Whole-genome sequencing in a pair of monozygotic twins with discordant cleft lip and palate subtypes.

OBJECTIVE: Orofacial clefts (OFCs) are common and etiologically complex birth defects. This study explored potential genetic differences in a pair of Japanese monozygotic (MZ) twins with different forms of OFC using whole-genome sequencing. SUBJECTS AND METHODS: One co-twin (MZ-1) presented with nonsyndromic bilateral cleft lip and palate; the other co-twin (MZ-2) had nonsyndromic bilateral cleft lip and unilateral left-sided cleft alveolus. Neither parent had an OFC. Craniofacial morphologic features and potential genetic differences were compared using standard cephalometry and whole-genome

[Surgical correction of craniofacial dysostosis with midface distraction osteotogenesis].

OBJECTIVE: Correction of craniofacial dysostosis with midface distraction osteotogenesis. METHODS: Le Fort III osteotomy has been employed through coronal route on patients with midface craniofacial dysostosis such as Crouzon and Apert syndrome. Then a REDII distraction device was set up, and the device bars directed. The distraction begins 6.4 days after the surgery, with a rate of 1 mm per day. When midface approaching the right position, i.e. an slightly over correction of occlusion is reached, the distraction stops and the device is held for the next 2-4 months. RESULTS: There are 8 case

Prevalence of sleep-related breathing disorders in children with malocclusion.

BACKGROUND: The paediatric population has a high incidence of sleep-related breathing disorders (SRBD). A notable risk factor is the presence of craniofacial abnormalities. The objective of the study was therefore to survey the prevalence of SRBD in patients presenting for interceptive treatment. MATERIAL AND METHODS: Prospective study with a sample of 249 healthy patients. The "Paediatric Sleep Questionnaire" and "Sleep Disturbance Scale for Children" were completed by the children's parents and the results were evaluated. All patients had their medical records reviewed and underwent orthodo

Dual midfacial distraction osteogenesis for Crouzon syndrome: long-term follow-up study for relapse and growth.

PURPOSE: Rigid external distraction osteogenesis is regarded as a standard treatment for congenital midfacial hypoplasia. However, external distraction for the upper portion of the midface is not as effective and tends to rotate the midfacial segment in a counterclockwise direction. Moreover, patients poorly tolerate it because of the device's bulkiness. To prevent such drawbacks of an external distractor, both external and internal distractors were synchronously applied to patients with Crouzon syndrome. PATIENTS AND METHODS: In 6 patients with Crouzon syndrome in whom a dual-distraction tec

Adult skeletal profile in isolated cleft palate: a comparison of the von Langenbeck and Wardill procedures for primary repair of the palate.

Sixty-four adult patients operated on for isolated cleft palate were evaluated with regard to facial skeletal morphology using conventional radio-cephalometry. Dental occlusion was assessed clinically. Forty-two had had a von Langenbeck repair at the age of 7 months and 22 a Wardill repair at 18 months. The mean error of the method was 0.7 degree for angular, and 0.9 mm for linear, measurements. The group with clefts had less maxillary prognathism (s-n-ss), more maxillary inclination (NSL/NL), more retroclined lower incisors (ILI/ML), and shorter total and upper facial heights (n-gn, n-sp) com

The effect of posture on upper airway dimensions in normal subjects and in patients with the sleep apnea/hypopnea syndrome.

The effect of posture on upper airway dimensions was assessed for two reasons. First, some patients with untreated sleep apnea/hypopnea syndrome (SAHS) report they sleep better sitting upright. Second, to allow comparison of the differing techniques used to determine the site of maximal airway narrowing in awake patients with SAHS, as some are carried out in the erect and others in the supine posture. Lateral cephalometry was therefore carried out in 33 nonsnoring normal subjects and in 29 patients with obstructive SAHS (mean apneas plus hypopneas, 46 per hour; range, 17 to 103). In both norma

A comparative study of facial growth following cleft lip repair with or without soft-tissue undermining: an experimental study in rabbits.

The present study was performed in rabbits to test the hypothesis that incisions in the sulcus and soft-tissue undermining at the time of cleft lip repair contribute more to midfacial growth inhibition and aberrations than lip repair only. The design of this study, using three control groups, permitted assessment of the effects of soft-tissue undermining independently of the other surgical procedures. This study revealed that lip repair combined without soft-tissue undermining. Direct cephalometry on the skulls indicated that anteroposterior facial growth was most severely affected mean quanti
