OBJECTIVE: This study assessed the psychopathological effects of combat in veterans with and without combat stress reaction.
METHOD: Veterans (N=214) from the 1982 Lebanon War were assessed in a prospective longitudinal design: 131 suffered from combat stress reaction during the war, and 83 did not. They were evaluated 1, 2, 3, and 20 years after the war.
RESULTS: Combat stress reaction is an important vulnerability marker. Veterans with combat stress reaction were 6.6 times more likely to endorse posttraumatic stress disorder (PTSD) at all four measurements, their PTSD was more severe, and

BACKGROUND: Research suggests that personality traits have both direct and indirect effects on the development of psychological symptoms, with indirect effects mediated by stressful or traumatic events. This study models the direct influence of personality traits on residualized changes in internalizing and externalizing symptoms following a stressful and potentially traumatic deployment, as well as the indirect influence of personality on symptom levels mediated by combat exposure.
METHOD: We utilized structural equation modeling with a longitudinal prospective study of 522 US National Guard

One hundred and eight veterans with posttraumatic stress disorder (PTSD) were compared with 60 age-matched controls with regard to family history of psychiatric illness. Depressed controls had a higher morbidity risk (MR) for depression and generalized anxiety in siblings/parents and children, respectively. Patients with PTSD did not differ from alcoholics or nonpsychiatric controls on the basis of family history. PTSD was associated with greater familial anxiety when compared with controls who had experienced combat. When World War II and Vietnam veterans with PTSD were compared, a higher MR

OBJECTIVE: Although stigma associated with serious mental illness, substance abuse disorders, and depression has been studied very little is known about stigma associated with Posttraumatic Stress Disorder (PTSD). This study explored stigma related to PTSD among treatment-seeking Operation Enduring Freedom and Operation Iraqi Freedom (OEF/OIF) combat veterans.
METHOD: Sixteen treatment-seeking OEF/OIF veterans with combat-related PTSD participated in focus groups. We used qualitative methods to explore PTSD-related stigma.
RESULTS: Common perceived stereotypes of treatment-seeking veterans w

This study investigated the relationship between combat exposure and adult antisocial behavior in a sample of 2,490 male Army veterans of the Vietnam War who completed questionnaires about their psychological functioning. After adjustment for history of childhood behavior problems, posttraumatic stress disorder diagnosis, and demographic and military characteristics, it was found that veterans who experienced high and very high levels of combat were twice as likely to report adult antisocial behavior as veterans with no or low levels of combat and were also more likely to meet criteria for ant

INTRODUCTION: Adverse childhood experiences (ACEs) and combat exposure are risk factors for developing posttraumatic stress disorder (PTSD) in adulthood. Higher proportions of military service members (SMs) self-report ACEs than do civilians. Combat exposure subsequent to ACEs has been found to predict PTSD severity beyond the expected effect of combat exposure alone. Adverse childhood experiences appear to impede responsivity to treatment of mood disorders; less is known about their impact on responsivity to treatment of PTSD, including following combat exposure. The current study examined wh

Diagnosis of combat post-traumatic stress disorder (PTSD) is largely based on the subjective reports of the claimant and is therefore prone to over-diagnosis of symptoms. Over-diagnosis of combat PTSD is associated with harm. Therefore, it is important to assess the claimant's credibility in assessing PTSD as accurately as possible.

Combat stress reactions (CSR) are a source of significant morbidity on the battlefield, and they initiate the cascade of psychophysiological dysfunction that results in the posttraumatic stress disorders. During Operation Desert Storm in early 1991, very few allied CSR casualties were observed. However, Iraqi soldiers experienced intense conditions likely to result in CSR. Using semistructured interviews, the authors examined Iraqi enemy prisoners of war for the presence of CSR and compared their responses to two groups of U.S. soldiers. Iraqi prisoners of war experienced many CSR symptoms tha

INTRODUCTION: Special Operations Forces (SOF) combat arms and combat support Soldiers are at risk for impaired mental health, such as mood- and stress-related disorders, due to operational and training demands. Additionally, these individuals experience high risk for sustaining mild traumatic brain injury (mTBI). These mTBIs have also been linked to negative psychological outcomes, such as anxiety and depressive symptoms. Studying mental illnesses and their related symptoms alone does not fully address mental health, which may be better understood by 2 separate but overlapping continua measuri

Combat-related mild traumatic brain injury (cmTBI) is a leading cause of sustained physical, cognitive, emotional, and behavioral disabilities in Veterans and active-duty military personnel. Accurate diagnosis of cmTBI is challenging since the symptom spectrum is broad and conventional neuroimaging techniques are insensitive to the underlying neuropathology. The present study developed a novel deep-learning neural network method, 3D-MEGNET, and applied it to resting-state magnetoencephalography (rs-MEG) source-magnitude imaging data from 59 symptomatic cmTBI individuals and 42 combat-deployed

BACKGROUND: Posttraumatic stress disorder (PTSD) may be associated with dysfunctional reward processing. The present study assessed for such dysfunction in both the expectancy and outcome phases of reward processing.
METHODS: Male Vietnam veterans with (n=15) and without (n=11) combat-related PTSD were administered a wheel of fortune-type gambling task. Self-reported ratings of expectancy and satisfaction were collected respectively before and after each experience of monetary gain or loss.
RESULTS: PTSD participants reported both lower expectancy of reward and lower satisfaction with reward

We examined morale as a moderator of the relationship between combat exposure and posttraumatic stress disorder (PTSD) symptoms in a longitudinal study of U.S. soldiers who had participated in a deployment to Iraq. Soldiers (N = 636) completed assessments at 4 (Time 1) and 10 (Time 2) months following their combat deployment. Combat exposure (both breadth and perceived stressfulness), morale, and PTSD symptoms were assessed at Time 1, and PTSD symptoms were assessed again at Time 2. Results of multivariate multiple regressions revealed that morale at Time 1 interacted with both the breadth and

The current article examines the history of therapeutic response in the Israel Defense Forces toward combat stress reaction during two decades, and in the course of three wars: the 1948 War of Independence, the 1956 Sinai Campaign, and the 1967 Six Day War. Three independent sources were used: recorded debriefing materials; interviews conducted with therapists, commanders and military historians; and professional literature on the wars in Hebrew and other languages. The collected material is described and issues of attitudes, treatment approaches and preparedness are discussed.

We investigated explicit (cued recall) and implicit (word completion) memory in Vietnam combat veterans with (n = 24) and without (n = 24) post-traumatic stress disorder (PTSD). Half of the subjects in each group encoded combat, social threat, positive, and neutral words elaboratively, whereas the others encoded these words nonelaboratively. On the cued recall test, under both encoding conditions, both groups recalled more combat words than other words. However, difference scores obtained by subtracting the mean recall for neutral words from the mean recall scores for the other words revealed

OBJECTIVES: Combat exposure is known to increase the risk for mental disorders; however, less is known about the temporal relationship between mental disorders and alcohol misuse or smoking. To better understand these interrelationships, this study investigated mental disorders in association with hazardous drinking and cigarette smoking.
METHODS: Using data from a large population-based military cohort, standardized instruments were used to screen for posttraumatic stress disorder, depression, panic, and other anxiety syndromes. Self-reported use of cigarettes and hazardous drinking was also

OBJECTIVE: The objective of this study was to examine mental health screening practices and attitudes from both counselor and client perspectives in the U.S. Marine Corps substance abuse program.
METHOD: This study examined mental health screening practices and attitudes of 23 substance abuse counselors and 442 clients from six Marine Corps substance abuse counseling centers.
RESULTS: After receiving training on screening and enhanced counseling practices, 76% of counselors reported that they almost always screened their clients for post-traumatic stress symptoms. Seventy-three percent of cl

INTRODUCTION: This narrative review sets out to explore the current literature surrounding the mental health of serving personnel and veterans.
SOURCES OF DATA: UK literature published in peer-reviewed scientific journals and publicly available UK Ministry of Defence reports.
AREAS OF AGREEMENT: Evidence suggests that common mental disorders are more prevalent in the UK serving and veteran personnel than in the general population. Risk factors include being female, adverse childhood events, combat exposure, and physical combat injury.
AREAS OF CONTROVERSY: Post-traumatic stress disorder rat

Studies of trauma commonly concentrate on the psychological and physiological effects of recent violent events. Although today connections are becoming more explicitly drawn, early studies of the aftermath of amputation serve to shed light on modern understanding of the interaction of the physical and emotional. The study of combat amputation, dissociation, and related posttraumatic stress largely began with the work of 19th-century Philadelphia physician Silas Weir Mitchell, who brought attention to the phenomenon of phantom limb pain. Less known, however, are the data he and his son, John K.
