variants associated with higher levels of urinary uromodulin (uUMOD) increase the risk of chronic kidney disease (CKD) and hypertension. However, uUMOD levels also reflect functional kidney tubular mass in observational studies, questioning the causal link between uromodulin production and kidney damage. We used Mendelian randomization to clarify causality between uUMOD levels, kidney function and blood pressure in individuals of European descent. The link between uUMOD and estimated glomerular filtration rate (eGFR) was first investigated in a population-based cohort of 3851 individuals.

A case of giant retroperitoneal lymphangioma and IgG4-positive fibrosis: Causality or coincidence?

Several chronic inflammatory diseases have been found to be a subtype of IgG4-related disease, all of which have a typical clinical and histological change, which is based in particular on an overexpression of IgG4 and subsequent fibrosis. At least a part of the retroperitoneal fibrosis, which was originally classified as idiopathic, seems to be assigned to IgG4-related disease. Lymphangiomas are benign, cystic tumors that rarely occur in adults. However, there is no firm association with IgG4-related disease described in the literature to date. This report is about a patient suffering from ac

Investigating causality in the association between vitamin D status and self-reported tiredness.

Self-reported tiredness or low energy, often referred to as fatigue, has been linked to low levels of circulating 25-hydroxyvitamin D (25OHD), a biomarker of vitamin D status. Although it is uncertain if the association is causal, fatigue is a common indication for testing, and correcting, low 25OHD-levels. We used two-sample Mendelian randomization to test for genetic evidence of a causal association between low 25OHD-levels and fatigue. Genetic-25OHD associations were estimated from the largest genome-wide association study of vitamin D to date, and genetic-fatigue associations were estimate

Understanding Nasal Emission During Speech Production: A Review of Types, Terminology, and Causality.

There are several different types of nasal emission that can occur during speech due to either velopharyngeal dysfunction or abnormal articulation in the pharynx. Nasal emission can be inaudible or very loud and distracting, depending on the size of the velopharyngeal opening and the physics of the flow. Nasal emission can be obligatory and/or compensatory (due to abnormal structure) or it can be caused by a misarticulation that results in a substitution of a pharyngeal sound for an oral sound, despite normal velopharyngeal structure. Nasal emission can occur on all pressure-sensitive phonemes

Differentiating causality and correlation in allometric scaling: ant colony size drives metabolic hypometry.

Metabolic rates of individual animals and social insect colonies generally scale hypometrically, with mass-specific metabolic rates decreasing with increasing size. Although this allometry has wide ranging effects on social behaviour, ecology and evolution, its causes remain controversial. Because it is difficult to experimentally manipulate body size of organisms, most studies of metabolic scaling depend on correlative data, limiting their ability to determine causation. To overcome this limitation, we experimentally reduced the size of harvester ant colonies (Pogonomyrmex californicus) and q

Causality of small and large intestinal microbiota in weight regulation and insulin resistance.

OBJECTIVE: The twin pandemics of obesity and Type 2 diabetes (T2D) are a global challenge for health care systems. Changes in the environment, behavior, diet, and lifestyle during the last decades are considered the major causes. A Western diet, which is rich in saturated fat and simple sugars, may lead to changes in gut microbial composition and physiology, which have recently been linked to the development of metabolic diseases. METHODS: We will discuss evidence that demonstrates the influence of the small and large intestinal microbiota on weight regulation and the development of insulin r

Causality analysis of the relationship between bleeding and lethality in Acute Coronary Syndromes.

BACKGROUND: Hemorrhagic events in Acute Coronary Syndromes (ACS) have been independently associated with death in international multicenter registries. However, that association has not been tested in Brazil and the true causal relationship between bleeding and death has not been completely shown. OBJECTIVE: To test the following hypotheses: (1) major bleeding is an independent predictor of in-hospital death in ACS; (2) the relationship between those two endpoints is causal. METHODS: This study included patients meeting predefined criteria of unstable angina, non-ST-segment elevation myocard

Systemic sclerosis and superficial siderosis of the central nervous system: casuality or causality?

We describe the case of a 59 year old man with systemic sclerosis and superficial siderosis of the central nervous system characterized by progressive worsening of walking with signs of pyramidal liberation associated with cerebellar ataxia and bilateral hypoacusia. Ferritin and oligoclonal bands were found on lumbar puncture and Magnetic Resonance Imaging of the brain and spinal cord revealed a rim of hypointensity in T2-weighted images enveloping the surface of brain and spinal cord, typical of hemosiderosis. Neurological complications, both central and peripheral, are thought to be rare and

Fractal meta-analysis and 'causality' embedded in complexity: advanced understanding of disease etiology.

Epidemiologists have gradually come to realize that repeated testing of 'causal' hypotheses normally results in inconsistent outcomes. Contradictory results pop up often enough to arouse suspicion that central epidemiological paradigms may be invalid. In this paper, we introduce complexity to explain inconsistency. Linear models produce inconsistency simply because they are not sufficiently rooted in the complex nature of living things. We design a meta-analysis consisting of (a) fractal investigation, used in this paper to reveal the possibly fractal nature of ordered series of relative risks

Political identification and perceptions of homelessness: attributed causality and attitudes on public policy.

The study investigated relationships between political orientation, causal perceptions of poverty, and attitudes toward government programs for the poor. The test sample of 200 women and 200 men were recruited from introductory psychology classes. In support of hypotheses based on previous research and Weiner's attribution-emotion-action theory, when compared with self-identified Democrats, self-identified Republicans (a) were significantly more inclined to attribute homelessness to internal vs external factors and (b) expressed significantly less favorable attitudes toward publically funded p

Causal K-Means Clustering.

Causal effects are often characterized with population summaries. These might provide an incomplete picture when there are heterogeneous treatment effects across subgroups. Since the subgroup structure is typically unknown, it is more challenging to identify and evaluate subgroup effects than population effects. We propose a new solution to this problem: Causal k-Means Clustering, which leverages the k-means clustering algorithm to uncover the unknown subgroup structure. Our problem differs significantly from the conventional clustering setup since the variables to be clustered are unknown cou

Causal Inference in Audiovisual Perception.

In our natural environment the senses are continuously flooded with a myriad of signals. To form a coherent representation of the world, the brain needs to integrate sensory signals arising from a common cause and segregate signals coming from separate causes. An unresolved question is how the brain solves this binding or causal inference problem and determines the causal structure of the sensory signals. In this functional magnetic resonance imaging (fMRI) study human observers (female and male) were presented with synchronous auditory and visual signals at the same location (i.e., common cau

Causal Inference in the Multisensory Brain.

When combining information across different senses, humans need to flexibly select cues of a common origin while avoiding distraction from irrelevant inputs. The brain could solve this challenge using a hierarchical principle by deriving rapidly a fused sensory estimate for computational expediency and, later and if required, filtering out irrelevant signals based on the inferred sensory cause(s). Analyzing time- and source-resolved human magnetoencephalographic data, we unveil a systematic spatiotemporal cascade of the relevant computations, starting with early segregated unisensory represent
