Five-Year Outcomes After Decompression and Fusion Versus Decompression Alone in the Treatment of Lumbar Synovial Cysts.

BACKGROUND: Spinal synovial cysts are acquired, fluid-filled lesions of the facet joint that most commonly occur in the lumbar spine. They are thought to arise from degenerative changes and to result from segmental instability. Although the treatment of these lesions has been studied, the long-term implications and effects of the different strategies for surgical intervention (i.e., decompression and fusion vs. decompression alone) have not yet been elucidated or established. METHODS: Using an all-payer database with 53 million patient records (MARINER-53), patients with a diagnosis of lumbar

Multilevel Decompression Surgery for Degenerative Lumbar Spinal Canal Stenosis Is Similarly Effective With Single-level Decompression Surgery.

STUDY DESIGN: Retrospective case series. OBJECTIVE: The purpose of this study was to investigate the outcome of multilevel posterior decompression surgery for degenerative lumbar spinal stenosis compared with single-level surgery. SUMMARY OF BACKGROUND DATA: The clinical outcomes of multilevel decompression surgery are still controversial because previous studies have not been designed to randomize or adjust the patient background. MATERIALS AND METHODS: A retrospective review of prospectively collected data from 659 surgically treated lumbar spinal stenosis patients with a minimum 2-year f

Decompression versus decompression plus fusion for treating degenerative lumbar spinal stenosis: A systematic review and meta-analysis.

BACKGROUND: Degenerative lumbar spinal stenosis (DLSS) is a complex clinical syndrome that leads to spinal compression. Decompression with fusion has been the most commonly used surgical procedure for treating DLSS symptoms for many years. However, the exact role of fusion and its effectiveness in DLSS therapy has recently been debated. OBJECTIVE: The main purpose of this study was to compare the efficacy and safety of decompression alone and decompression plus fusion in the treatment of DLSS with or without spondylolisthesis. STUDY DESIGN: A systematic review and meta-analysis of the therap

Degenerative lumbar spondylolisthesis with spinal stenosis. A prospective study comparing decompression with decompression and intertransverse process arthrodesis.

Fifty patients who had spinal stenosis associated with degenerative lumbar spondylolisthesis were prospectively studied clinically and radiographically to determine if concomitant intertransverse-process arthrodesis provided better results than decompressive laminectomy alone. There were thirty-six women and fourteen men. The mean age of the twenty-five patients who had had an arthrodesis was 63.5 years and that of the twenty-five patients who had not had an arthrodesis, sixty-five years. The level of the operation was between the fourth and fifth lumbar vertebrae in forty-one patients and bet

Analysis of Risk Factors for Postoperative Progressive Segment Degeneration at the Decompression and Non-decompression Segments after Minimally Invasive Lumbar Decompression Surgery: A 5-year Follow-up Study.

INTRODUCTION: The risk factors for the development of progressive segment degeneration (PSD) after decompression surgery are still unknown. In this study, the risk factors for PSD in patients who undergo decompression surgery for lumbar spinal stenosis with and without coexisting spondylolisthesis and scoliosis were examined, focusing on decompression and non-decompression segments. METHODS: We reviewed the data of patients with >5 years of postoperative follow up. Radiographic PSD was defined as either the development of an anterolisthesis or retrolisthesis of >3 mm or a decrease in disc hei

Decompression alone versus decompression with fusion in the treatment of lumbar degenerative spondylolisthesis: evaluating the overlapping meta-analyses.

BACKGROUND: Although many meta-analyses have compared decompression alone with decompression combined with fusion for treating lumbar degenerative spondylolisthesis (LDS), their conclusions remain inconsistent. This study aims to analyze these overlapping meta-analyses to provide clinicians with access to the most reliable evidence, thereby enabling informed treatment recommendations for LDS based on the best available data. METHODS: We performed a comprehensive search for relevant meta-analyses published up to December 20, 2025 across PubMed, Embase, Cochrane Library, and Web of Science datab

Outcomes After Decompression Only Versus Decompression and Fusion for Lumbar Facet Cysts: A Systematic Review and Meta-Analysis.

BACKGROUND AND OBJECTIVES: Lumbar facet cysts (LFCs) are considered the result of facet degeneration and segmental instability. Their surgical management has been controversial. Decompression only (DO) is less invasive, but it does not address the underlying degeneration/spondylolisthesis. Decompression and fusion (DF) is more invasive with higher perioperative morbidity. Comparative studies are scant in current literature. The objective of this study was to identify all such studies and synthesize outcomes including recurrence and reoperation rates. Secondary outcomes included back pain resol

Comparative analysis of lumbar spinal decompression: Propensity-matched evaluation of multilevel vs. single-level decompression for spinal stenosis.

INTRODUCTION: Lumbar spinal stenosis results in pain in the buttocks and/or legs due to reduced space in the spinal canal. MRI findings indicate that approximately 40 % of clinically diagnosed patients have multilevel stenosis. For patients with severe symptoms requiring spinal decompression, surgeons must decide the optimal number of levels to decompress. However, the benefits of operating on multiple stenotic levels remain uncertain, as limited research has explored this issue. RESEARCH QUESTION: Do patients with multi-level lumbar stenosis experience less benefit from spinal decompression

Comparison of Decompression Alone Versus Decompression with Fusion for Stenotic Lumbar Spine: A Systematic Review and Meta-analysis.

The first line of treatment for lumbar spinal stenosis (with or without lumbar degenerative spondylolisthesis) involves conservative options such as anti-inflammatory drugs and analgesics. Approximately, 10%-15% of patients require surgery. Surgical treatment aims to decompress the spinal canal and dural sac from degenerative bony and ligamentous overgrowth. Different studies have given conflicting results. The aim of our study is to clear the confusion by comparing two surgical techniques. This meta-analysis was conducted in accordance with the preferred reporting items for systematic reviews
