Cochlear implant surgery through oval window: A novel approach in a child with facial nerve aberration.

Cochlear implantation is safe for simple case presentations. However, facial nerve (FN) aberrations may impede insertion of the electrode through a round window and increase the risk of iatrogenic FN injury. We developed a novel method of cochlear implantation, a present the case of a child with FN aberrations. Inferior displacement of a horizontal segment of FN lay on the surface of th e promontory, obscuring the round window. The oval window was used and the electrode successfully inserted. No postoperative complications occurred, and significant improvement in speech perception was achieved

Peri-operative electrically evoked auditory brainstem response assessment of facial nerve/cochlea interaction at cochlear implantation.

OBJECTIVES: Dehiscence between the cochlear otic capsule and the facial nerve canal is a rare and relatively newly described pathology. In cochlear implantation (CI), this dehiscence may lead to adverse electric facial nerve stimulation (FNS) already at low levels, rendering its use impossible. Here, we describe an assessment technique to foresee this complication. METHODS: Pre- and postoperative computed tomography (CT) scans and intraoperative electrically evoked auditory brainstem response (e-ABR) measurements were analyzed in two patients with cochlear-facial dehiscence (CFD). RESULTS: B

Contralateral cochlear implantation prior to vestibular nerve section for 'drop attacks' in the only hearing ear.

BACKGROUND: A dilemma occurs in the treatment of second-sided Ménière's disease in the only hearing ear, particularly in patients with severe symptoms such as 'drop attacks'. This paper describes a patient treated with contralateral cochlear implantation prior to vestibular nerve section of the symptomatic ear. CASE REPORT: A 53-year-old man, with second-sided Ménière's disease and drop attacks in the only serviceable right ear, underwent successful left cochlear implantation 30 years after hearing loss, followed by right vestibular nerve section. The patient achieved control of Ménière's att

Evaluation of cochlear nerve size by magnetic resonance imaging in elderly patients with sensorineural hearing loss.

PURPOSE: We aimed to determine any differences, if present, between the cross-sectional area of the cochlear nerve (CN) of elderly patients with sensorineural hearing loss (SNHL) and of young patients with normal hearing. MATERIALS AND METHODS: The study group included ten patients with age-related SNHL. Fourteen volunteer individuals with normal hearing were recruited as the control group. T1-weighted, T2-weighted and parasagittal three-dimensional Fourier transformation constructive interference in steady state (3DFT-CISS) reconstruction images of all cases were evaluated. Images were exami

Retrofacial approach of cochlear implantation in inner ear malformation with aberrant facial nerve: a case report.

Cochlear implantation is regarded as a safe surgery for young children with minimal complications. However, inner ear malformations and aberrant course of facial nerves may impede electrode insertion via the round window approach and increase the risk of iatrogenic facial nerve injury. We report a case of cochlear incomplete partition in a patient with anomalous facial nerve anatomy. The anterior and inferior displacement of the facial nerve obscured the round window. A retrofacial approach was used to expose the round window and the electrode was inserted successfully. No surgical complicatio

The effect of the acoustic nerve chronic electric stimulation upon the guinea pig cochlear nucleus development.

14 newborn guinea pigs have been deafened three days after birth by bilateral destruction of the organ of Corti, and among them 8 have been supplied with a chronic auditory electric stimulation by means of an intracochlear implanted electrode. The histological study and the anatomical reconstructions of these animals' cochlear nuclei demonstrate that this chronic stimulation prevents, at least partially, these auditory pathway medullary formations from atrophy due to the cochlear destruction.

Facial and Cochlear Nerve Complications following Microsurgical Resection of Vestibular Schwannomas in a Series of 221 Cases.

BACKGROUND: Despite improvements in microsurgical technique and the use of intraoperative electrophysiological monitoring, the potential for facial and cochlear nerve injury remains a possibility in the resection of vestibular schwannomas (VS). We reviewed a series of 221 cases of VS resected via a retrosigmoid approach at our institution from October 2008 to April 2014 and determined the incidence of postoperative facial and cochlear deficits. MATERIAL AND METHODS: A total of 221 patients - 105 (47.5%) male and 116 (52.5%) female - with a mean age of 46.1 years (range 29-73 years), with VS ≥

Histological considerations of the cleavage plane for preservation of facial and cochlear nerve functions in vestibular schwannoma surgery.

OBJECT: The authors analyzed the tumor capsule and the tumor-nerve interface in vestibular schwannomas (VSs) to define the ideal cleavage plane for maximal tumor removal with preservation of facial and cochlear nerve functions. METHODS: Surgical specimens from 21 unilateral VSs were studied using classical H & E, Masson trichrome, and immunohistochemical staining against myelin basic protein. RESULTS: The authors observed a continuous thin connective tissue layer enveloping the surfaces of the tumors. Some nerve fibers, which were immunopositive to myelin basic protein and considered to be r

Microsurgical results with large vestibular schwannomas with preservation of facial and cochlear nerve function as the primary aim.

OBJECTIVE: To evaluate our microsurgical results in dealing with vestibular schwannomas (VS) greater than or equal to 30 mm when preservation of cranial nerve function was considered more important than total tumour removal. METHODS: Sixteen consecutive cases were operated on by the same neurosurgeon according to a prospective protocol using intraoperative neuro-monitoring (IONM) based on electromyographic and brain stem auditory evoked potential recordings. Facial nerve function was evaluated on the House-Brackmann Scale and cochlear nerve function on the Gardner-Robertson Scale. Someone not

Recurrence of vestibular (acoustic) schwannomas in surgical patients where preservation of facial and cochlear nerve is the priority.

The risk of tumour recurrence was measured in a series of surgically treated vestibular (acoustic) schwannoma patients where preservation of facial and cochlear nerve function was a routine objective. This report describes the influence of this surgical philosophy on the hazards of tumour recurrence or continued growth from residual tumour cells left in situ. A series of 116 consecutive vestibular schwannoma patients underwent primary surgical resection in a general community hospital by a single neurosurgeon. Recurrence of a tumour was assessed radiologically. Eighteen patients experienced a

Immunohistochemical localization of vimentin and S-100 antigen in small acoustic tumors and adjacent cochlear nerves.

To clarify the involvement of cochlear nerves in small acoustic tumors, we used immunoperoxidase techniques to determine the presence and distribution of vimentin and S-100 antigens in two acoustic tumor specimens and a transected vestibular nerve. Schwann cells and acoustic tumor cells failed to react positively with monoclonal antibody to vimentin. Reaction was observed in mesenchymal-appearing cells within both the normal nerve and the acoustic tumors, predominantly in association with blood vessels. Normal schwann cells and acoustic tumor cells reacted with polyclonal antibody to S-100 ant

Loss of osteoprotegerin expression in the inner ear causes degeneration of the cochlear nerve and sensorineural hearing loss.

Osteoprotegerin (OPG) is a key regulator of bone remodeling. Mutations and variations in the OPG gene cause many human diseases that are characterized by not only skeletal abnormalities but also poorly understood hearing loss: Paget's disease, osteoporosis, and celiac disease. To gain insight into the mechanisms of hearing loss in OPG deficiency, we studied OPG knockout (Opg(-/-)) mice. We show that they develop sensorineural hearing loss, in addition to conductive hearing loss due to abnormal middle-ear bones. OPG deficiency caused demyelination and degeneration of the cochlear nerve in vivo.

Electrically evoked compound action potentials in adult and pediatric cochlear implant users: II. Assessing cochlear nerve health status with the interphase gap effect and the phase locking value.

This study characterized and compared neural synchrony in the cochlear nerve (CN) and the number of CN fibers activated by electrical stimulation. Neural synchrony was quantified using the phase locking value. The number of activated CN fibers was assessed using the effect of increasing the interphase gap (IPG) of biphasic pulses on the electrically evoked compound action potential (eCAP), referred to as the IPG effect. The study included 50 postlingually deafened adults and 51 pediatric cochlear implant (CI) users with Cochlear™ Nucleus® devices. PLV and IPG effect measures were obtained at f

[Cochlear implantation through retro-facial approach with congenital microtia malformation with facial nerve deformity: a case report].

The difficulty of cochlear implantation in patients with congenital microtia is usually increased due to the vague anatomical marks and facial nerve malformation. The common types of facial nerve malformation include facial nerve bony cover loss, aberrant position, and bifurcation malformation. Bifurcation malformation may obscure the oval window, press against stapes, and bifurcate in the vestibular window while obscuring the round window. It is important to correctly identify the facial nerve and choose a reasonable surgical approach to avoid postoperative complications. This article describ
