[Differential diagnosis in descending perineum syndrome].

Clinical symptoms in descending perineum syndrome show considerable variations, ranging from obstructed defecation to combined fecal and urinary incontinence and including different types of prolapse. Differential diagnosis has to compete with this complexity. Common pelvic floor disorders associated with descending perineum are rectocele, rectal prolapse, enterocele, and sigmoidocele. Standardized diagnostic tools include detailed history and clinical examination with proctorectoscopy as well as anorectal manometry, endoanal ultrasound, defecography, and dynamic MR of the pelvic floor. Transanal minimally invasive (TAMIS) mucosal resection with muscular plication for patients with obstructed defecation syndrome-A prospective pilot study.

BACKGROUND: Rectocele and intussusception are frequently observed during defecography as potential contributors to obstructed defecation syndrome (ODS). We aimed to describe our initial experience with transanal minimally invasive surgery (TAMIS) mucosectomy with muscular plication, as a novel surgical approach to treat patients with ODS. METHODS: Conducted between August 2021 and October 2023 at the Medical University of Vienna, 11 patients (8 female) were prospectively enrolled and underwent TAMIS mucosectomy with circular mucosectomy and longitudinal muscular plication (internal Delorme's

Differences in Pelvic Muscular Shape and Thickness in Women With and Without Significant Bladder Descent.

INTRODUCTION AND HYPOTHESIS: Pelvic organ prolapse (POP) is the descent of pelvic organs, causing patient discomfort. Despite its increasing prevalence, agreement in the field as to the cause is elusive. Weakness in pelvic floor muscles is implicated, but specific structural changes remain unclear. This study aimed to identify such differences in the levator ani, hypothesizing differences in pelvic muscle shape between greater and lesser bladder descent. METHODS: We reviewed our patient database for cystocele diagnoses via MR defecography, including patients with static axial MRIs and dynamic

Effect of posture on anorectal manometric measurements in female patients with fecal incontinence and rectoanal intussusception.

PURPOSE: This study aimed to investigate the influence of erect position on anorectal manometry in patients with rectoanal intussusception (RAI). METHODS: This was a single center prospective observational study. Eighty female patients with fecal incontinence (FI) who underwent defecography between 1st January 2016 and 30th April 2022 were included. The effect of posture on commonly measured parameters during manometry was assessed in the left-lateral and erect positions. The severity of FI was assessed using FI Severity Index (FISI). RESULTS: Defecography showed that 30 patients had circumf

[Anteroposterior rectopexy for disorders of rectal stasis: clinical and radiologic results. Value of digital subtraction rectography. Apropos of 30 cases].

We report the results of 30 antero-posterior rectopexies (APR) for rectal kinetic disorders with descending perineum syndrome. All patients were investigated by digital subtraction defecography and ano-rectal manometry. The associated surgical procedures were: sphincterotomy (n = 13) for outlet obstruction demonstrated by anal manometry or balloon expulsion test: hypertonic sphincter (n = 7), narrow fibrous sphincter (n = 6); 10 cases of prolapsectomy with extended anterior mucosectomy to reduce anterior rectal prolapse; 2 sigmoidectomy for dolichosigmoid. Best results (mean follow-up: 12 mont

Rectocele.

Rectocele is one of the more common pathologies of the pelvic floor. It is a clinical problem that significantly worsens the quality of life by causing obstructed defecation symptoms such as anal digitation, incomplete emptying, and dyschesia. Proper differential diagnosis, distinguishing rectocele from enterocoele or perineal body descent, allows for making proper therapeutic decisions and consists of medical history, ultrasound examination, dynamic magnetic resonance imaging (MRI), and defecography. The most common treatment procedures are vaginal native tissue repair, transanal repair, and

Assessing Anorectal Function in Constipation and Fecal Incontinence.

Constipation and fecal incontinence are commonly encountered complaints in the gastrointestinal clinic. Assessment of anorectal function includes comprehensive history, rectal examination, and prospective stool diary or electronic App diary that accurately captures bowel symptoms, evaluation of severity, and quality of life of measure. Evaluation of a suspected patient with dyssynergic constipation includes anorectal manometry, balloon expulsion test, and defecography. Investigation of a suspected patient with fecal incontinence includes high-resolution anorectal manometry; anal ultrasound or

Toward a selection of the most appropriate procedure in the treatment of complete rectal prolapse.

Defecographic evaluation was performed in 30 patients with rectal prolapse to assess the effect of posterior rectopexy on rectal function and to arrive at a selection of the best procedure. Preoperative defecography revealed rectal intussusception in all patients. Postoperative control studies showed adequate rectal fixation to the anterior sacral surface. Intussusception no longer occurred. Rectal stenosis due to the surgical procedure was absent. The described technique of posterior rectopexy eliminates the prolapse mechanism without creating new disorders and is therefore a rational procedu

Stapled mucosectomy for rectocele repair: a preliminary report.

There is no optimum surgical method of repair for rectoceles; however, recent interest in the use of the circular hemorrhoidal stapler gun to treat rectoceles has stirred interest. We describe our early results using the circular hemorrhoidal stapler gun for repair of rectoceles. Seven patients (median age, 45 (range, 31-62) years; all females) have been treated. All seven patients presented with incomplete or difficult defecation, four patients required digital vaginal manipulation, and all patients had tried a variety of aperients to aid defecation. The procedure involved two purse-strings a

Barium trapping in rectoceles: are we trapped by the wrong definition?

BACKGROUND: Barium trapping within a rectocele is a criterion used by surgeons to select which patients with rectoceles should undergo operative repair. This proctographic study compared the presence and depth of barium trapping within a rectocele on postevacuation radiography with those seen on posttoilet radiography after further evacuation in the privacy of the bathroom. METHODS: Eighty-two consecutive patients with evidence of barium trapping on postevacuation radiographs of a fluoroscopic dynamic cystoproctographic examination were reviewed retrospectively. The size of the rectoceles and

Association between rectocele and paradoxical sphincter response.

Rectocele as well as paradoxical sphincter reaction may lead to rectal emptying difficulties and outlet obstruction. Forty-one patients with emptying disturbances and rectocele were investigated with defecography, anorectal manometry, colon transit time, and electromyography. Twenty-nine patients (71 percent) had concomitant paradoxical sphincter reaction, and 13 of these also had increased colon transit time. The functional results after surgical treatment of rectocele are not always satisfactory, probably because patients often have several causes for their emptying disturbances. It is empha

[New physiological, diagnostic and therapeutic concepts in the management of rectoceles in women].

Anterior rectocele is not only a herniation of the anterior rectal wall into the vagina, but rather a complex anatomical and functional abnormality which may be isolated or associated with other pelvic floor disorders. It could result in rectal obstruction with dyschezia, manual extraction of feces, and fecal or gas incontinence. The purpose of this review is to describe and to assess the most useful methods for the diagnosis and for the treatment of the rectocele. Data from physical examination may be improved by defecography. Surgery remains the main treatment: several surgical ways (perinea

Anorectal anatomy and physiology.

The rectum and anal canal form the last portion of the gastrointestinal tract. The rectum serves as a reservoir for fecal contents, and the anal canal regulates continence and defecation via synchronization of events regulated by complex interactions between sympathetic and parasympathetic nerves, striated and smooth muscle, and environmental factors. Normal function can be compromised by various pathologies. Investigation into these pathologies includes a detailed history and thorough physical exam and can be augmented by a number of different studies, including manometry, electromyelography,

The assessment and management of defecatory dysfunction: a critical appraisal.

PURPOSE OF REVIEW: To summarize the advances in diagnostic modalities and management options for defecatory dysfunction and highlight the areas in need of further research. RECENT FINDINGS: The diagnostic utility of high-resolution anorectal manometry (ARM), which has emerged as a promising tool for the diagnosis of defecatory dysfunction, appears to be questionable in differentiating disease from normal physiology. There also seems to be discrepancy between results of various tests of anorectal function in the diagnosis of defecatory dysfunction. New revisions in diagnostic criteria for defe
