A case of Cheyne-Stokes breathing emerging in a patient with atrial fibrillation and an implanted hypoglossal nerve stimulator.

UNLABELLED: Treatment emergent central sleep apnea (TECSA) can occur with positive airway pressure (PAP) therapy, mandibular advancement devices, and now recent cases with hypoglossal nerve stimulator (HNS) therapy during treatment for obstructive sleep apnea (OSA). There have been few published reports of Cheyne-Stokes breathing (CSB) emerging after implantation of an HNS. We present a case of a 76-year-old male with chronic atrial fibrillation and OSA who developed significant CSB after implantation of an HNS device. As popularity increases for alternative treatments of OSA, there should be

Response of Home-Use Adaptive Pressure Modes to Simulated Transient Hypoventilation.

BACKGROUND: Adaptive servoventilation (ASV) is a recently developed ventilation mode designed to stabilize ventilation in patients with central sleep apnea and Cheyne-Stokes respiration. Alternatively, modes aiming to maintain average ventilation over several breaths, such as average volume-assured pressure support (AVAPS) and intelligent volume-assured pressure support (iVAPS), could be efficient during ventilation instability by reducing central events. These modes are available on a variety of devices. This bench evaluation studied the response of these different modes and devices to simula

Positive Airway Pressure Device Technology Past and Present: What's in the "Black Box"?

Since the introduction of continuous positive airway pressure (PAP) for the treatment of obstructive sleep apnea (OSA) in 1981, PAP technology has diversified exponentially. Compact and quiet fixed continuous PAP flow generators, autotitrating PAP devices, and bilevel PAP devices that can treat multiple sleep-disordered breathing phenotypes including OSA, central sleep apnea (CSA), combinations of OSA and CSA, and hypoventilation are available. Adaptive servo-ventilators can suppress Hunter-Cheyne-Stokes breathing and CSA and treat coexisting obstructive events. Volume-assured pressure support

Computational models for the study of heart-lung interactions in mammals.

The operation and regulation of the lungs and the heart are closely related. This is evident when examining the anatomy within the thorax cavity, in the brainstem and in the aortic and carotid arteries where chemoreceptors and baroreceptors, which provide feedback affecting the regulation of both organs, are concentrated. This is also evident in phenomena such as respiratory sinus arrhythmia where the heart rate increases during inspiration and decreases during expiration, in other types of synchronization between the heart and the lungs known as cardioventilatory coupling and in the associati

[Theophylline in the treatment of sleep-related breathing disorders].

The methylxanthine derivates are known to have respiratory stimulant properties. Therefore theophylline is used in sleep related disturbances of breathing. Theophylline reduces central apneas and periodic breathing in infants. The efficiency of theophyllin is confirmed in reducing central apneas in patients with neurologic diseases or Cheyne Stokes breathing in patients with congestive heart failure. In patients with obstructive sleep apnoea the effect of theophylline is doubtful. An effect of therapy exists in some mild forms of sleep apnoea (apnoea index < 20/h total sleep time). Further stu

Apnea and periodic breathing in normal full-term infants during the first twelve months.

The pediatric pneumogram is a frequently used tool in the diagnosis and management of apnea during infancy. We analyzed 287 pneumographic recordings from 123 full-term infants (63 males) obtained during the first 12 months of life to establish normative values for apnea, periodic breathing, and bradycardia. The results of the analysis were compared by sex and age. The number of infants who exhibited periodic breathing decreased significantly over time (78% at 0-2 weeks vs 29% at 39-52 weeks; P less than 0.05). However, for those infants who did breathe periodically, the percent of sleep time s

Possible mechanisms of periodic breathing during sleep.

To determine the effect of respiratory control system loop gain on periodic breathing during sleep, 10 volunteers were studied during stage 1-2 non-rapid-eye-movement (NREM) sleep while breathing room air (room air control), while hypoxic (hypoxia control), and while wearing a tight-fitting mask that augmented control system gain by mechanically increasing the effect of ventilation on arterial O2 saturation (SaO2) (hypoxia increased gain). Ventilatory responses to progressive hypoxia at two steady-state end-tidal PCO2 levels and to progressive hypercapnia at two levels of oxygenation were meas

How to take arms against central apneas in heart failure.

Introduction Despite being a risk mediator in several observational studies, central apneas are currently orphan of treatment in heart failure. After the neutral effects on survival of two randomized controlled trials (RCTs) based on the use of positive airway pressure (the CANPAP and SERVE-HF trials), two alternative hypotheses have been formulated: 1) Periodic breathing/Cheyne-Stokes respiration (PB/CSR) in HF is protective. Indeed, the Naughton's hypothesis assumes that hyperventilation leads to increased cardiac output, lung volume, oxygen storage and reduced muscle sympathetic nerve activ

[Circulatory sleep apnea: Preliminary report of clinical observation on sleep apnea in patients with chronic heart failure].

OBJECTIVE: The aim of this study is to investigate the occurrence and mechanism of Cheyne-Stokes breathing pattern in patients with heart failure. METHODS: Fifty-six patients who performed polusomnography sleep testing at National Center of Cardiovascular Diseases Fuwai Hospital from March to May in 2015. We divided them into chronic heart failure (CHF) group and non-CHF group. RESULTS: The occurrences of sleep apnea in two groups were high. In CHF group (n = 11) , there were 10 patients with apnea hypopnea index (AHI) > 5; and their AHI was 23.93 ±14.63. In non-CHF group (n = 45), there wer

Bench test evaluation of adaptive servoventilation devices for sleep apnea treatment.

RATIONALE: Adaptive servoventilation devices are marketed to overcome sleep disordered breathing with apneas and hypopneas of both central and obstructive mechanisms often experienced by patients with chronic heart failure. The clinical efficacy of these devices is still questioned. STUDY OBJECTIVES: This study challenged the detection and treatment capabilities of the three commercially available adaptive servoventilation devices in response to sleep disordered breathing events reproduced on an innovative bench test. METHODS: The bench test consisted of a computer-controlled piston and a St

Effects of neural drives on breathing in the awake state in humans.

We have developed a mathematical model of the regulation of ventilation that successfully simulates breathing in the awake as well as in sleeping states. In previous models, which were used to simulate Cheyne-Stokes breathing and respiration during sleep, the controller was only responsive to chemical stimuli, and allowed no ventilation at sub-normal carbon dioxide levels. The current model includes several new features. The chemical controller responds continuously to changes in P(CO(2)) with a lower sensitivity during hypocapnia than in the hypercapnic ranges. Hypoxia interacts multiplicativ

Neural bases for the genesis and CO2 therapy of periodic Cheyne-Stokes breathing in neonatal male connexin-36 knockout mice.

Periodic Cheyne-Stokes breathing (CSB) oscillating between apnea and crescendo-decrescendo hyperpnea is the most common central apnea. Currently, there is no proven therapy for CSB, probably because the fundamental pathophysiological question of how the respiratory center generates this form of breathing instability is still unresolved. Therefore, we aimed to determine the respiratory motor pattern of CSB resulting from the interaction of inspiratory and expiratory oscillators and identify the neural mechanism responsible for breathing regularization induced by the supplemental CO2 administrat

Assessing new treatments for central sleep Apnoea in heart failure: sample size considerations for reliable detection of safety signals.

PURPOSE: Central sleep apnoea (CSA) and Cheyne-Stokes respiration (CSR) are prevalent in patients with heart failure with reduced ejection fraction (HFrEF) and are associated with increased mortality. Previous trials evaluating adaptive servo-ventilation (ASV) for CSA/CSR in HFrEF have yielded conflicting safety outcomes. This analysis aims to determine the sample size and number of events required to reliably detect safety signals, specifically all-cause mortality, in future trials assessing new treatments for CSA/CSR in HFrEF patients. METHODS: Utilizing data from the SERVE-HF trial, we con

Central sleep apnea.

Central apnea syndrome is a disorder with protean manifestations and concomitant conditions. It can occur as a distinct clinical entity or as part of another clinical syndrome. The pathogenesis of central sleep apnea (CSA) varies depending on the clinical condition. Sleep-related withdrawal of the ventilatory drive to breathe is the common denominator among all cases of central apnea, whereas hypocapnia is the final common pathway leading to apnea in the majority of central apnea. Medical conditions most closely associated with CSA include heart failure, stroke, spinal cord injury, and opioid
