Why did NICE not Recommend AI for CXR Reading in the UK?

The National Institute for Health and Care Excellence (NICE), which advises the National Health Service (NHS) on cost-effective resource allocation, introduced its Early Value Assessment (EVA) pathway in 2020 to accelerate access to promising digital and artificial intelligence (AI) technologies while further evidence is generated. However, one of the first EVA cohorts not to receive a conditional recommendation comprised 13 AI tools designed to support interpretation of chest X-rays (CXR) for suspected lung cancer in primary care referrals. This manuscript examines the rationale for that deci

Health metrics for helminth infections.

Health metrics based on health-adjusted life years have become standard units for comparing the disease burden and treatment benefits of individual health conditions. The Disability-Adjusted Life Year (DALY) and the Quality-Adjusted Life Year (QALY) are the most frequently used in cost-effect analyses in national and global health policy discussions for allocation of health care resources. While sometimes useful, both the DALY and QALY metrics have limitations in their ability to capture the full health impact of helminth infections and other 'neglected tropical diseases' (NTDs). Gaps in curre

Using social indicators to predict addiction.

Because of cost and other constraints, states often find it difficult to estimate need for treatment of alcohol-related problems from routine surveys. The social undesirability of illegal drug use makes the assessment of need for treatment of their use even more difficult. This paper uses independently obtained treatment need estimates to provide parameters for short-term prediction. We obtained the parameters by regressing the proportions of people addicted to alcohol (or drugs) in counties on social-indicator-based relative treatment need estimates for alcohol (or drugs). In addition to inte

Accounting for unmet need in equitable healthcare resource allocation: Synopsis.

BACKGROUND: The National Health Service uses formulae based on historic patterns of service use to distribute funding locally. These formulae are adjusted using avoidable mortality rates, to account for unmet needs and address health inequalities. We do not know whether this approach accurately reflects variation in unmet need between areas. METHODS: We define need for National Health Service resources as the expected expenditure required to provide all standard National Health Services from which a population can benefit. We clarify the two components required to measure need: the relevant c

Age Is a Predictor of In-Hospital Outcomes for Left Ventricular Assist Device Implantation: A Nationwide Analysis.

The 2018 heart allocation system has significantly influenced heart transplantation and left ventricular assist device (LVAD) utilization. Our study aims to investigate age-related outcomes following LVAD implantation in the post-allocation era. Using the National Inpatient Sample, we analyzed data from 7375 patients who underwent LVAD implantation between 2019 and 2020. The primary endpoint was in-hospital mortality following LVAD implantation, stratified by age categories. The age groups were 18-49, 50-59, 60-69, and over 70. These represented 26%, 26%, 31%, and 17% of patients, respectively

Cost Analysis of an Intervention to Prevent Methicillin-Resistant Staphylococcus Aureus (MRSA) Transmission.

INTRODUCTION: Our objective was to assess the cost implications of a vertical MRSA prevention program that led to a reduction in MRSA bacteremia. METHODS: We performed a matched historical cohort study and cost analysis in a single hospital in Israel for the years 2005-2011. The cost of MRSA bacteremia was calculated as total hospital cost for patients admitted with bacteremia and for patients with hospital-acquired bacteremia, the difference in cost compared to matched controls. The cost of prevention was calculated as the sum of the cost of microbiology tests, single-use equipment used for

Informing the Equitable Implementation of Hepatitis C Interventions: Cost-Effectiveness Modeling in Rural Populations of People Who Inject Drugs.

OBJECTIVES: This study evaluates the cost effectiveness of implementing evidence-based interventions among persons who inject drugs (PWID) in rural settings, where structural barriers and limited resources contribute to disproportionate hepatitis C virus (HCV) burden. Findings are intended to guide equitable implementation planning and resource allocation. METHODS: We developed an economic-epidemiological model to describe HCV transmission among PWID to assess the cost effectiveness of testing, treatment, and harm reduction interventions over a 20-year time horizon. Outcomes included healthca

The cost comparison of rhythm and rate control strategies in persistent atrial fibrillation.

UNLABELLED: Our sub study was designed to analyze the cost effectiveness of two alternative treatment strategies with a view to improved allocation of the limited therapeutic resources. To that effect we conducted detailed analysis of the related costs and other relevant data collected in the course of the HOT CAFE study. METHODS: The prospective costs related to 205 patients randomly assigned to rhythm or rate control were traced over a 12 month period. Since, both strategies produced similar clinical outcomes a cost minimization analysis was undertaken. The cost of diagnostic and treatment

Perspectives in the Value Assessment of Comprehensive Genomic Profiling: A Multi-Criteria Decision Analysis in NSCLC among Stakeholders.

OBJECTIVES: Comprehensive genomic profiling (CGP) can identify more actionable variants than targeted profiling (TP) in advanced non-small cell lung cancer (NSCLC), yet its clinical benefit and cost-effectiveness remain uncertain. Additional criteria may also influence reimbursement decisions. We conducted a multi-criteria decision analysis (MCDA) to explore how Dutch stakeholders value CGP compared to TP. METHODS: Seven decision criteria were evaluated: clinical benefit, cost-effectiveness, feasibility, patient/physician test journey, wider implications of diagnostic results, organization of

Economic evaluation of caregiver interventions for children with developmental disabilities: A scoping review.

Globally, families with children with developmental disabilities (DDs) experience challenges, including social isolation, stigma, and poverty, especially in low-income settings in Africa. Most children with DDs in Africa remain unidentified and receive no formal support. Caregiver interventions focusing on education and training for the carers/parents have been shown to be adaptable and low intensity in implementation, although the economic evidence is limited. This review aimed to describe the evidence and methodological aspects of economic evaluations for caregiver interventions for DDs. The

Cost-effectiveness of surgical interventions in low-income and middle-income countries: a systematic review and critical analysis of recent evidence.

BACKGROUND: Cost-effectiveness evidence is a critical tool to support resource allocation decisions. There is growing recognition that the development of benefit packages for surgical care should be guided by such evidence, particularly in resource-constraint settings. METHODS: We conducted a systematic review of evidence (Medline, Embase, Global Health, EconLit and grey literature) on the cost-effectiveness of surgery across low-income and middle-income countries published between January 2013 and January 2023. We included studies with minor and major therapeutic surgeries and minimally inva

The economic evaluation of Cystic echinococcosis control strategies focused on zoonotic hosts: A scoping review.

BACKGROUND: Cystic echinococcosis (CE) is a zoonotic neglected tropical disease (zNTD) which imposes considerable financial burden to endemic countries. The 2021-2030 World Health Organization's roadmap on NTDs has proposed that intensified control be achieved in hyperendemic areas of 17 countries by 2030. Successful interventions for disease control, and the scale-up of programmes applying such interventions, rely on understanding the associated costs and relative return for investment. We conducted a scoping review of existing peer-reviewed literature on economic evaluations of CE control st

High flow, at a higher cost? A pilot study comparing direct vs. indirect bypass for Moyamoya disease using time-driven activity-based costing.

BACKGROUND AND OBJECTIVE: In the management of adult moyamoya disease (MMD), there is clinical equipoise regarding direct bypass (DB) vs indirect bypass (IB). One factor that may influence the choice of procedure is cost. Therefore, we sought to compare the true intraoperative costs (not charges or reimbursement) of DB vs. IB using a novel costing methodology, time-driven activity-based costing (TDABC). METHODS: A retrospective cohort study (2017-22) was conducted of all patients undergoing revascularization for symptomatic MMD. TDABC was used to calculate intraoperative costs. Supply cost wa

[Model for the regional allocation of the National Health Care Fund].

In 1978 a National Health Service (Servizio Sanitario Nazionale = SSN) was constituted in Italy which exercises jurisdiction in the sector of health care and is duty bound to assist all citizens. Basically speaking, the NHS is organized on three levels (national, regional and local) with the management of direct operations assigned to the (about 700) Local Health Boards (Unità Sanitaria Locale = USL) each of which covers a well determined territorial area. The Authors indicate that rarely discussed or evaluated are the procedures for the regional allocation of health care funding which is dete

Testing optimal defense theory: Root resistance selection in chicory (Cichorium intybus) reduces foliar defense and alters aphid performance and feeding behavior.

BACKGROUND: Plants are constantly exposed to a wide range of herbivores at both aboveground (AG) and belowground (BG) levels, involving multiple constitutive defenses in leaves and roots. However, the Optimal Defense Theory predicts that the energy cost of defenses prevents plants from defending all organs equally. In an agroeconomic context, selecting genotypes for their root-specific resistance may therefore create trade-offs in the allocation of constitutive defenses, potentially leaving leaves more vulnerable to aerial herbivores. To test this hypothesis, we assessed the resistance to the

Techno-economic and environmental assessments of optimal planning of waste-to-energy based CHP-DG considering load growth on a power distribution network.

Load growth puts pressure on existing electric infrastructure and impacts on the system's performance parameters which may necessitate network expansion. Conventionally, electric network expansion is done by building new substations or reinforcing the existing ones with new transformers and upgrading the network feeders. However, optimal allocation of combined heat and power distributed generators (CHP-DGs) on distribution networks (DNs) can be adopted for network expansion planning problem. Optimal DG allocation is an optimisation problem which requires an efficient optimisation approach. In
