Three cases of middle cerebral artery occlusion emergently revascularized with a balloon-expandable coronary bare stent after intravenous tissue plasminogen activator.

BACKGROUND: Revascularization with emergency stent placement in patients with acute middle cerebral artery occlusion is still controversial in Japan. METHODS: We placed balloon-expandable coronary bare stents in 3 patients in whom revascularization was not obtained after intravenous tissue plasminogen activator therapy (IV t-PA) for acute ischemic stroke (middle cerebral artery M1 occlusion). RESULTS: Patient 1 was an 87-year-old woman with left hemiplegia. Her National Institutes of Health Stroke Scale score (NIHSS) was 12, and her magnetic resonance imaging diffusion-weighted image Alberta

A Comparative Analysis of Short-Term Outcomes of Off Pump Coronary Artery Bypass Grafting Alone or Simultaneous Cerebral Revascularization: A Single-Center Retrospective Study.

OBJECTIVES: The impact of simultaneous coronary artery bypass grafting (CABG) with cerebral revascularization versus CABG alone on perioperative complications and short-term outcomes remains controversial in patients with simultaneous coronary and cerebrovascular disease. Our study was specifically designed to evaluate the effects of simultaneous cardio-cerebrovascular revascularization versus isolated off-pump CABG (OPCABG) in patients with severe carotid stenosis. METHODS: This retrospective analysis included patients undergoing OPCABG between May 2023 and May 2025. Participants were divide

Tandem middle cerebral artery-internal carotid artery occlusions: reduced occlusion-to-revascularization time using a trans-anterior communicating artery approach with a penumbra device.

Rapid revascularization of tandem extracranial and intracranial acute thromboembolic occlusions can be challenging and can delay restoration of blood flow to the cerebral circulation. Taking advantage of collateral pathways in the circle of Willis for thrombectomy can reduce the occlusion-to-revascularization time significantly, thereby protecting brain tissue from ischemic injury. The authors report using the trans-anterior communicating artery (ACoA) approach by using the Penumbra microcatheter to rapidly restore blood flow to the middle cerebral artery (MCA) territory prior to treating the

Pseudo-continuous and territorial arterial spin labeling MRI for assessment of cerebral perfusion in moyamoya disease after revascularization: A comparative study with digital subtraction angiography.

PURPOSE: To evaluate if pseudo-continuous arterial spin labeling (pcASL) and territorial ASL (tASL) can assess cerebral perfusion post-revascularization in Moyamoya disease and compare with digital subtraction angiography (DSA) outcomes. MATERIALS AND METHODS: Patients diagnosed with Moyamoya disease who underwent pcASL using two post-labeling delays (short ASL, 1,525 ms; delayed ASL, 2,525 ms), tASL, and DSA 3 months after surgery at a single institution were retrospectively evaluated. Manual delineation on pcASL cerebral blood flow (CBF) maps covered middle cerebral artery (MCA) territory o

Efficacy of prophylactic blood pressure lowering according to a standardized postoperative management protocol to prevent symptomatic cerebral hyperperfusion after direct revascularization surgery for moyamoya disease.

BACKGROUND: Cerebral hyperperfusion is a potential complication of superficial temporal artery-middle cerebral artery (STA-MCA) anastomosis for moyamoya disease, but the optimal postoperative management has not been determined. Aggressive blood pressure lowering is controversial because of the risk of ischemic complications. OBJECTIVE: To establish the optimal postoperative management protocol to prevent symptomatic cerebral hyperperfusion in moyamoya disease. METHODS: N-isopropyl-p-[(123)I]-iodoamphetamine single-photon emission computed tomography was performed 1 and 7 days after STA-MCA a

Mechanical revascularization of acute iatrogenic anterior cerebral artery occlusions: use of a new coaxial dual-lumen balloon catheter results in rapid access and flow restoration.

Acute iatrogenic occlusion of cerebral vessels is a risk during the performance of neuroendovascular procedures. Most commonly this is a result of thrombus formation within the vessel, thromboembolism or an acute vasospastic response. There are several options for flow restoration including pharmacological, mechanical and microsurgical. Mechanical flow restoration, usually by thrombectomy, is typically performed in large cerebral vessels. Current mechanical thrombectomy devices have limited utility in smaller vessels that are more difficult to access, such as the anterior cerebral artery (ACA)

Emboli formation rather than inflammatory mediators are responsible for increased cerebral water content after conventional and assisted beating-heart myocardial revascularization in a porcine model.

BACKGROUND AND PURPOSE: Emboli and proinflammatory mediators are suspected of generating cerebral edema after coronary surgery. In contrast to cardiopulmonary bypass (CPB), off-pump coronary artery bypass surgery (OPCAB) reduces microemboli count and proinflammatory mediator release but carries the risk of hemodynamic instability. A microaxial blood pump can augment cardiac output. METHODS: Coronary bypasses were constructed in pigs with CPB and cardioplegia (n=9), OPCAB (n=9), or blood-pump support CAB (n=9). Nine animals underwent sham operation. Embolus count was monitored and regional cer

Intraoperative Cortical Indocyanine Green Extravasation as a Predictor of Cerebral Hyperperfusion following Direct Revascularization for Moyamoya Disease: Impact of Prolonged Observations of Indocyanine Green Videoangiography.

INTRODUCTION: Local vasogenic edema following direct revascularization for moyamoya disease (MMD) is considered to result from an intrinsic vulnerability of blood-brain barrier (BBB) and is consequently associated with transient focal cerebral hyperperfusion (CHP). However, intraoperative identification of the local vasogenic edema remains challenging. To address this, we implemented a prolonged observation of indocyanine green videoangiography (ICG-VAG) as an extension of routine clinical practice. This approach aimed to investigate intraoperative cortical ICG extravasation following direct r

Cerebral Hyperperfusion and Concomitant Reversible Lesion at the Splenium after Direct Revascularization Surgery for Adult Moyamoya Disease: Possible Involvement of MERS and Watershed Shift Phenomenon.

Superficial temporal artery (STA)-middle cerebral artery (MCA) bypass is the standard surgical treatment for moyamoya disease (MMD). Local cerebral hyperperfusion (CHP) is one of the potential complications, which could enhance intrinsic inflammation and oxidative stress in MMD patients and accompany concomitant watershed shift (WS) phenomenon, defined as the paradoxical decrease in the cerebral blood flow (CBF) near the site of CHP. However, CHP and simultaneous remote reversible lesion at the splenium have never been reported. A 22-year-old man with ischemic-onset MMD underwent left STA-MCA

The Potential Role of Preoperative Posterior Cerebral Artery Involvement in Predicting Postoperative Transient Neurological Deficits and Ischemic Stroke After Indirect Revascularization in Patients With Moyamoya Disease.

OBJECTIVE: Transient neurological deficits (TNDs) are known to develop after direct bypass for Moyamoya disease and may be risk factors for subsequent stroke. However, the factors involved in the development of TNDs and stroke after indirect revascularization alone, including their association with subsequent stroke, remain unclear. The purpose of this study was to investigate this issue. METHODS: The subjects of the study were 30 patients with Moyamoya disease who underwent a total of 40 indirect revascularization procedures at our institution. Clinical and radiological data were collected r

Long-term clinical and radiographic outcomes after pial pericranial dural revascularization: a hybrid surgical technique for treatment of anterior cerebral territory ischemia in pediatric moyamoya disease.

OBJECTIVE: Isolated anterior cerebral artery (ACA) territory ischemia in pediatric moyamoya disease (MMD) is rare but has been increasingly recognized, particularly in children manifesting progression of disease in a delayed fashion after middle cerebral artery revascularization surgery. Surgical treatment is complicated by limited graft choices, with the small number of case series largely focused on complex, higher-risk operations (omental flap transfers, large interhemispheric rotational grafts); direct bypass (often untenable in children due to vessel size); or, alternatively, the technica

A double-barrel superficial temporal artery-to-superior cerebellar artery (STA-SCA) and STA-to-posterior cerebral artery (STA-PCA) bypass for revascularization of the basilar apex.

Basilar apex aneurysms are challenging lesions for both microsurgical and endovascular therapy. In patients in whom direct clipping and coil embolization are not options, cerebral revascularization and occlusion of the basilar artery is a possible treatment strategy. We report a patient with a large basilar apex aneurysm treated by double-barrel anastomoses of the superficial temporal artery to the superior cerebellar artery (STA-SCA) and the STA to the posterior cerebral artery (STA-PCA) followed by occlusion of the basilar artery below the SCAs. A double-barrel bypass augments blood flow to

Establishment and external validation of prediction models for risk factors of cerebral hyperperfusion syndrome after combined revascularization in moyamoya disease based on meta-analysis and retrospective cohort studies.

OBJECTIVE: Cerebral hyperperfusion syndrome (CHS) is a complication affecting up to 50% of moyamoya disease (MMD) patients after combined revascularization. The aim of this study was to identify reliable CHS risk factors and develop predictive models. METHODS: The authors performed a meta-analysis including studies on both combined and direct revascularization to capture potential risk factors, followed by a sensitivity analysis on the combined bypass group. They also performed a retrospective cohort analysis. Then three models were developed using the derivation cohort and externally validat

Extra-anatomic revascularization and a new cannulation strategy for preoperative cerebral malperfusion due to severe stenosis or occlusion of supra-aortic branch vessels in acute type A aortic dissection.

OBJECTIVES: Acute type A aortic dissection (ATAAD) with severe stenosis or occlusion of the true lumen of aortic arch branch vessels often leads to an increased incidence of severe postsurgical neurological complications and mortality rate. In this study, we aimed to introduce our institutional extra-anatomic revascularization and cannulation strategy with improved postoperative outcomes for better management of patients with cerebral malperfusion in the setting of ATAAD. METHODS: Twenty-eight patients with ATAAD complicated by severe stenosis or occlusion of the aortic arch branch vessels, a

Characteristic Pattern of the Cerebral Hemodynamic Changes in the Acute Stage After Combined Revascularization Surgery for Adult Moyamoya Disease: N-isopropyl-p-[123I] iodoamphetamine Single-Photon Emission Computed Tomography Study.

OBJECTIVE: Surgical revascularization for moyamoya disease (MMD) prevents cerebral ischemic attack by improving cerebral blood flow (CBF) and could also reduce the risk of re-bleeding in hemorrhagic-onset patients. We sought to clarify the cerebral hemodynamic changes in the acute stage after revascularization surgery for adult MMD. MATERIALS AND METHODS: The present study includes 54 consecutive adult patients with MMD (21-76 years old, 43.1 average), undergoing superficial temporal artery-middle cerebral artery anastomosis with indirect pial synangiosis on 65 affected hemispheres.
