The Latarjet-Patte Procedure Past, Present, and Future.

The Latarjet-Patte procedure has a long and storied history that predates shoulder arthroscopy, but its popularity has increased over the past two decades as a treatment for anterior glenohumeral instability with bone loss. Transfer of the coracoid process and conjoint tendon to the anteroinferior glenoid creates a triple-blocking effect that is both static and dynamic, lending it biomechanical superiority over alternative bone block techniques. Patients undergoing the Latarjet-Patte procedure have low rates of recurrent instability compared to arthroscopic soft-tissue stabilization procedures

Full Arthroscopic Eden-Hybinette Procedure Using 2 Cortical Suture Buttons for the Reconstruction of Anteroinferior Glenoid Defects.

Therapeutic management of recurrent anterior shoulder instability with an anterior glenoid defect and a Hill-Sachs lesion requires a bone graft to restore the width of the glenoid. The Latarjet procedure is the most popular technique but an iliac crest bone graft is preferred when the coracoid process is dysplastic or too short or after failure of Latarjet or Bristow-Latarjet. The purpose of this manuscript is to describe a full arthroscopic Eden-Hybinette-guided technique with 2 cortical suture buttons for bone graft fixation. This procedure allows reconstruction of severe glenoid bone defect

Anomalous pectoral musculature.

Anomalous disposition of pectoral muscles was encountered in an adult female cadaver on the left side. A prominent cleft separating the sternocostal and clavicular portions of the pectoralis major was noticed. The fibers of pectoralis major were partially fused with the deltoid, resulting in obliteration of the deltopectoral groove. Interestingly, cephalic vein was seen traversing superficial to the clavicular portion of the pectoralis major and pierced it to drain into the axillary vein. The pectoralis minor was inserted mainly on the coracoid process and few fibers were found blending with t

Bilateral variation of subclavius muscle resembling subclavius posticus.

During routine anatomical dissection, a bilateral variation of the muscle subclavius was discovered with additional morphological features consistent with the muscle subclavius posticus. Attached medially to the first rib by tendon and to the clavicle by fibrous bands, the long triangular shaped muscle ran dorsal-laterally to attach onto the transverse scapular ligament and the coracoid process. The scapular insertion of the omohyoid muscle was just medial to this muscle, with some intermingling of fibers close to their insertions. In addition, the muscle on the left was split into two heads b

[Rotator cuff ruptures with predominant involvement of the subscapular tendon].

Among the all rotator cuff tears, the subscapularis lesions are quite rare. But a careful analysis leads to recognize them specially in case of antero-medial impingement between the coracoid process and the head of the humerus. This study of 25 observations where the rupture of the subscapularis was the predominant lesion, allows to emphasize some characteristics of them. The patients are often younger than for the other ruptures, a traumatic experience is not rare at the beginning of the history, the pain is usually the first symptom before the functional disability, the alterations of the ro

A complicated variation of the upper extremity vascularisation.

In the right upper extremity of a cadaver, the radial artery originated from the medial side of the brachial artery at a higher level than usual, which was 16.7 cm below the coracoid process. Then, this artery crossed over the brachial artery, passed to its lateral side and ran to the forearm in front of the aponeurosis of the biceps brachii muscle. The brachial artery ended 1 cm distal to the elbow joint by dividing into the ulnar, anterior interosseous and posterior interosseous arteries. The common interosseous artery was absent. As the radial and ulnar arteries may be used as microvascular

Biceps tenodesis for painful inferior subluxation of the shoulder in adult acquired hemiplegia.

Painless inferior subluxation of the shoulder is a common finding following stroke and is a manifestation of the adynamic nature of the disorder. The weight of the limb is not counterbalanced by shoulder musculature. This is a preliminary report of a simple tenodesis procedure performed in six patients with uniformly successful reduction of the subluxation. The tenodesis is performed through a short deltopectoral approach. The tendon of the long head of the biceps is looped over the coracoid process of the scapula and secured with a barbed staple, thus reducing the subluxation. The indications

Shoulder Acromioclavicular and Coracoclavicular Ligament Injuries: Common Problems and Solutions.

Injuries to the acromioclavicular joint and coracoclavicular ligaments are common. Many of these injuries heal with nonoperative management. However, more severe injuries may lead to continued pain and shoulder dysfunction. In these patients, surgical techniques have been described to reconstruct the function of the coracoclavicular ligaments to provide stable relationship between the clavicle and scapula. These surgeries have been fraught with high complication rates including clavicle and coracoid fractures, infection, loss of reduction and fixation, hardware migration, and osteolysis. This

Unusual Variation of the Biceps Brachii with Possible Median Nerve Entrapment.

The biceps brachii is one of three muscles of the anterior compartment of arm. Variations of the biceps brachii are not rare. The most frequent is the existence of a third head called the humeral head by Le Double (1897) (Rodríguez-Vázquez et al., 1999). Our article is based on the unexpected result of a routine dissection class held for medical students. Dissection was performed according to the guidelines accepted by the anatomy department (Seichert, 1999). We describe a third (accessory) head of the biceps brachii. In addition of two regular heads, the third head originated together with th

Coracoclavicular involvement--an atypical manifestation in rheumatoid arthritis.

An inception cohort of 74 patients with seropositive and erosive RA were followed up for 15 years. All 148 shoulders were radiographed with a standard method at the 15-year follow-up. The coracoclavicular region was evaluated from each radiograph. In addition, the distance between the processus coracoideus and the superior margin of the clavicle was measured. Only one clavicle had elongated, shallow erosion on the undersurface at the insertion area of the coracoclavicular ligaments. In this case the distance between the coracoid process and clavicle was 25 mm, whereas the mean distance of all

Giant cell tumor of the scapula. A review of 13 cases.

This study reviews the demographic, radiologic, and histologic characteristics of 13 cases of an important primary skeletal neoplasm, giant cell tumor of bone, occurring in an uncommon location, the scapula. That eight of 13 patients presented prior to 20 years of age contrasts significantly with the typical age distribution (between 20-40 years) encountered in giant cell tumors arising in long bones. As it does elsewhere in the skeleton, giant cell tumor of the scapula frequently demonstrates cystic and/or telangiectatic components on histologic examination. The radiologic appearances of gian

Is the two-dimensional computed tomography scan analysis reliable for coracoid graft positioning in Latarjet procedures?

BACKGROUND: The aim of the study was to develop a computed tomography (CT)-based measurement protocol for coracoid graft (CG) placement in both axial and sagittal planes after a Latarjet procedure and to test its intraobserver and interobserver reliability. METHODS: Fifteen postoperative CT scans were included to assess the intraobserver and interobserver reproducibility of a standardized protocol among 3 senior and 3 junior shoulder surgeons. The evaluation sequence included CG positioning, its contact area with the glenoid, and the angle of its screws in the axial plane. The percentage of C

Ectopic tendons of the pectoralis minor muscle as cause for shoulder pain and motion inhibition-Explaining clinically important variabilities through phylogenesis.

OJECTIVE: Clinical consequences of ectopic tendons of the pectoralis minor muscle (PMM) for shoulder pain and range-of-motion limitation have been demonstrated. For better understanding the existence of such ectopic tendons, a phylogenetic hypothesis is proposed. METHODS: Forty-five shoulders of anatomical specimens were dissected and examined. Insertions areas of PMM were measured and occurring aberrant tendons were identified. Their relationship with the coracohumeral ligament (CHL) described and samples of the ligament were collected and histologically stained. RESULTS: The prevalence of

Unobvious diagnosis: subcoracoid impingement. Current knowledge about this condition.

Subcoracoid impingement (SCI) is one of the possible causes in anterior shoulder pain that can be misdiagnosed or missed. It results from a conflict between the coracoid process and the lesser tuberosity of the humeral head. The collision mainly affects the subscapularis tendon that is at risk of degeneration and tearing. The symptoms arise by positioning the shoulder in flexion, adduction and internal rotation, dynamically decreasing the coracohumeral distance, the most important diagnostic parameter in the SCI. The impingement has been associated with shoulder overuse that may occur in speci

[Pedicled supraclavicular osteocutan island flap for tracheostoma closure].

Authors present a case of a 52-year-old female patient, who underwent an emergency tracheostomy due to life threatening dyspnoea caused by an external compression of a large goiter. Total thyreoidectomy needed to be carried out later, too. Since the atypical tracheostomy did not close spontaneously a reconstruction was planned. A part of the anterior wall of the trachea needed to be replaced, which was done by an osteocutaneous flap on raised on the supraclavicular artery. An island on the artery was harvested with a thin bone chip taken from the coracoid process of the clavicle, which was rot

Acromioclavicular joint injuries: diagnosis and management.

Acromioclavicular joint injuries represent nearly half of all athletic shoulder injuries, often resulting from a fall onto the tip of the shoulder with the arm in adduction. Stability of this joint depends on the integrity of the acromioclavicular ligaments and capsule as well as the coracoclavicular ligaments and the trapezius and deltoid muscles. Along with clinical examination for tenderness and instability, radiographic examination is critical in the evaluation of acromioclavicular joint injuries. Nonsurgical treatment is indicated for type I and II injuries; surgery is almost always recom
