Venous air embolism is a rare but clinically significant complication of hyperbaric oxygen therapy (HBOT), which is widely used in the treatment of conditions including carbon monoxide poisoning, decompression sickness, and wound healing. We report a case of systemic venous air embolism following HBOT in an 85-year-old woman treated for carbon monoxide poisoning. The patient had syncope, dizziness and nausea. Abdominal CT revealed multiple venous air bubbles. This case highlights a scarcely reported complication of HBOT and the importance of recognizing imaging findings and potential clinical

McArdle disease is a glycogen storage disease that results in rhabdomyolysis during intense exercise. A number of different triggers have been described. We evaluated a patient with McArdle disease who presented with rhabdomyolysis after recreational scuba diving. There was no concern for barotrauma or decompression sickness. His symptoms resolved with standard-of-care management for non-diving-related rhabdomyolysis. Features of his experience provoked questions about the diving-related factors contributing to his presentation. We present the case and explore possible mechanisms of diving-rel

Headache occasionally occurs during or after scuba diving. Although its significance often is benign, headache may signal a serious neurological disorder in some circumstances. In addition to the usual causes of headache, the diagnostic evaluation should consider otic and paranasal sinus barotrauma, arterial gas embolism, decompression sickness, carbon dioxide retention, carbon monoxide toxicity, hyperbaric-triggered migraine, cervical and temporomandibular joint strain, supraorbital neuralgia, carotid artery dissection, and exertional and cold stimulus headache syndromes. Focal neurologic sym

HBO is the definitive treatment for carbon monoxide poisoning, air or gas embolism, and decompression sickness. It is an important adjunct to other treatment modalities in the management of patients with cyanide poisoning, exceptional blood loss (anemia), crush injury, and gas gangrene. In addition, HBO has many investigational uses such as reduction of edema in acute bowel obstruction before surgical intervention. It should be remembered that the best hope for tissue recovery occurs when HBO is initiated within 4 to 6 hours of injury. After this time irreversible muscle and peripheral nerve d

INTRODUCTION: This study developed a mathematical model capable of accurately estimating inert gas tension within human tissues under continuously changing ambient pressure.
METHODS: Based on Haldane's decompression theory and a multi-tissue model, and grounded in the principle that the rate of gas diffusion is proportional to the pressure gradient, a differential equation was derived and solved. Analytical solutions were obtained for four prototypical ambient pressure patterns: constant pressure, exponential compression, linear compression, and linear decompression. The model's validity was

Decompression sickness can occur in divers even when recommended decompression procedures are followed. Furthermore, the physiological state of individuals can significantly affect bubbling variability. These informations highlight the need for personalized input to improve decompression in SCUBA diving. The main objective of this study is to propose a fundamental framework for a new approach to inert gas exchanges. A physiological model of oxygen delivery to organs and tissues has been built and adapted to nitrogen. The validation of the model was made by transferring the N2 to CO2. Under nor

INTRODUCTION: This study reviews heliox saturation procedures used in offshore commercial diving and focuses on bell excursion dives. It excludes initial compression and final decompression. Our first objective was to trace the history and the reasons behind the successive changes that led to the current practice. Our second objective was to review the current practice and identify problem areas and perspectives.
METHODS: We first present the background of excursion diving and reference key procedures from the US Navy, Comex, and international standards. We then review the procedures of 13 an

A 60-year-old man with hypertension and dyslipidemia complained of chest pain upon ascending from a maximum depth of 27 meters while diving. After reaching the shore, his chest pain persisted, and he called an ambulance. When a physician checked him on the doctor's helicopter, his electrocardiogram (ECG) was normal, and there were no bubbles in his inferior vena cava or heart on a portable ultrasound examination. The physician still suspected that he had acute coronary syndrome instead of decompression illness; therefore, he was transported to our hospital. After arrival at the hospital, stand

Our understanding of decompression physiopathology has slowly improved during this last decade and some uncertainties have disappeared. A better understanding of anatomy and functional aspects of patent foramen ovale (PFO) have slowly resulted in a more liberal approach toward the medical fitness to dive for those bearing a PFO. Circulating vascular gas emboli (VGE) are considered the key actors in development of decompression sickness and can be considered as markers of decompression stress indicating induction of pathophysiological processes not necessarily leading to occurrence of disease s

Despite decompression surgery being a widespread intervention for patients with dorsopathies (i.e. back pain) affecting the lumbar spine, the scientific knowledge on patterns and characteristics of work disability before and after the surgery is limited. Sickness absence (SA) and disability pension (DP) were examined three years before and after surgery in 8558 patients aged 25-60 years who underwent lumbar spine decompression surgery in Sweden. They were compared to individuals with diagnosed dorsopathies but no surgery and individuals from the general population as matched comparison groups.

BACKGROUND: There is an increasing interest in 'transfer under pressure' (TUP) decompression in commercial diving, bridging traditional surface-oriented diving and saturation diving. In TUP diving the diver is surfaced in a closed bell and transferred isobarically to a pressure chamber for final decompression to surface pressure.
METHODS: Tables for air diving and air and oxygen decompression have been compared for total decompression time (TDT), oxygen breathing time as well as high and low gradient factors (GF high and low). These have been considered surrogate outcome measures of estimated

INTRODUCTION: The lung surfactant dipalmitoylphosphatidylcholine (DPPC) settles on the luminal aspect of blood vessels to form active hydrophobic spots (AHS). The nanobubbles that develop at these spots are gas micronuclei from which decompression bubbles expand upon decompression. Variability between "bubblers" and "non-bubblers" is derived from the variability in the AHS. We expected that the level of DPPC in the heart would correlate with the severity of decompression illness (DCI).
METHODS: Thirty-six rats were exposed to 8 bar for 32 minutes.
RESULTS: After decompression, they were exam

People dive out of fascination or for economic reasons. Today, technical devices allow us land-dwelling organisms to breathe underwater. The effect of water pressure has a significant impact on physiological processes in the organism and leads to specific pathophysiologies. A sound knowledge of this is the basis of good medical care for people who dive to depths below the surface. Based on a number of exemplary accident scenarios, the article will examine the important pathophysiologies in diving medicine. Various accident mechanisms produce gas bubbles in the body, which can lead to symptoms

Doppler ultrasound (DU) is used in decompression research to detect venous gas emboli in the precordium or subclavian vein, as a marker of decompression stress. This is of relevance to scuba divers, compressed air workers and astronauts to prevent decompression sickness (DCS) that can be caused by these bubbles upon or after a sudden reduction in ambient pressure. Doppler ultrasound data is graded by expert raters on the Kisman-Masurel or Spencer scales that are associated to DCS risk. Meta-analyses, as well as efforts to computer-automate DU grading, both necessitate access to large databases

The systemic effects of oxygen deficiency or excess are not thoroughly described. Knowledge is evolving towards the description of beneficial and detrimental effects of both extremes of partial pressure of oxygen (PaO2). The cellular and tissue mediators derived from the modulation of the oxidative tone and the production of reactive oxygen species (ROS) are widely characterized biochemically, but the pathophysiological characterization is lacking. Preclinical models support the use of hypobaric hypoxia preconditioning, based on its beneficial effects on ventricular function or its reduction i

INTRODUCTION: This study compared the thermal balance of spinal cord injured (SCI) divers and able-bodied (AB) divers during recreational cold-water dives.
METHODS: Ten divers (5 AB, 5 SCI) in matched pairs dived in a shallow lake (temperature 6°C) for 30 to 36 min wearing 5 mm 'Long John' neoprene wetsuits. A gastrointestinal temperature radio pill recorded gastro-intestinal temperature (Tgi) prior to, immediately after and at 5, 10, 15, 30, 60, 120 min post-dive. Subjective ratings of temperature perception were recorded concomitantly using a visual analogue scale (VAS).
RESULTS: No differ

Introduction: Internationally it is estimated that six million people participate in self-contained underwater breathing apparatus (SCUBA) diving each year. Registries suggest a significant proportion of divers have a current or historical diagnosis of asthma. Previously individuals with asthma were prohibited from diving, however, several contemporary guidelines suggest a select population of patients with asthma may be able to dive with an acceptable degree of risk. Areas covered: Divers with asthma may be at an increased risk of a variety of diving-related medical injuries including; pulmon

IMPORTANCE: Scuba diving is becoming increasingly popular. However, scuba diving is associated with specific risks; 80% of adults and 85% of juvenile divers (aged 6-17 years) have been reputed to have an ear, nose, or throat complaint related to diving at some point during their diving career. Divers frequently seek advice from primary care physicians, diving physicians, and otorhinolaryngologists, not only in the acute setting, but also related to the long-term effects of diving.
OBSERVATIONS: The principles underpinning diving-related injuries that may present to the otorhinolaryngologist r

Diving Accidents in Lakes - a Retrospective Study of a Level-1 Emergency Centre in Switzerland Abstract. Switzerland is a country in the middle of Europe without access to an open sea. Here one does not assume a noteworthy number of diving accidents. However, this study shows a large number and attempts to explore the main risks of diving accidents. The data from 2001 to 2016 of patients had been collected und retrospectively evaluated using the electronic database of the emergency center of the university hospital in Bern, Switzerland. Barotrauma of the ear (69.0 %), decompression accidents (

The frequency of decompression illness was high among the extinct marine "reptiles" and very low among the marine mammals. Signs of decompression illness are still found among turtles but whales and seals are unaffected. In humans, the risk of decompression illness is five times increased in individuals with Patent Foramen Ovale; this condition allows blood shunting from the venous circuit to the systemic circuit. This right-left shunt is characteristic of the "reptile" heart, and it is suggested that this could contribute to the high frequency of decompression illness in the extinct reptiles.
