Automated pupillometry predicts neurological deterioration in the neurosurgical ICU: A retrospective observational study with secondary analysis in traumatic brain injury.

The primary objective of this study was to test whether sedation depth mediates the association between pupillary reactivity, quantified by the Neurological Pupil index (NPi) and significant neurological deterioration in critically ill neurosurgical patients. A prespecified secondary objective was to explore whether this relationship differs between traumatic brain injury (TBI) and non-TBI diagnoses. We conducted a retrospective, single-center study of 360 adults admitted to a neurosurgical ICU (2019-2022) with daily automated pupillometry and detailed sedation records. "Significant deteriorat

Future Directions in Sedation and Analgesia.

The evolution of sedation in intensive care is complex but can help guide expectations of what the future might bring. Sedation and analgesia together with delirium prevention and treatment have evolved in lock step. Longer-acting drugs like lorazepam have been replaced with shorter-acting agents like propofol. Dexmedetomidine has become a frequent treatment of delirium. Longer-acting opioids remain dominant. The target has become lighter sedation rather than deep sedation, but monitoring of sedation and pain remains imperfect and subjective. The future will likely bring the introduction of ul

Design considerations of a randomized controlled trial of sedation level during hip fracture repair surgery: a strategy to reduce the incidence of postoperative delirium in elderly patients.

Background Delirium is an acute change in mental status characterized by sudden onset, fluctuating course, inattention, disorganized thinking, and abnormal level of consciousness. The objective of the randomized controlled trial "A STrategy to Reduce the Incidence of Postoperative Delirium in Elderly Patients" (STRIDE) is to assess the effectiveness of light versus heavy sedation on delirium and other outcomes in elderly patients undergoing hip fracture repair surgery. Our goal is to describe the design considerations and lessons learned in planning and implementing the STRIDE trial. Methods D

Comparison of propofol with pentobarbital/midazolam/fentanyl sedation for magnetic resonance imaging of the brain in children.

OBJECTIVE: Propofol and pentobarbital, alone or combined with other agents, are frequently used to induce deep sedation in children for MRI. However, we are unaware of a previous comparison of these 2 agents as part of a randomized, controlled trial. We compared the recovery time of children after deep sedation with single-agent propofol with a pentobarbital-based regimen for MRI and considered additional variables of safety and efficacy. METHODS: This prospective, randomized trial at a tertiary children's hospital enrolled 60 patients 1 to 17 years old who required intravenous sedation for e

Dreaming in sedation during spinal anesthesia: a comparison of propofol and midazolam infusion.

BACKGROUND: Although sedation is often performed during spinal anesthesia, the details of intraoperative dreaming have not been reported. We designed this prospective study to compare 2 different IV sedation protocols (propofol and midazolam infusion) with respect to dreaming during sedation. METHODS: Two hundred twenty adult patients were randomly assigned to 2 groups and received IV infusion of propofol or midazolam for deep sedation during spinal anesthesia. Patients were interviewed on emergence and 30 minutes later to determine the incidence, content, and nature of their dreams. Postoper

[Practical aspects of sedation in dentistry].

In modern dental practice, the use of sedation to eliminate the fear and anxiety of the patient has become frequent. Unfortunately, according to the legislation, the concept of sedation is inseparable from anesthesia and it can only be performed by an anesthesiologist in a group or Department of Anesthesiology and Resuscitation. The article is devoted to a detailed comprehensive review of sedation in dental and surgical interventions in the maxillofacial area. It details the physiological and pathophysiological data of stress and psycho-emotional discomfort. The article indicates drugs for sed

Early Sedation Intensity and Psychological Outcomes in Critically Ill Adults.

BACKGROUND: Long-term psychological impairment is a major concern for intensive care unit (ICU) survivors. Early deep sedation during mechanical ventilation has been associated with poor short-term outcomes and mortality after ICU discharge; however, its relationship with psychological outcomes remains unclear. AIM: To investigate sedation intensity during the first 24 h of mechanical ventilation and its association with psychological impairment 3 months after ICU discharge. STUDY DESIGN: This retrospective ancillary analysis of a single-centre cohort study was conducted in two general ICUs

Intranasal sedation with diazepam-butorphanol in yellow-legged gulls (Larus michahellis): a preliminary clinical evaluation.

BACKGROUND: Yellow-legged gulls (Larus michahellis) are frequently presented to wildlife clinics, often requiring safe and effective sedation and analgesia for diagnostic procedures. Minimising stress and restraint during handling are critical for both animal welfare and staff safety. To address this need, this study evaluates the first intranasal protocol in gulls, aiming to facilitate routine examination and diagnostic imaging without physical restraint. A total of 16 gulls were included in this prospective, preliminary clinical evaluation. A combination of diazepam (8 mg/kg) and butorphanol

Effects of sedation on subjective perception of pain intensity and autonomic nervous responses to pain: A preliminary study.

Rather than relying solely on subjective pain evaluation using means such as the visual analogue scale (VAS), in clinical situations it is possible to observe evoked responses of the autonomic nervous system (ANS) as objective indicators. Few studies, however, have reported these relationships under finely controlled sedation. 16 healthy male participants were administrated in intravenous sedation with either propofol or midazolam randomly. We initially determined, using pharmacokinetic simulation, the effect-site concentration (Ce) of anaesthetic at loss of response to verbal command and eyel

Sedation with midazolam and ketamine for invasive procedures in children with malignancies and hematological disorders: a prospective study with reference to the sympathomimetic properties of ketamine.

Different pharmacological agents have been used for sedation in children undergoing invasive procedures. The authors prospectively evaluated the efficacy, the occurrence of adverse side effects, and cardiovascular parameters in midazolam and ketamine sedation for invasive procedures in children with malignancies and hematological disorders. A total of 183 invasive procedures were performed on 63 children (mean age 9.2 +/- 5.2 years). Intravenous sedation consisted of 0.1 mg midazolam/kg and 1.0 mg ketamine/kg. Incremental dosages of ketamine (0.33 mg/kg) were given if necessary to maintain dee

Sedation and anaesthesia in pulsed-field ablation for atrial fibrillation: mechanisms, procedural requirements, and real-world practice.

Pulsed-field ablation (PFA) has rapidly emerged as an important energy source for atrial fibrillation ablation, offering tissue selectivity and procedural efficiency while introducing specific anaesthetic and sedation requirements. This narrative review examines the rationale for anaesthesia and sedation during PFA, the mechanisms underlying pain, involuntary movement, and neuromuscular stimulation, and the implications of catheter design, waveform characteristics, and patient-related factors for peri-procedural management. Available evidence indicates that both deep sedation and general anaes

Clinical utility of the bispectral index score when compared to the University of Michigan Sedation Scale in assessing the depth of outpatient pediatric sedation.

This single blinded observational study compared the bispectral index (BIS) monitor with a validated pediatric sedation scale, the University of Michigan Sedation Scale (UMSS), to evaluate whether the BIS score can be used to judge the depth of sedation in pediatric outpatients. Thirty-eight children, with a mean age of 5.8 years, undergoing routine sedation for both noninvasive and gastrointestinal procedures, had simultaneous BIS and UMSS scores recorded. Sedation categories were defined as light, moderate, and deep for both UMSS and BIS. There was a moderate correlation between BIS and the

Prospective case series study on the use of a novel analgosedation protocol for ultrasound-guided catheterization in Neonatal Intensive Care Unit.

UNLABELLED: Critically ill newborns admitted to Neonatal Intensive Care Unit often require a centrally inserted central catheters (CICCs) inserted by ultrasound-guided puncture of the internal jugular or brachio-cephalic vein. Achieving an appropriate level of sedation and analgesia is paramount for procedure success and patient safety, avoiding the potential risks associated with excessive deep sedation. The aim of this study is to evaluate the feasibility of a novel protocol of sedation. Data from 46 patients were prospectively collected. The feasibility was assessed throughout the monitorin
