Interdisciplinary Safety Evaluation for Learning and Decision-Making.

The FDA IND safety reporting Final Rule (21CFR 312.32) applies to all human drugs and biological products being studied under an Investigational New Drug (IND). A sponsor must file an IND safety report for any serious unexpected suspected adverse reaction (SUSAR) of a medicinal product being investigated. Some events may be obviously drug-related (e.g., agranulocytosis, anaphylactic reaction, drug-induced hepatic injury, Stevens-Johnson Syndrome). For serious adverse events that are not interpretable as individual occurrences, additional processes and procedures need to be employed for identif

How Ancestral Trauma Informs Patients' Health Decision Making.

This article considers intergenerational trauma by drawing on the experience of a 37-year-old Black woman whose great-grandfather died as a result of involuntary involvement in the US Public Health Service Syphilis Study at Tuskegee. Although she never met her great-grandfather, the abuse, exploitation, and human rights violations he suffered at the hands of the US government profoundly influenced her health experiences. This article contextualizes her experiences in light of past medical abuse and microethics.

Intraoperative Decision-Making: Thermal Injury During Robotic Pyeloplasty.

OBJECTIVE: To describe how an intraoperative thermal injury to the ureter was handled during a robotic dismembered pyeloplasty. METHODS: The patient was a 5-year-old male who was undergoing robotic dismembered pyeloplasty for a symptomatic left ureteropelvic junction (UPJ) obstruction. The case started with cystoscopy and retrograde ureteral catheter placement prior to moving to the robotic part of the procedure. RESULTS: During initial proximal ureteral dissection, there was an inadvertent thermal injury to the ureter due to arcing of the activated monopolar scissors via the fenestrated bip

Decision Making With Imaging in Asymptomatic Aortic Regurgitation.

The authors review the optimal sequence of multimodality imaging studies in patients with chronic asymptomatic aortic regurgitation (AR). The initial questions in evaluating such patients are: 1) is AR severe; and 2) does it significantly impact the left ventricle? Although transthoracic echocardiography is usually able to answer these questions, a cardiovascular magnetic resonance imaging study might be necessary if the data from echocardiography are conflicting or inadequate. If AR is severe and the left ventricle is severely dilated, surgery is indicated. If AR is severe but the left ventri

Decision-Making in Adolescents with Profound Hearing Loss.

People with profound hearing loss show differences in language-related cognitive functions that may affect decision-making processes, but few studies have examined their decision-making behavior. The current study used the Iowa Gambling Task and the Game of Dice Task to explore the decision-making characteristics of adolescents with profound hearing loss. In the Iowa Gambling Task, deaf adolescents were more inclined to choose from the deck of infrequent losses with large immediate gains and larger future losses. In the Game of Dice Task, the deaf adolescents showed a preference for high-risk

Neurocognitive decision-making processes of casual methamphetamine users.

Neuroadaptations caused by chronic methamphetamine (MA) use are likely major contributors to high relapse rate following treatment. Thus, focusing intervention efforts at pre-empting addiction in vulnerable populations, thereby preventing MA-use-induced neurological changes that make recovery so challenging, may prove more effective than targeting chronic users. One approach is studying casual/recreational users, not diagnosed with substance use disorder. This group may be at high risk for addiction due to their experience with MA. On the other hand, they may be resilient against addiction sin

Reunification in foster care: Influences on decision-making.

An important goal of out of home care is to prepare the family and child for reunification. Practitioners are often required to make the decision whether to reunify a foster child with their biological family. This study examines this complex reunification decision in Portugal. Using the Judgments and Decisions Processes in Context model, the authors presented to Portuguese child protection workers and higher education students (n = 400) a case vignette of a child in foster care in which reunification is considered. The vignette consider two scenarios related to the child's wish to go back or

Sensory and decision-making processes underlying perceptual adaptation.

Perceptual systems adapt to their inputs. As a result, prolonged exposure to particular stimuli alters judgments about subsequent stimuli. This phenomenon is commonly assumed to be sensory in origin. Changes in the decision-making process, however, may also be a component of adaptation. Here, we quantify sensory and decision-making contributions to adaptation in a facial expression paradigm. As expected, exposure to happy or sad expressions shifts the psychometric function toward the adaptor. More surprisingly, response times show both an overall decline and an asymmetry, with faster responses

Evaluation and Decision Making in Frontal Sinus Surgery.

Management of frontal sinusitis can be challenging for even the most experienced otolaryngologists. A thorough understanding of the anatomy and pathophysiology of the frontal sinus is essential to properly manage disease affecting the frontal sinus. Being able to distinguish acute viral from acute bacterial and acute from chronic sinusitis is crucial because these distinctions guide appropriate management. Nasal endoscopy can confirm diagnosis, and radiologic imaging, including computed tomography and MRI, is often a necessary adjunct that aids in determining appropriate therapeutic decisions.

Decision Making Among Persons Living With Heart Failure.

BACKGROUND: Persons with heart failure (HF) are required to make decisions on a daily basis related to their declining health and make urgent decisions during acute illness exacerbations. However, little is known about the types of decisions patients make. OBJECTIVE: The aims of this study were to critically evaluate the current quantitative literature related to decision making among persons with HF and identify research gaps in HF decision-making research. METHODS: A systematic search of literature about decisions persons with HF make was conducted using PubMed, CINAHL, and PsychINFO datab

Device failure in sialoendoscopy: Intraoperative practical decision-making.

Sialoendoscopy has recently emerged as an alternative to gland excision in management of recurrent sialoadenitis and sialolithiasis. This technique has both diagnostic and therapeutic purposes. We report a case of unusual device failure during removal of a submandibular stone from Wharton's duct, which ultimately led to ductal avulsion. Sialoendoscopy is safe and effective in management of non-neoplastic major salivary gland disorders. Ductal avulsion can happen during mechanical procedures like stone removal or dilation of strictures. Failing to release the stone from the basket is very rare

Becoming a mother: Supported decision-making in context.

Little is known about how women with intellectual disabilities make decisions in relation to pregnancy. Social support is important for mothers with intellectual disabilities in many areas. This study explored how the support network influenced the decision-making of women with intellectual disabilities in relation to pregnancy. The study extended previous research to include the multiple perspectives of mothers, personal and professional support network. A model of decision-making was constructed, with implications of how services approach working with mothers with intellectual disabilities.

Decision-making impairments in patients with Wilson's disease.

Wilson's disease (WD) causes deposition of copper, mainly in the basal ganglia. One consequence of deposition seems to be impairment of executive functions, which could cause problems in decision making. In 30 WD patients and 30 healthy controls (HCs), we examined decision making under risk in the Game of Dice Task, and we assessed working memory and executive functions. WD patients exhibited a greater preference for disadvantageous choices than did HCs. Reduced decision-making performance was closely correlated to lower executive functions. Decision-making deficits of WD might be associated w

Treatment of premenstrual syndrome: a decision-making algorithm.

The aim of this short paper will be to guide the clinician through the plethora of possible interventions to help them to individualize treatment for their patients with PMS. The discussion will highlight management principles rather than evidence per se. It uses as its basis an updated version of the treatment algorithm published by the RCOG in its Green Top Guideline no. 48 on the management of PMS.

Development of decision making: sequential versus integrative rules.

Decisions can be made by applying a variety of decision-making rules-sequential rules in which decisions are based on a sequential evaluation of choice dimensions and the integrative normative rule in which decisions are based on an integration of choice dimensions. In this study, we investigated the developmental trajectory of such decision-making rules. In the Gambling Machine Task, participants choose between options that differ in three dimensions: frequency of loss, amount of loss, and certain gain. The task was administered to 231 children and adolescents (age range=8-17 years). Latent g

Brain function with complex decision making using electroencephalography.

A computer-administered assessment for decision making relevant to daily-living decisions, using the technique of complex decision making (CDM), has been previously developed and tested in our laboratory. The present study sought to identify unique patterns of brain activity in the alpha band associated with CDM. We recorded electroencephalogram (EEG) from 30 scalp sites, during a series of baseline, eyes open fixation tasks, and CDM tasks, in different contexts, in a group of 9 male and 7 female young healthy adults, aged 18 to 34. The decision making contexts, designed to simulate real-world
